Title:Neželeni učinki in interakcije zaviralcev imunskih kontrolnih točk

Abstract:Vedno več bolnikov z rakom zdravimo z zaviralci imunskih kontrolnih točk, zato je pomembno njihovo poznavanje s strani vseh zdravstvenih delavcev. Mehanizem, potek in zdravljenje neželenih učinkov kot tudi pričakovane interakcije zaviralcev imunskih kontrolnih točk se bistveno razlikujejo od ostalih zdravil. Zaviralci imunskih kontrolnih točk povzročajo široko paleto imunsko pogojenih neželenih učinkov. Njihovo zgodnje prepoznavanje je ključno pri preprečevanju njihovega napredovanja do resnih zapletov. Zdravila, ki posredno, preko spremembe črevesne mikrobiote, ali neposredno modulirajo imunski sistem, lahko vstopajo v interakcije z zaviralci imunskih kontrolnih točk. Med temi so tudi nekatera zdravila, ki so pogosto v uporabi, kot so antibiotiki in zaviralci protonske črpalke. Za preprečevanje njihovega negativnega vpliva na izide zdravljenja z zaviralci imunskih kontrolnih točk je zato pomembna njihova smotrna uporaba, za katero si moramo prizadevati vsi zdravstveni delavci.

Title:Adverse events and interactions of immune checkpoint inhibitors
Abstract:The number of cancer patients treated with immunotherapy using immune checkpoint inhibitors is steadily increasing; therefore, it is imperative that all healthcare professionals become familiar with these agents. The mechanisms, clinical manifestations and management of adverse events and drug interactions associated with immune checkpoint inhibitors differ substantially from those of conventional therapies. Early recognition of immune-related adverse events is crucial to prevent progression to severe complications. Medicines that modulate the immune system directly, or indirectly through alterations in the gut microbiota, may interact with immune checkpoint inhibitors. These include commonly used medicines such as antibiotics and proton pump inhibitors. Healthcare professionals should promote their rational use to minimize the detrimental effects on treatment outcomes with immune checkpoint inhibitors.
