Comparison of the field-based intermittent running fitness test 30-15 and the treadmill multistage incremental test for the assessment of cardiorespiratory fitness in elite handball players

Povzetek: The aim of the present study was to investigate whether the physiological parameters indicative of cardiorespiratory fitness obtained during the 30-15 intermittent fitness (30-15IFT) test and the multistage laboratory treadmill endurance (TR) test differ. Nineteen elite handball players were recruited for the current study and assigned in a cross-over manner to one of two tests to be performed 48 h apart at each visit to the testing facility. The results showed that VO2max (percentage difference [PC] = 6.1%; p = 0.004) and maximal running velocity (V) (PC = 19.4%; p < 0.001) were significantly higher for the 30-15IFT test than that obtained during the TR test. Furthermore, the onset of blood lactate accumulation was shown to be significantly higher for all measures considered to predict it during 30-15IFT compared to TR as follows: VO2max (PC = 12.6%; p = 0.001), running speed (PC = 33.9%; p < 0.001), and maximal heart rate (PC = 7.5%; p < 0.001). The current study highlights the importance of sport-specific testing, particularly for measuring individual cardiorespiratory fitness in elite handball players, as TR may underestimate crucial variables used for both diagnostics and training prescription.

Phenological variability and resistance to late spring frost of common beech in the international provenance test in Bosnia and Herzegovina

Povzetek: The research aimed to determine the start, end and duration of leafing phases, the degree of plant damage caused by late spring frost, and the retention of leaves in autumn. The research was conducted in the international common beech provenance test in Bosnia and Herzegovina. The test contains eight provenances from Bosnia and Herzegovina, four from Germany, three from Serbia, two each from Croatia, Romania and Switzerland, and one from Hungary. Leafing phenology, canopy damage caused by late spring frost, and leaf retention were assessed in 2019. Most provenances started opening buds on 17 April 2019. Complete canopy damage affected the highest percentage of plants in provenance BW Bad Wildbad, Germany (68 %), and the lowest in provenance Konjuh Kladanj, Bosnia and Herzegovina (3.2 %). Provenance Herzogenbuchsee from Switzerland had the highest percentage of winter leaf retention (37.5 % of plants). The results can be used in choosing provenances that are resistant to low temperatures in zones of late spring and early autumn frost.

Quantification of the link between timed up-and-go test subtasks and contractile muscle properties

Povzetek: Frailty and falls are a major public health problem in older adults. Muscle weakness of the lower and upper extremities are risk factors for any, as well as recurrent falls including injuries and fractures. While the Timed Up-and-Go (TUG) test is often used to identify frail members and fallers, tensiomyography (TMG) can be used as a non-invasive tool to assess the function of skeletal muscles. In a clinical study, we evaluated the correlation between the TMG parameters of the skeletal muscle contraction of 23 elderly participants (22 f, age 86.74 % 7.88) and distance-based TUG test subtask times. TUG tests were recorded with an ultrasonic-based device. The sit-up and walking phases were significantly correlated to the contraction and delay time of the muscle vastus medialis (% = 0.55%0.80, p < 0.01). In addition, the delay time of the muscles vastus medialis (% = 0.45, p = 0.03) and gastrocnemius medialis (% = %0.44, p = 0.04) correlated to the sit-down phase. The maximal radial displacements of the biceps femoris showed significant correlations with the walk-forward times (% = %0.47, p = 0.021) and back (% = %0.43, p = 0.04). The association of TUG subtasks to muscle contractile parameters, therefore, could be utilized as a measure to improve the monitoring of elderly people%s physical ability in general and during rehabilitation after a fall in particular. TUG test subtask measurements may be used as a proxy to monitor muscle properties in rehabilitation after long hospital stays and injuries or for fall prevention.

Surprising drought tolerance of Fir (Abies) species between past climatic adaptation and future projections reveals new chances for adaptive forest management

Povzetek: esearch Highlights: Data of advanced-age provenance tests were reanalyzed applying a new approach, to directly estimate the growth of populations at their original sites under individually generated future climates. The results revealed the high resilience potential of fir species. Background and Objectives: The growth and survival of silver fir under future climatic scenarios are insufficiently investigated at the xeric limits. The selective signature of past climate determining the current and projected growth was investigated to analyze the prospects of adaptive silviculture and assisted transfer of silver fir populations, and the introduction of non-autochthonous species. Materials and Methods: Hargreaves% climatic moisture deficit was selected to model height responses of adult populations. Climatic transfer distance was used to assess the relative drought stress of populations at the test site, relating these to the past conditions to which the populations had adapted. ClimateEU and ClimateWNA pathway RCP8.5 data served to determine individually past, current, and future moisture deficit conditions. Besides silver fir, other fir species from South Europe and the American Northwest were also tested. Results: Drought tolerance profiles explained the responses of transferred provenances and predicted their future performance and survival. Silver fir displayed significant within-species differentiation regarding drought stress response. Applying the assumed drought tolerance limit of 100 mm relative moisture deficit, most of the tested silver fir populations seem to survive their projected climate at their origin until the end of the century. Survival is likely also for transferred Balkan fir species and for grand fir populations, but not for the Mediterranean species. Conclusions: The projections are less dramatic than provided by usual inventory assessments, considering also the resilience of populations. The method fills the existing gap between experimentally determined adaptive response and the predictions needed for management decisions. It also underscores the unique potential of provenance tests.

Usefulness of rapid antigen testing for SARS-CoV-2 screening of healthcare workers : ǂa ǂpilot study

Povzetek: Background. Identification of infected healthcare workers (HCWs) is an important step in severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) transmission control. Rapid antigen tests (RATs) are considered an important addition to molecular tests in diagnosing coronavirus disease 2019 (COVID-19), mainly because of their fast turnaround time, easier analytical procedure and lower price. However, real-life studies on the usefulness of such testing for screening of HCWs are limited. Methods. Physicians, nurses and hospital attendants currently working at the University Clinic of Respiratory and Allergic Diseases Golnik were invited to participate in the pilot study. Nasopharyngeal swabs were obtained three times per week for two consecutive weeks and tested with a point-of-care RAT and reverse transcription polymerase chain reaction (RT-PCR). Serum samples were obtained at the beginning of the study and 2 weeks after the last swab was collected to evaluate the serological status. Results. A total of 191 nasopharyngeal swabs from 36 HCWs were obtained. None of the samples tested was positive for the presence of SARS-CoV-2 antigen, whereas two HCWs tested positive on RT-PCR. Of these, one HCW had a newly identified SARS-CoV-2 infection, whereas RT-PCR probably detected a previous but recent infection in the other HCW. Conclusio.n Based on the results of this pilot study, it is unlikely that RAT will reliably detect novel SARS-CoV-2 infections among asymptomatic HCWs despite serial sampling. Although RT-PCR-based screening of HCWs may not be feasible due to high sample volume, molecular methods may identify SARS-CoV-2-infected HCWs already during the presymptomatic stage.

Drought stress can induce the pathogenicity of Cryptostroma corticale, the causal agent of sooty bark disease of sycamore maple

Povzetek: Reports of sooty bark disease of maples caused by the fungus Cryptostroma corticale have recently been emerging from across Europe. The aims of our study were to describe the first report of sooty bark disease in Slovenia, to determine the pathogenicity of C. corticale, to confirm the optimum temperature for the growth of the fungus, and to determine the mass loss of Acer pseudoplatanus wood inoculated by C. corticale. We confirmed the presence of C. corticale on A. pseudoplatanus via morphological and molecular analysis. The optimal growth of C. corticale was measured in vitro on potato dextrose agar and was determined to occur at 25 °C. Pathogenicity tests were performed on 30 saplings of A. pseudoplatanus under two treatments, humid and drought stress, and the fungus was pathogenic in both treatments. The mean length of bark lesions and wood discoloration of the drought-stressed saplings was significantly greater than that in the humid treatment. Re-isolations of C. corticale were successful from all inoculated saplings, and thus Koch%s postulates were confirmed. The mass loss of A. pseudoplatanus wood was determined by mini-block test in a period of 10 weeks and was observed as minimal. Based on the results, we conclude that C. corticale is a weak and opportunistic pathogen that most likely expresses itself intensively under hot and dry conditions.

Mast cell activation test in the diagnosis of allergic disease and anaphylaxis

Povzetek: Background. Food allergy is an increasing public health issue and the commonest cause of life-threatening anaphylactic reactions. Conventional allergy tests assess for the presence of allergen-specific IgE, significantly overestimating the rate of true clinical allergy resulting in over-diagnosis and adverse impact on health-related quality of life. Objective. To undertake initial validation and assessment of a novel diagnostic tool, the mast cell activation test (MAT). Methods. Primary human mast cells (hMCs) were generated from peripheral blood precursors, and sensitized using patient sera and then incubated with allergen. Mast cell degranulation was assessed by flow cytometry and mediator release. We compared the diagnostic performance of MAT to existing diagnostic tools to assess in a cohort of peanut-sensitized individuals undergoing double-blind, placebo-controlled challenge. Results. hMCs sensitized with sera from peanut, grass pollen and hymenoptera- (wasp venom) allergic patients demonstrated allergen-specific and dose-dependent degranulation by both expression of surface activation markers (CD63 and CD107a) and functional assays (prostaglandins D2 and ß-hexosaminidase release). In this cohort of peanut-sensitized individuals, MAT was found to have superior discrimination performance compared to other testing modalities including component-resolved diagnostics and basophil activation test. Using functional principle component analysis, we identified 5 clusters or patterns of reactivity in the resulting dose-response curves, which at preliminary analysis corresponded to the reaction phenotypes seen at challenge. Conclusion. MAT is a robust tool which may confer superior diagnostic performance compared to existing allergy diagnostics, and may be useful to explore differences in effector cell function between basophils and mast cells during allergic reactions.

The use of drug provocation testing in the investigation of suspected immediate perioperative allergic reactions : current status

Povzetek: Suspected perioperative allergic reactions are often severe. To avoid potentially life-threatening re-exposure to the culprit drug, establishing a firm diagnosis and identifying the culprit is crucial. Drug provocation tests are considered the gold standard in drug allergy investigation but have not been recommended in the investigation of perioperative allergy, mainly because of the pharmacological effects of drugs such as induction agents and neuromuscular blocking agents. Some specialised centres have reported benefits of provocation testing in perioperative allergy investigation, but the literature on the subject is limited. Here we provide a status update on the use of drug provocation testing in perioperative allergy, including its use in specific drug groups. This review is based on a literature search and experiences of the authors comprising anaesthesiologists and allergists with experience in perioperative allergy investigation. In addition, 19 participating centres in the International Suspected Perioperative Allergic Reaction Group were surveyed on the use of provocation testing in perioperative allergy investigation. A response was received from 13 centres in eight European countries, New Zealand, and the USA. Also, 21 centres from the Australian and New Zealand Anaesthetic Allergy Group were surveyed. Two centres performed provocation routinely and seven centres performed no provocations at all. Nearly half of the centres reported performing provocations with induction agents and neuromuscular blocking agents. Drug provocation testing is being used in perioperative allergy investigation in specialised centres, but collaborations between relevant specialties and multicentre studies are necessary to determine indications and establish common testing protocols.
