Natural history of the hymenoptera venom sensitivity reactions in adults : study design

Povzetek: Background: Allergic reactions to Hymenoptera stings can have varying levels of severity, according to the Müller grading system. Methods: By an epidemiological concept, this is a retrospective cohort study. The observed cohort was represented by patients referred to the University Clinic Golnik due to Hymenoptera allergic reaction in the period from 1997 to 2015. From the immunological database of the University Clinic Golnik, we obtained laboratory data (sIgE, skin tests and basophil activation test). The clinical characteristics of patients were obtained from BIRPIS. With the help of a questionnaire, which was sent to each patient in the period from May 2019 to April 2021, we obtained epidemiological data. For the assessment of the association between the severity of allergic reaction for the observed outcome, the severity of the first allergic reaction after Hymenoptera sting was used. Other variables were grouped according to risk factors. Discussion: We will identify the risk factors that could play an important role in a severe systemic reaction: the aetiology of the Hymenoptera sting, sex, age, history and severity of previous systemic reactions, being re-stung in an interval of two months, the frequency of re-stings, atopy, genetic predisposition, preventive medication use, other medication use, beekeeping or living next to beehives and why immunotherapy was not taken. Laboratory data will also be analysed to determine if there is any association with laboratory tests and the severity of the allergic reactions after Hymenoptera stings. Conclusions: Several new approaches are introduced in the study design. The most important is that the protocol covers epidemiological data gained from the questionnaire, as well as clinical data gained from the Immunological database and BIRPIS database. We expect to obtain significant results that will explain the risk factors for the natural history of Hymenoptera sting allergic reactions and will help allergologists, as well as general doctors, when facing those patients allergic to Hymenoptera venom without immunotherapy.

Characterizing the groundwater flow regime in a landslide recharge area using stable isotopes: a case study of the Urbas landslide area in NW Slovenia

Povzetek: Slope stability strongly depends on the prevailing hydrological and hydrogeological conditions. The amount and intensity of precipitation and changing groundwater levels are important landslide triggering factors. Environmental tracers, including the chemical and stable isotope compositions of precipitation and groundwater, were used to gain insight into the groundwater dynamics of the Urbas landslide. The landslide is situated in a mountainous area with steep slopes and high precipitation amount and poses a high risk for the safety of the Koroška Bela settlement that lies downstream. The stable isotope analyses of oxygen-18 (18O) and deuterium (2H) in the precipitation and groundwater were used to estimate the groundwater mean residence time and the average altitude of the landslide recharge area. This information will help to plan and prioritize remedial landslide measures aiming to reduce the recharge of the landslide body and, thus, lower the risk of transformation of the sliding material into debris flow. The results of the chemical analysis of samples taken from springs and a piezometer show a Ca–HCO3 water type. This indicates low water–rock interaction in a landslide area composed of Upper Carboniferous and Permian clastic rocks and points to upper laying carbonate rocks and scree deposits as the main recharge area. Water samples for stable isotope analyses of δ18O and δ2H were collected from a rain gauge, springs, and a piezometer over a two-year period (2018–2020). The estimated mean recharge altitude of the groundwater at sampling points was from approximately 1700 to 1800 m a.s.l. with a mean residence time of 2–5 months.

Allergies and COVID-19 vaccines : an ENDA/EAACI position paper

Povzetek: Anaphylaxis, which is rare, has been reported after COVID 19 vaccination, but its management is not standardized. Method. Members of the European Network for Drug Allergy and the European Academy of Allergy and Clinical Immunology interested in drug allergy participated in an online questionnaire on pre-vaccination screening and management of allergic reactions to COVID-19 vaccines, and literature was analysed. Results. No death due to anaphylaxis to COVID-19 vaccines has been confirmed in scientific literature. Potential allergens, polyethylene glycol (PEG), polysorbate and tromethamine, are excipients. The authors propose allergy evaluation of persons with the following histories: 1- anaphylaxis to injectable drug or vaccine containing PEG or derivatives; 2- anaphylaxis to oral/topical PEG containing products; 3-recurrent anaphylaxis of unknown cause; 4-suspected or confirmed allergy to any mRNA vaccine, 5-confirmed allergy to PEG or derivatives. We recommend a prick-to-prick skin test with the left over solution in the suspected vaccine vial to avoid waste. Prick test panel should include PEG 4000 or 3500, PEG 2000 and polysorbate 80. The value of in vitro test is arguable. Conclusions. These recommendations will lead to a better knowledge of the management and mechanisms involved in anaphylaxis to COVID-19 vaccines and enable more people with history of allergy to be vaccinated.

Empirical vs. light-use efficiency modelling for estimating carbon fluxes in a mid-succession ecosystem developed on abandoned karst grassland

Povzetek: Karst systems represent an important carbon sink worldwide. However, several phenomena such as the CO2 degassing and the exchange of cave air return a considerable amount of CO2 to the atmosphere. It is therefore of paramount importance to understand the contribution of the ecosystem to the carbon budget of karst areas. In this study conducted in a mid-succession ecosystem developed on abandoned karst grassland, two types of model were assessed, estimating the gross primary production (GPP) or the net ecosystem exchange (NEE) based on seven years of eddy covariance data (2013%2019): (1) a quadratic vegetation index-based empirical model with five alternative vegetation indices as proxies of GPP and NEE, and (2) the vegetation photosynthesis model (VPM) which is a light use efficiency model to estimate only GPP. The Enhanced Vegetation Index (EVI) was the best proxy for NEE whereas SAVI performed very similarly to EVI in the case of GPP in the empirical model setting. The empirical model performed better than the VPM model which tended to underestimate GPP. Therefore, for this ecosystem, we suggest the use of the empirical model provided that the quadratic relationship observed persists. However, the VPM model would be a good alternative under a changing climate, as it is rooted in the understanding of the photosynthesis process, if the scalars it involves could be improved to better estimate GPP.
