Title:Prepoznavanje neželenih učinkov zdravil na Interni kliniki Univerzitetnega kliničnega centra Ljubljana

Abstract:Farmakovigilanca zajema odkrivanje in obravnavo neželenih učinkov zdravil ter drugih težav, povezanih z zdravili. V Univerzitetnem kliničnem centru Ljubljana smo v okviru Centra za klinično toksikologijo in farmakologijo v preteklih letih izvedli več raziskav, osredotočenih na prepoznavanje in obravnavo neželenih učinkov zdravil na Interni kliniki. Posebno pozornost smo namenili ocenjevanju vzročne povezanosti med domnevnimi neželenimi učinki zdravil in povzročitelji. V ta namen smo razvili orodje, ki poleg objektivne ocene vzročne povezanosti po lestvici Naranjo vključuje tudi ločeni strokovni oceni zdravnika in raziskovalcev. Novo orodje omogoča razvrstitev večjega števila primerov med potrjene neželene učinke. Pri prepoznavi neželenih učinkov zdravil iz elektronske dokumentacije bolnikov pomembno vlogo igrajo ključne besede, ki zajemajo nespecifične in specifične ključne besede, imena zdravil in laboratorijske kazalce. Žal ima večina ključnih besed nizko napovedno vrednost, zato si v prihodnosti želimo razvoj programskih orodij, ki bodo omogočila kompleksnejše načine iskanja in prepoznave neželenih učinkov ter s tem zmanjšala količino zamudnega ročnega dela

Title:Recognition of adverse drug reactions at the division of internal medicine, University Medical Centre Ljubljana
Abstract:Pharmacovigilance includes the detection and treatment of adverse drug reactions and other drug-related complications. In the last years, several studies have been conducted at the Centre for clinical toxicology and pharmacology of the University medical centre Ljubljana, focusing on the recognition and management of adverse drug reactions in internal patients. Particular focus was placed on the assessment of the causality relationship between suspected adverse drug reactions and the causative drugs. We developed a new causality assessment tool that included the objective Naranjo score and separate expert judgments of the physician and investigators. With the new tool, a larger number of adverse drug reaction cases were assessed as certain. Keywords play an important role in the identification of adverse drug reactions from the patients’ electronic medical records. These include non-specific and specific keywords, drug names and laboratory markers. Unfortunately, most keywords have a low positive predictive value. In the future, we would like to implement improved software tools that would enable more complex methods of automatized adverse drug reaction search and thus reduce the need for time-consuming manual search methods
