Title:Pregled zdravilnih učinkovin za zdravljenje tuberkuloze

Abstract:Tuberkuloza, ki jo povzroča mikobakterija Mycobacterium tuberculosis, ostaja vodilni vzrok smrti zaradi nalezljivih bolezni kljub številnim protituberkuloznim učinkovinam, ki so danes na trgu. Pojav večkratno odpornih in izredno odpornih sevov je močno zmanjšal učinkovitost standardnih režimov zdravljenja prvega izbora, ki temeljijo na izoniazidu, rifampicinu, pirazinamidu in etambutolu. V tem preglednem članku so povzeti mehanizmi delovanja obstoječih in novih protituberkuloznih zdravilnih učinkovin ter poudarjen pomen razvoja odpornosti in potrebe po novih terapijah. Nove učinkovine – bedakilin, delamanid in pretomanid – predstavljajo pomemben napredek, saj z novimi mehanizmi delovanja ciljajo energijski metabolizem in delujejo komplementarno že obstoječi terapiji, hkrati pa s ciljanjem novih tarč še dodatno vplivajo na sintezo mikobakterijske celične stene. Kljub temu se že pojavljajo primeri odpornosti, kar poudarja nujnost ustrezne kombinirane terapije, spremljanja bolnikov in nadaljnjega razvoja učinkovin. Kljub navedenim izzivom trenutne raziskave ponujajo nove možnosti za krajše zdravljenje, boljše izide in približevanje ciljem Svetovne zdravstvene organizacije glede zmanjševanja incidence in števila smrtnih izidov.

Title:Overview of active pharmaceutical ingredients for the treatment of tuberculosis
Abstract:Tuberculosis, caused by mycobacteria Mycobacterium tuberculosis, remains the world’s leading cause of death from infectious disease despite the availability of effective chemotherapy. The emergence and spread of multidrug-resistant and extensively drug-resistant strains have severely compromised the efficacy of conventional first-line regimens based on isoniazid, rifampicin, pyrazinamide, and ethambutol. This review summarizes the mechanisms of action of current and newly approved anti-tuberculosis agents highlighting the evolution of resistance and the urgent need for novel therapeutic options. Newly introduced agents – bedaquiline, delamanid and pretomanid – represent significant progress. They target energy metabolism with new mechanisms of action and act complementary to existing therapies, while also targeting new targets to further affect mycobacterial cell wall synthesis. however, emerging resistance to these novel compounds underscores the necessity for optimized combination regimens, adherence monitoring, and continuous drug development. Despite these challenges, current research offers promising strategies to shorten treatment duration, improve outcomes, and move closer to the goals of world health Organization for reducing incidence and mortality rates.
