B-IMPACT project : eco-friendly and non-hazardous coatings for the protection of outdoor bronzes

Povzetek: Application of protective coatings is the most widely used conservation treatment for outdoor bronzes. Eco-friendly and non-hazardous coatings are currently needed for conservation of outdoor bronze monuments. To fulfil this need, the M-ERA.NET European research project B-IMPACT (Bronze-IMproved non-hazardous PAtina CoaTings) aimed at assessing the protectiveness of innovative coatings for historical and modern bronze monuments exposed outdoors. In this project, two bronze substrates (historical Cu-Sn-Zn-Pb and modern Cu-Si-Mn alloys) were artificially patinated, by acid rain solution using dropping test and by "liver of sulphur" procedure (K2S aqueous solution) to obtain black patina, respectively. Subsequently, the application of several newly developed protective coatings was carried out and their performance was investigated by preliminary electrochemical tests. In the following steps of the work, the assessment of the best-performing coatings was carried out and their performance was compared to Incralac, one of the most widely used protective coatings in conservation practice. A multi-analytical approach was adopted, considering artificial ageing (carried out in representative conditions, including exposure to rain runoff, stagnant rain and UV radiation) and metal release, as well as visual aspect (so as to include aesthetical impact among the coating selection parameters) and morphological and structural evolution of the coated surfaces due to simulated outdoor exposure. Lastly, also the health impact of selected coatings was assessed by occupational hazard tests. The removability and re-applicability of the best-performing coatings were also assessed. The best alternatives to the conventional Incralac exhibited were: (i) fluoroacrylate blended with methacryloxy-propyl-trimethoxy-silane (FA-MS) applied on patinated Cu-Sn-Zn-Pb bronze and (ii) 3-mercapto-propyl-trimethoxysilane (PropS-SH) applied on patinated Cu-Si-Mn bronze.

Adjuvant treatment of breast cancer patients with trastuzumab

Povzetek: Background. Trastuzumab is a monoclone antibody directed against HER2 receptors that are overexpressed in approximately 20% of breast cancer patients. The present paper presents five clinical trials in which trastuzumabwas applied in breast cancer patients in adjuvant setting. The results of all the trials consistently demonstrate a high efficacy of this target drug in the patients with HER2 positive tumours. So far, no formal guidelines for using trastuzumab in adjuvant setting for breast cancer have been approved. The reasons are many: (i) mean observation time in the studies done so far was considerably short; (ii) the drug was used according to different schedules, (iii) the overall time of treatment with trastuzumab was different in each trial, (iv) late side effects of treatment with trastuzumab are inadequately investigated, and (v) nobody can so far say for sure for which HER2 status patients therapy with trastuzumab is really beneficial. Conclusions. Trastuzumab is definitely very helpful in the treatment of the HER2-positive breast cancer patients that are hormone-independent and of anatomically larger tumours; but, what the absolute benefit of trastuzumab therapy in the treatment of small hormone-dependent tumours is remains a mystery. Incidentally, it must be borne in mind that cardiotoxicity, the well known side effect, may put particularly elderly patients at risk of death, thus beating any treatment advantages down. It has also not been yet resolved at what time it would be most appropriate to start with the therapy with trastuzumab, what would be the optimal duration of the therapy and whether trastuzumab is to be administered concurrently with chemotherapy or immediately after it? What is the optimal treatment duration, one or two yearsor only a few months? In addition there is still a question of optimal HER2 status determination and which HER2 status predicts for trastuzumab benefit. (Abstract truncated at 2000 characters)

Radiologic imaging of acute pancreatitis

Povzetek: Background. Acute pancreatitis comprises a broad spectrum of inflammatory changes in the gland and has a variety of causes. The most common are alcoholism and biliary tract disease. This paper discusses the current concepts of diagnosis and the relationship between morphology as depieted by imaging, pancreatic function tests and treatment planning. Conclusions. Our understanding of the morphologic changes produced by the inflammatory process has expanded in the last decade as more sophisticated imaging techniques have been used for evaluation.

SBA-15 mesoporous particles with adsorbed cresol red dye and functionalized with 3-aminopropyl groups : materials properties and dye release studies

Povzetek: Research in the field of sensing has focused on tailoring the responsiveness of materials through the variation in materials properties achieved via functionalization of the mesoporous matrix. pH indicator dyes capable of protonation/deprotonation reactions are known to display different forms, resulting in changes in colour. This research focused on the preparation of mesoporous substrates SBA-15 with integrated pH indicator dye o-cresolsulphonephtalein (cresol red) grafted with (3-aminopropy) trimethoxysilane (gaining functionalized-SBA-15 material) in order to gain a colour change response following release of the dye. UV-Vis reflectance and absorbance spectra were used to evaluate forms of cresol red. The textural properties, including specific surface area, pore size distribution and pore volume were evaluated using nitrogen sorption, and XRD analysis was conducted to evaluate crystallinity of the material and pore ordering. The presence of 3-aminopropyl groups was determined by thermal decomposition using TGA–DSC/MS. The functionalized SBA-15 materials loaded with cresol red mostly retained ordering of the pores and showed a slight reduction in surface area. Functionalization changed the surface properties of the material. In the non-functionalized SBA-15, cresol red was present in both double (H2L) and single (HL−) protonated forms, whereas in the functionalized SBA-15 the deprotonated L2−form prevailed. Through desorption experiments cresol red was successfully desorbed from selected sample to different media. The change of colour caused by alterations in the form of the dye once released from the mesoporous silica material into the medium was confirmed. The form of cresol red released was determined by the pH of the medium.

Renal cell carcinoma in the ectopic kidney prospects of diagnosting and treatment of the carcinoma of the kidney: case report

Povzetek: Background. An extensive use of ultrasonography and computed tomography have enormously contributed to the early detection of adenocarcinoma of kidney taking into consideration that they have been so far often detected by chance.In addition to provide us with an image of a tumor and contributing to define more easily the nature of tumor, MRI is most helpful in detecting the infiltrations of tumor into its surroundings and changes in the veins. Case report. This case report presents the patient with adenocarcinoma of the ectopic kidney. The ultrasonography, computed tomography and magnetic resonance imaging detected and abundant non-homogenous tumor mass in pelvis minor. Additionally to the suspected ectopia of one of the kidney, it was alsosuspected that there was another pathological process of different etiology too. This suspicion was based on the detection of a different tumor mass in between the intestinal loops. Two months after the nonradical excisionof the adenocarcinoma from the ectopic kidney, the following changes were observed: progress of the residual tumor in the pelvis minor and the tumor in between the intestinal loops (that was not removed during the first operation). The patient was given immunochemotherapy and local radiotherapy. Conclusions. According to the available data in literature, the localization of tumor in ectopic kidney is extremely rare.

Exploration of macromolecular phenotype of human skeletal muscle in diabetes using infrared spectroscopy

Povzetek: Introduction: The global burden of diabetes mellitus is escalating, and more efficient investigative strategies are needed for a deeper understanding of underlying pathophysiological mechanisms. The crucial role of skeletal muscle in carbohydrate and lipid metabolism makes it one of the most susceptible tissues to diabetes-related metabolic disorders. In tissue studies, conventional histochemical methods have several technical limitations and have been shown to inadequately characterise the biomolecular phenotype of skeletal muscle to provide a holistic view of the pathologically altered proportions of macromolecular constituents. Materials and methods: In this pilot study, we examined the composition of five different human skeletal muscles from male donors diagnosed with type 2 diabetes and non-diabetic controls. We analysed the lipid, glycogen, and collagen content in the muscles in a traditional manner with histochemical assays using different staining techniques. This served as a reference for comparison with the unconventional analysis of tissue composition using Fourier-transform infrared spectroscopy as an alternative methodological approach. Results: A thorough chemometric post-processing of the infrared spectra using a multi-stage spectral decomposition allowed the simultaneous identification of various compositional details from a vibrational spectrum measured in a single experiment. We obtained multifaceted information about the proportions of the different macromolecular constituents of skeletal muscle, which even allowed us to distinguish protein constituents with different structural properties. The most important methodological steps for a comprehensive insight into muscle composition have thus been set and parameters identified that can be used for the comparison between healthy and diabetic muscles. Conclusion: We have established a methodological framework based on vibrational spectroscopy for the detailed macromolecular analysis of human skeletal muscle that can effectively complement or may even serve as an alternative to histochemical assays. As this is a pilot study with relatively small sample sets, we remain cautious at this stage in drawing definitive conclusions about diabetes-related changes in skeletal muscle composition. However, the main focus and contribution of our work has been to provide an alternative, simple and efficient approach for this purpose. We are confident that we have achieved this goal and have brought our methodology to a level from which it can be successfully transferred to a large-scale study that allows the effects of diabetes on skeletal muscle composition and the interrelationships between the macromolecular tissue alterations due to diabetes to be investigated.
