Title:Komunikacija in kakovost odnosov med medicinsko sestro in starostniki v patronažnem varstvu

Abstract:Uvod: V zdravstveni negi starostnikov sta komunikacija in z njo povezan odnos do starih ljudi zelo pomembna dejavnika kakovostne zdravstvene nege v patronažnem varstvu. Ugotoviti smo želeli zadovoljstvo starih ljudi v zdravstveni oskrbi ter njihovo vključevanje v proces odločanja v zdravstveni negi. Metode: Raziskava temelji na kvantitativni metodi dela. Izvedena je bila terenska raziskava, podatki so bili zbrani s strukturiranim vprašalnikom,ki je vseboval 28 vprašanj. Vzorec je sestavljalo 110 starostnikov, ki so bili izbrani na način priročnega vzorca, stari so bili med 65 in 100 let in so živeli v domačem okolju. Podatke so zbrale patronažne medicinske sestre ob kurativnih in preventivnih obiskih v januarju in februarju 2012. Podatki so bili obdelani s pomočjo opisne statistike, enofaktorske analize variance in testa hi-kvadrat. Rezultati: V največjem delu so anketiranci navedli, da lastno zdravje ocenjujejo kot zadovoljivo (42,7 %), v 30,9 % pa kot slabo. Rezultati so pokazali statistično pomembno razliko med oceno zdravja anketirancev (p = 0,028) in željo anketirancev o vključevanju v odločanje (p = 0,028). Rezultati pokažejo, da med starostnimi skupinami anketirancev ni statistično pomembnih razlik pri spremenljivkah vljudnost in prijaznost (p = 0,316), poznavanje osebnega, socialnega in kulturnega ozadja starostnikov (p = 0,345), želje anketirancev, da se obisk neprekinja s telefonskimi pogovori (p = 0,195) in vključevanje anketirancev v odločanje pri zdravstveni negi (p = 0,836). Diskusija in zaključek: V zdravstveni negi starejših ljudi na domu je pomemben dejavnik komunikacija in kakovost odnosov patronažnih medicinskih sester do anketirancev. Rezultati so lahko podlaga za izboljševanje zdravstvene nege starejših ljudi na domu.

Title:Communication and quality of relations beetwen community nurses and elderly people in community health care
Abstract:Introduction: Communication and the attitude to elderly people present a significant factor of quality community nursing. A field study was designed to collect data on the levels of elderly people' s satisfaction with nursing care and the patients' involvement in decision-making related to the choice of treatment and any other services available in their home environment. Methods: A quantitative method was used to ascertain the relative size of the phenomena studied. The data were collected by community nurses in January and February of 2012 during their preventive and curative home visits. The questionnaire was composed of 28 questions. A convenience sample of patients recruited consisted of 110 elderly people, aged between 65 and 100 living in their homes. Descriptive statistics, analysis of one factor variance and the chi-square test were used in the analysis of the data. Results: The respondents most frequently reported that their health was satisfactory (42.7%) while 30.9% rated their current state of health as bad. The results indicate a statistically significant difference between the health self-assessment (p = 0.028) and the willingness of the respondents to be included in decision-making (p = 0.028). No statistically significant differences were found between different age groups of the respondents related to politeness and kindness of community nurses (p = 0.316), community nurse information on the respondents' personal, social and cultural background(p = 0.345), the respondents' wish that the home visits be interrupted by telephone calls (p = 0.195) and their inclusion in shared health care decision-making (p = 0.836). Discussion and conclusion: The nurse-patient communication, proper interaction and relationship are important factors in community nursing which have a significant impact on the quality of community nursing services. The results of the study may provide a sound basis for further service excellence in community health nursing.
