Title:Ali lahko s prehranskimi ukrepi vplivamo na izide imunoterapije raka?

Abstract:Klinična prehrana je stroka, ki se ukvarja s preprečevanjem, diagnosticiranjem in obravnavo prehranskih ter presnovnih stanj. Onkološki bolniki so izrazito ogroženi za razvoj podhranjenosti, kaheksije in sarkopenije. Te motnje prehranjenosti vodijo v slabše klinične izide zdravljenja raka, vključno z imunoterapijo z zaviralci imunskih kontrolnih točk. Prehranjenost na izide zdravljenja z zaviralci imunskih kontrolnih točk vpliva preko različnih mehanizmov, med drugim preko vpliva na črevesno mikrobioto. Večja raznolikost in prisotnost določenih bakterijskih vrst sta pri bolnikih povezana z boljšimi izidi, zato modulacija črevesne mikrobiote predstavlja potencialno strategijo za izboljšanje izidov zdravljenja z zaviralci imunskih kontrolnih točk. Raziskave kažejo predvsem ugoden vpliv zadostnega vnosa prehranskih vlaknin. Za rutinsko uporabo komercialno dostopnih probiotičnih izdelkov pri bolnikih, zdravljenih z zaviralci imunskih kontrolnih točk, trenutno ni dovolj dokazov o klinični koristi. Sistematična, pravočasna in individualizirana prehranska obravnava onkoloških bolnikov pomembno prispeva k varnosti in učinkovitosti onkološkega zdravljenja.

Title:Can dietary interventions affect cancer immunotherapy outcomes?
Abstract:Clinical nutrition deals with prevention, diagnosis and management of nutritional and metabolic changes. Cancer patients are at high risk of malnutrition, cachexia, and sarcopenia. These disorders are linked to poorer clinical outcomes in cancer treatment, including immunotherapy with immune checkpoint inhibitors. Nutrition influences the outcomes of immune checkpoint inhibitors treatment through various mechanisms, including its effect on the gut microbiota. greater microbial diversity and the presence of certain bacterial species are associated with better outcomes in patients, therefore modulation of the gut microbiota represents a potential strategy to enhance immune checkpoint inhibitors efficacy. Clinical studies show a particularly beneficial effect of adequate dietary fibre intake. There is currently insufficient evidence of clinical benefit for the routine use of commercially available probiotics in patients treated with immune checkpoint inhibitors. Systematic, timely, and individualised nutritional management of cancer patients is essential for optimising both the safety and effectiveness of cancer treatment.
