The last sanctum of archetypes : rethinking dreams in the light of ancient knowledge and artificial intelligence

Povzetek: Despite numerous attempts to integrate dream research into a vast array of sci-entific disciplines, there appears to be no consensus on why and how we dream. This millennia-old universal human phenomenon appears to be too elusive to be thoroughly understood by a single scientific discipline and too complex and data--rich to be studied only theoretically. However, another dimension to dreams and dreaming could promise an integrative approach: the culture-historical compo-nent that merges with recent advances in artificial Intelligence. This paper briefly examines conceptual understandings of dreams before the dawn of modern science – specifically, the Native american, Mesopotamian, ancient Greek, and Hippocra-tic principles of dream practices and knowledge – in an attempt to understand the contemporary dream research field better and to outline future avenues for a data-driven approach while remaining grounded in its epistemological foundation.

Experimental investigation of the shear force capacity of prismatic cross laminated timber beams

Povzetek: Experimental tests of Cross Laminated Timber (CLT) under in-plane beam loading conditions are presented. The influence of the element layup, the individual lamination width, and the beam overhang at the supports on the shear force capacity was investigated. All the CLT beams had the same gross cross section, and a 4-point-bending test setup was used. The experimentally determined load-bearing capacities are compared with the load-bearing capacities resulting from analytical methods proposed for structural design, focusing on shear failure in the crossing areas of flatwise bonded laminations (shear failure mode III). The test results indicate no or very small influence of the element layup and the lamination width on the shear force capacity. These results partly contradict the predictions of the proposed design methods. Of the three studied beam geometry parameters, the beam overhang at the support had the greatest influence on the load-bearing capacity.

Experimental investigation of the axial strength of glued-in rods in cross laminated timber

Povzetek: This paper presents results from an experimental assessment of glued-in rods in cross laminated timber (CLT). For the purposes of the study more than 60 pull-pull tests were performed, where the specimens varied in terms of bonded-in length (from 80 to 400 mm), rod diameter (16-24 mm) and rod-to-grain angle (parallel and perpendicular). Several different failure modes that are not common for other applications of glued-in rods (e.g., a failure between CLT layers) were obtained for the analysed CLT specimens. It was found that these failure mechanisms can substantially influence the obtained ultimate tension loads. At the end, the experimental results were compared with empirical and semi-empirical equations for estimating the pull-out strength of glued-in rods in structural timber and glulam. The comparison showed that most of the existing equations overestimate the ultimate tension loads for specimens with the rod parallel to the grain and underestimate the ultimate tension load for specimens with the rod perpendicular to the grain. The results vary because the possible CLT failure modes were not included in previous studies. Further studies are proposed to improve the equations for glued-in rods in CLT.

Glued-in rods in cross laminated timber - numerical simulations and parametric studies

Povzetek: Numerical simulations and parametric studies of glued-in rods in cross-laminated timber have been performed. The simulations were based on 3D finite element analysis, using a cohesive surface model for the bond-lines between the laminations and the bond-line along the rod. The parametric studies investigated the influence of the glued-in length, the rod diameter, and the rod-to-grain angle on the load-bearing capacity and stiffness of the connection. The analyses showed that the load-bearing capacity generally increases with the glued-in length and the rod diameter, which agrees well with experiments. For different rod-to-grain angles, different mechanical behaviour was observed, especially considering the failure modes.

Brittle failure of laterally loaded self-tapping screw connections for cross-laminated timber structures

Povzetek: The performance of structural timber connections is of utmost importance since they control the global response of the building. A ductile failure mechanism on the global scale is desirable, especially in the design of structures in seismic areas, where dissipative components in which ductile failure modes need to be ensured are considered. Therefore, the knowledge of possible brittle failure modes of connections is crucial. The paper investigates the brittle failures of laterally loaded dowel-type connections in cross-laminated timber subjected to tensile load in a lap joint configuration through experimental investigations and analytical estimations. A set of 13 different test series has been performed with fully threaded self-tapping screws of 8 mm diameter and different lengths (40 to 100 mm) in cross-laminated timber composed of 3 or 5 layers (layer thickness range from 20 to 40 mm), giving rise to the activation of different brittle failure modes at different depths. Plug shear was among the most typically observed failure modes. A previously proposed model for the brittle capacity was applied to the tested connections at the characteristic level. As shown by the performed statistical analysis, the existing model is not reliable and mainly unconservative. A very low performance is observed (CCC = 0.299), but with a good correlation (c = 0.750) for the tests in the parallel direction. Further research work is required to improve the current model predictions and to gain a better understanding of the underlying resisting mechanisms.

Decontamination strategies and bloodstream infections with antibiotic-resistant microorganisms in ventilated patients : a randomized clinical trial

Povzetek: Importance: The effects of chlorhexidine (CHX) mouthwash, selective oropharyngeal decontamination (SOD), and selective digestive tract decontamination (SDD) on patient outcomes in ICUs with moderate to high levels of antibiotic resistance are unknown. Objective: To determine associations between CHX 2%, SOD, and SDD and the occurrence of ICU-acquired bloodstream infections with multidrug-resistant gram-negative bacteria (MDRGNB) and 28-day mortality in ICUs with moderate to high levels of antibiotic resistance. Design, setting, and participants: Randomized trial conducted from December 1, 2013, to May 31, 2017, in 13 European ICUs where at least 5% of bloodstream infections are caused by extended-spectrum [beta]-lactamase-producing Enterobacteriaceae. Patients with anticipated mechanical ventilation of more than 24 hours were eligible. The final date of follow-up was September 20, 2017. Interventions: Standard care was daily CHX 2% body washings and a hand hygiene improvement program. Following a baseline period from 6 to 14 months, each ICU was assigned in random order to 3 separate 6-month intervention periods with either CHX 2% mouthwash, SOD (mouthpaste with colistin, tobramycin, and nystatin), or SDD (the same mouthpaste and gastrointestinal suspension with the same antibiotics), all applied 4 times daily. Main outcomes and measures: The occurrence of ICU-acquired bloodstream infection with MDRGNB (primary outcome) and 28-day mortality (secondary outcome) during each intervention period compared with the baseline period. Results: A total of 8665 patients (median age, 64.1 years; 5561 men [64.2%]) were included in the study (2251, 2108, 2224, and 2082 in the baseline, CHX, SOD, and SDD periods, respectively). ICU-acquired bloodstream infection with MDRGNB occurred among 144 patients (154 episodes) in 2.1%, 1.8%, 1.5%, and 1.2% of included patients during the baseline, CHX, SOD, and SDD periods, respectively. Absolute risk reductions were 0.3% (95% CI, -0.6% to 1.1%), 0.6% (95% CI, -0.2% to 1.4%), and 0.8% (95% CI, 0.1% to 1.6%) for CHX, SOD, and SDD, respectively, compared with baseline. Adjusted hazard ratios were 1.13 (95% CI, 0.68-1.88), 0.89 (95% CI, 0.55-1.45), and 0.70 (95% CI, 0.43-1.14) during the CHX, SOD, and SDD periods, respectively, vs baseline. Crude mortality risks on day 28 were 31.9%, 32.9%, 32.4%, and 34.1% during the baseline, CHX, SOD, and SDD periods, respectively. Adjusted odds ratios for 28-day mortality were 1.07 (95% CI, 0.86-1.32), 1.05 (95% CI, 0.85-1.29), and 1.03 (95% CI, 0.80-1.32) for CHX, SOD, and SDD, respectively, vs baseline. Conclusions and relevance: Among patients receiving mechanical ventilation in ICUs with moderate to high antibiotic resistance prevalence, use of CHX mouthwash, SOD, or SDD was not associated with reductions in ICU-acquired bloodstream infections caused by MDRGNB compared with standard care.

Proteases and cytokines as mediators of interactions between cancer and stromal cells in tumours

Povzetek: Proteolytic enzymes are highly relevant in different processes of cancer progression. Their interplay with other signalling molecules such as cytokines represents important regulation of multicellular cross-talk. In this review, we discuss protease regulation mechanisms of cytokine signalling in various types of cancer. Additionally, we highlight the reverse whereby cytokines have an impact on protease expression in an autocrine and paracrine manner, representing complex feedback mechanisms among multiple members of these two protein families. The relevance of the protease-cytokine axis is illustrated in glioblastoma, where interactions between normal mesenchymal stem cells and cancer cells play an important role in this very malignant form of brain cancer.
