Podporna in paliativna oskrba bolnika s pljučnim rakom

Povzetek: Pljučni rak je zelo pogosto odkrit šele v napredovali fazi in je zato velikokrat neozdravljiv. Bolniki s pljučnim rakom imajo sočasno tudi številna druga pridružena stanja in obolenja, ki jim znižujejo kvaliteto in dolžino življenja. Paliativna oskrba, katere namen je zagotavljati celostno in neprekinjeno podporo bolniku (in bližnjim) skozi vsa obdobja življenja z neozdravljivo boleznijo, je zato za njih ključnega pomena. Cilj je zagotavljanje čim boljše kakovosti življenja in ohranjanje dostojanstva. Izvajanje paliativne oskrbe v našem okolju je dvonivojsko – osnovna in specializirana paliativna oskrba. Izvajalci osnovne paliativne oskrbe so družinski zdravniki s svojimi timi in zdravniki v zdravstvenih institucijah z osnovnimi znanji iz paliativne oskrbe. Specializirano paliativno oskrbo zagotavljajo za to delo dodatno izobraženi timi. Za kakovostno izvajanje posamezniku prilagojene paliativne oskrbe je pomembno dobro povezovanje obstoječih struktur s področja zagotavljanja paliativne oskrbe v posamezni regiji. Pri tem ima pomembno povezovalno vlogo koordinator paliativne oskrbe, ki bolniku omogoča optimalno oskrbo v danem času in kraju glede na njegove trenutne potrebe.

In-depth study of tomato and weed viromes reveals undiscovered plant virus diversity in an agroecosystem

Povzetek: Background: In agroecosystems, viruses are well known to influence crop health and some cause phytosanitary and economic problems, but their diversity in non-crop plants and role outside the disease perspective is less known. Extensive virome explorations that include both crop and diverse weed plants are therefore needed to better understand roles of viruses in agroecosystems. Such unbiased exploration is available through viromics, which could generate biological and ecological insights from immense high-throughput sequencing (HTS) data. Results: Here, we implemented HTS-based viromics to explore viral diversity in tomatoes and weeds in farming areas at a nation-wide scale. We detected 125 viruses, including 79 novel species, wherein 65 were found exclusively in weeds. This spanned 21 higher-level plant virus taxa dominated by Potyviridae, Rhabdoviridae, and Tombusviridae, and four non-plant virus families. We detected viruses of non-plant hosts and viroid-like sequences and demonstrated infectivity of a novel tobamovirus in plants of Solanaceae family. Diversities of predominant tomato viruses were variable, in some cases, comparable to that of global isolates of the same species. We phylogenetically classified novel viruses and showed links between a subgroup of phylogenetically related rhabdoviruses to their taxonomically related host plants. Ten classified viruses detected in tomatoes were also detected in weeds, which might indicate possible role of weeds as their reservoirs and that these viruses could be exchanged between the two compartments. Conclusions: We showed that even in relatively well studied agroecosystems, such as tomato farms, a large part of very diverse plant viromes can still be unknown and is mostly present in understudied non-crop plants. The overlapping presence of viruses in tomatoes and weeds implicate possible presence of virus reservoir and possible exchange between the weed and crop compartments, which may influence weed management decisions. The observed variability and widespread presence of predominant tomato viruses and the infectivity of a novel tobamovirus in solanaceous plants, provided foundation for further investigation of virus disease dynamics and their effect on tomato health. The extensive insights we generated from such in-depth agroecosystem virome exploration will be valuable in anticipating possible emergences of plant virus diseases and would serve as baseline for further post-discovery characterization studies.

Stroop in motion : neurodynamic modulation underlying interference control while sitting, standing, and walking

Povzetek: There is conflicting evidence about how interference control in healthy adults is affected by walking as compared to standing or sitting. Although the Stroop paradigm is one of the best-studied paradigms to investigate interference control, the neurodynamics associated with the Stroop task during walking have never been studied. We investigated three Stroop tasks using variants with increasing interference levels – word-reading, ink-naming, and the switching of the two tasks, combined in a systematic dual-tasking fashion with three motor conditions – sitting, standing, and treadmill walking. Neurodynamics underlying interference control were recorded using the electroencephalogram. Worsened performance was observed for the incongruent compared to congruent trials and for the switching Stroop compared to the other two variants. The early frontocentral event-related potentials (ERPs) associated with executive functions (P2, N2) differentially signaled posture-related workloads, while the later stages of information processing indexed faster interference suppression and response selection in walking compared to static conditions. The early P2 and N2 components as well as frontocentral Theta and parietal Alpha power were sensitive to increasing workloads on the motor and cognitive systems. The distinction between the type of load (motor and cognitive) became evident only in the later posterior ERP components in which the amplitude non-uniformly reflected the relative attentional demand of a task. Our data suggest that walking might facilitate selective attention and interference control in healthy adults. Existing interpretations of ERP components recorded in stationary settings should be considered with care as they might not be directly transferable to mobile settings.
