Prognostic value of some tumor markers in unresectable stage IV oropharyngeal carcinoma patients treated with concomitant radiochemotherapy

Povzetek: The aim of the study was to investigate how the expression of tumor markers p21, p27, p53, cyclin D1, EGFR, Ki-67, and CD31 influenced the outcome of advanced inoperable oropharyngeal carcinoma patients, treated with concomitant radiochemotherapy. Patients and methods. The pretreatment biopsy specimens of 74 consecutive patients with inoperable stage IV oropharyngeal squamous cell carcinoma treated with concomitant radiochemotherapy were in retrospective study processed by immunochemistry for p21, p27, p53, cyclin D1, EGFR, Ki-67, and CD31. Disease-free survival (DFS) was assessed according to the expression of tumor markers. Results. Patients with a high expression of p21 (%10%), p27 (>50%), Ki-67 (>50%), CD31 (>130 vessels/mm2) and low expression of p53 (<10%), cyclin D1 (<10%) and EGFR (<10%) (favorable levels - FL) had better DFS than patients with a low expression of p21 (<10%), p27 (%50%), Ki-67 (%50%), CD31 (<130 vessels/mm2) and high expression of p53 (%10%), cyclin D1 (%10%) and EGFR (%10%) (unfavorable levels - UL). However, statistical significance in survival between FL and UL was achieved only for p27 and cyclin D1. DFS significantly decreased with an increasing number of markers with an unfavorable level per tumor (1%4 vs. 5%7) (78% vs. 32%, respectively; p = 0.004). The number of markers per tumor with UL of expression retained prognostic significance also in multivariate analysis. Conclusions. Statistical significance in survival between FL and UL emerged only for p27 and cyclin D1. The number of markers per tumor with UL of expression was an independent prognostic factor for an adverse outcome .

Effectiveness of adjuvant trastuzumab in daily clinical practice

Povzetek: Background. Human epidermal growth factor receptor 2 (HER2) positive breast cancer is an entity with aggressive behaviour. One year of adjuvant trastuzumab significantly improves the disease free survival in the range of 40-50% and reduces the risk of dying from HER2 positive breast cancer by one third. Adjuvant treatment with trastuzumab became available in Slovenia in 2005 and the aim of this study is to explore, if the exceptional results reported in adjuvant clinical trials are achieved also in daily clinical practice. Patients and methods. An analysis of tumour and patient characteristics, type of treatment and outcome (relapse free and overall survival) of 313 patients (median age 52 years) treated at the Institute of Oncology Ljubljana in years 2005-2009 was performed. Results. Median follow-up was 4.4 years. Sixty-one patients relapsed and 24 died. Three and four years relapse free survival was 84.2% and 80.8% and the overall survival was 94.4% and 92.5%, respectively. Independent prognostic factors for relapse were tumour grade (HR 2.10; 95% CI 1.07-4.14; p = 0.031) and nodal stage (HR 1.35; 1.16-1.56; p < 0.0001) and for the overall survival nodal stage only (HR 1.36; 1.05-1.78; p = 0.021). Conclusions. The outcome in patients with adjuvant trastuzumab in daily clinical practice, treated by medical oncologists, is comparable to results obtained in international adjuvant studies.

Diketopyrrolopyrrole pigment core@multi-layer ▫$SiO_2$▫ shell with improved photochemical stability

Povzetek: A model organic pigment (diketopyrrolopyrrole) is encapsulated by a silica coating in order to improve its photochemical stability. Unlike in previous works where single-layer silica coatings were used for similar purposes, we here propose creation of a multi-layer silica shell synthesized via in-situ sol-gel method, which should significantly improve the homogeneity and thus the protection ability of the coating. This is done by repeating the basic two-step process (pigment surface modification and silica encapsulation) several times, creating the final protective shell in a layer-by-layer fashion. The compositional and structural properties of the prepared coatings is studied using Fourier-transform infrared spectroscopy, differential thermal and thermogravimetric analysis, nitrogen adsorption measurements and transmission electron microscopy. Photochemical stability of non-encapsulated and encapsulated pigment particles is evaluated via the so-called fast-irradiation method. Various correlations between the essential coating properties, such as thickness, porosity, SiO2 content, and the corresponding photochemical stability of the samples are established and discussed in detail. As a whole, our results confirm the basic hypothesis, that multiple-layered silica shells show improved pigment protection ability in comparison to the single-layer coatings.

Importance of habitat context in modelling risk maps for two established invasive alien plant species : the case of Ailanthus altissima and Phytolacca americana in Slovenia (Europe)

Povzetek: Forests are important ecosystems that face threats from climate change and global environmental shifts, with invasive alien plant species being a significant concern. Some of these invasive species have already become established, while others are in the process of naturalisation. Although forests are a relatively stable ecosystem, extreme weather events increase their vulnerability to change, and clearings left after natural disturbances are particularly susceptible to invasion by alien plant species (IAPS). We created risk maps of two species that have spread rapidly in the last decade: American pokeweed (Phytolacca americana) and the tree of heaven (Ailanthus altissima). We prepared a generalised linear model based on the occurrence data collected within the LIFE ARTEMIS project. Eleven environmental variables were used to determine habitat characteristics. We constructed two models for each species: one covering the entirety of Slovenia and the other specifically for the forested areas in Slovenia, with the latter incorporating forest-specific variables (such as forest sanitation felling and monocultures). We observed the presence of both species at lower altitudes and in close proximity to water sources. American pokeweed tends to occur nearer to railways, while the presence of the tree of heaven is associated with areas lacking carbonate parent material and influenced by land use patterns. In forested areas, the occurrence of American pokeweed is influenced by forest habitat characteristics, such as disturbances caused by extreme weather events or the prevalence of Norway spruce monocultures. In contrast, the occurrence of the tree of heaven is influenced by more general environmental variables, such as altitude and proximity to railways. Consequently, we have generated risk maps for the entirety of Slovenia and separately for forested areas, both of which indicate similar levels of risk, particularly for the tree of heaven. The risk map for American pokeweed highlights numerous vulnerable areas, especially forest edges, which are highly susceptible to invasion. Furthermore, there is a higher likelihood of this species occurring in areas that have undergone sanitation felling. This study suggests that the production of risk maps of IAPS could be improved by focussing on habitat types and taking into account habitat-specific variables. This approach could enhance the early detection and management of these invasive species.

Triple negative breast cancer : prognostic factors and survival

Povzetek: Background. Triple negative breast cancer (TNBC) is defined by a lack of expression of both estrogen (ER) and progesteron(PgR) receptors as well as human epidermal growth factor receptor 2 (HER2). Our retrospective analysis addressed prognostic factors for short- and long-term outcomes of patients (pts) with TNBC pts treated in routine clinical practice. Patient and methods.Our retrospective study included 269 TNBC treated at Institute of Oncology Ljubljana between March 2000 and December 2006. The collected data included patientsć, tumoursć and treatmentsć characteristics. The survival analyses were performed using the Kaplan-Meier method. The Cox proportional hazard model was used in the multivariate analysis. Results. The median age ofour patients was 55.3 yrs (23-88.5) and the median follow-up was 5.9 yrs (0.3-9.6). Six (2%) pts experienced local only, 79 (92%) pts distal recurrenceand 66 (24%) died. The predominant localisation of the first relapsewas in visceral organs (70.4%). The 5-year disease-free survival (DFS) for the entire group was 68.2% and the 5-year overall survival (OS) was 74.5%.We found a pattern of high recurrence rate in the first 3 years following the diagnosis and a clear decline in recurrence rate over the next 3years. In the univariate analysis age, nodal status, size and lymphovascular invasion (LVI) were found to have a significant impact on DFS as well as on OS. In the multivariate analysis only age (HR=1.79; 95%CI=1.14-2.82; p=0.012) and nodal status (HR=2.71; 95%CI=1.64-4.46; p<0.001) retained their independent prognostic value for DFS and for OS only the nodal status (HR=2.96; 95%CI=1.51-5.82; p=0.002).
