Percutaneous image guided electrochemotherapy of hepatocellular carcinoma : technological advancement

Povzetek: Background. Electrochemotherapy is an effective treatment of colorectal liver metastases and hepatocellular carcinoma (HCC) during open surgery. The minimally invasive percutaneous approach of electrochemotherapy has already been performed but not on HCC. The aim of this study was to demonstrate the feasibility, safety and effectiveness of electrochemotherapy with percutaneous approach on HCC. Patient and methods. The patient had undergone the transarterial chemoembolization and microwave ablation of multifocal HCC in segments III, V and VI. In follow-up a new lesion was identified in segment III, and recognized by multidisciplinary team to be suitable for minimally invasive percutaneous electrochemotherapy. The treatment was performed with long needle electrodes inserted by the aid of image guidance. Results. The insertion of electrodes was feasible, and the treatment proved safe and effective, as demonstrated by control magnetic resonance imaging. Conclusions. Minimally invasive, image guided percutaneous electrochemotherapy is feasible, safe and effective in treatment of HCC.

Leafhopper males compensate for unclear directional cues in vibration-mediated mate localization

Povzetek: Ambient noise and transmission properties of the substrate pose challenges in vibrational signal-mediated mating behavior of arthropods, because vibrational signal production is energetically demanding. We explored implications of these challenges in the leafhopper Aphrodes makarovi (Insecta: Hemiptera: Cicadellidae) by exposing males to various kinds of vibrational noise on a natural substrate and challenging them to find the source of the female playback. Contrary to expectations, males exposed to noise were at least as efficient as control males on account of similar searching success with less signaling effort, while playing back male–female duets allowed the males to switch to satellite behavior and locate the target without signaling, as expected. We found altered mitochondrial structure in males with high signaling effort that likely indicate early damaging processes at the cellular level in tymbal muscle, but no relation between biochemical markers of oxidative stress and signaling effort. Analysis of signal transmission revealed ambiguous amplitude gradients, which might explain relatively low searching success, but it also indicates the existence of behavioral adaptations to complex vibrational environments. We conclude that the observed searching tactic, emphasizing speed rather than thorough evaluation of directional cues, may compensate for unclear stimuli when the target is near.

Explainable anomaly detection of 12-Lead ECG signals using denoising autoencoder

Povzetek: The basic idea was to create a proof of concept demonstrating that the indoor navigation and localisation is possible using only passive tags. For this purpose, a smartphone navigation app was developed to be used in inner parts of the buildings and which can operate with no satellite positioning service available and no communication network present. The satellite navigation systems (GPS, GLONASS, Galileo, BeiDou, QZSS, IRNSS) inside buildings, at least on lower floors and cellars, in central parts, and away from windows, usually do not work, or their accuracy is very reduced due to a small number of visible satellites. The communication networks (WiFi, LTE, 5G…) might not be available in some circumstances such as catastrophic incidents, power reductions, or similar, which disables the localisation systems based on them. The purpose of the smartphone app was twofold. First, to develop a reliable, simple-to-use, and cheap indoor navigation system that could be used in large buildings like hospitals, shopping malls, trade centres, fairs, etc. where no other positioning service is available. Second, to develop an indoor position reporting system that can be used in accidents and mass-casualty incidents for reporting triage decisions to the server. Both functionalities are based on the QR codes [1] holding all the information needed.

Kinetics of surface wettability of aromatic polymers (PET, PS, PEEK, and PPS) upon treatment with neutral oxygen atoms from non-equilibrium oxygen plasma

Povzetek: The wettability of polymers is usually inadequate to ensure the appropriate spreading of polar liquids and thus enable the required adhesion of coatings. A standard ecologically benign method for increasing the polymer wettability is a brief treatment with a non-equilibrium plasma rich in reactive oxygen species and predominantly neutral oxygen atoms in the ground electronic state. The evolution of the surface wettability of selected aromatic polymers was investigated by water droplet contact angles deposited immediately after exposing polymer samples to fluxes of oxygen atoms between 3 × 1020 and 1 × 1023 m−2 s −1 . The treatment time varied between 0.01 and 1000 s. The wettability evolution versus the O-atom fluence for all aromatic polymers followed similar behavior regardless of the flux of O atoms or the type of polymer. In the range of fluences between approximately 5 × 1020 and 5 × 1023 m−2 , the water contact angle decreased exponentially with increasing fluence and dropped to 1/e of the initial value after receiving the fluence close to 5 × 1022 m−2

Nutrition of patients with severe neurologic impairment

Povzetek: Background. Commercial enteral formulas are generally recommended for gastrostomy feeding in patients with severe neurologic impairment. However, pureed food diets are still widely used and even gaining popularity among certain groups. We tried to compare the effectiveness of gastrostomy feeding for treatment of severe malnutrition with either enteral formulas or pureed feeds. Patients and methods. A 6-month nutritional intervention was made with 37 malnourished children, adolescents and young adults (2%26 years old) with severe neurologic impairment (Gross Motor Function Classification system [GMFCS] grade V). The individual needs were calculated. Participants were fed by gastrostomy with either enteral formulas (n = 17) or pureed food (n = 20). Measurements to assess nutritional status were made at the beginning and at the end of intervention. Results. The Z scores for weight-for-age and for the body-mass index increased more in enteral formula than in pureed food group (2.07 vs. 0.70, p = 0.0012; and 3.75 vs. 0.63, p = 0.0014, respectively). Fat mass index increased more in enteral formula than in pureed food group (1.12 kg/m2 vs. 0.38 kg/m2; p = 0.0012). Patients in the enteral formula group showed increase in lean body mass expressed as fat-free mass index (0.70 kg/m2), while those in pureed food group did not (-0.06 kg/m2) (p = 0.0487). Conclusions. The results suggest that even professionally planned pureed food diet is less effective than commercial enteral formula for nutritional rehabilitation of malnourished patients with severe neurologic impairment. However, larger and if possible randomised clinical studies should be made to confirm our findings.
