Title:Results of longitudinal Nutri-D study : factors influencing winter and summer vitamin D status in a Caucasian population

Abstract:Introduction: Inadequate vitamin D status is a worldwide public health issue. In humans, vitamin D status is affected by diet, and even more by exposure to ultraviolet B (UVB) light and consequential endogenous synthesis. Various personal and environmental factors influence endogenous synthesis. Factors affecting vitamin D status were investigated in a prospective longitudinal cohort study with a summer and winter observation period. Methods: The final sample included 292 adults, of those 111 (38%) males and 181 (62%) females, with a mean age of 38.2 (±11.8) years from Slovenia who were not supplementing vitamin D. Serum 25-hydroxyvitamin D [25(OH)D] concentrations were measured in both periods; vitamin D intake, self-reported body mass index (BMI), and protective behaviors against sun were also recorded. Other measured parameters included measurements of constitutive skin color using the objective individual typology angle (ITA), and difference in the melanin index (ΔMI) for assessment of objective sun exposure. Results: In winter a high prevalence (63.4%) of insufficient vitamin D status (< 50 nmoL/L) was observed with higher odds ratios (OR) for insufficiency in those with a higher BMI and light ITA. During summer, insufficiency prevalence was low (5.5%), but half of the participants (50.0%) had suboptimal 25(OH)D concentration (< 75 nmol/L). In summer OR for suboptimal status were higher in those with obesity, lower ΔMI, light ITA, low vitamin D intake, and protective clothing behaviors. Conclusion: Using a series of measures, we showed that vitamin D status is hugely affected by several personal factors such as BMI, ITA, vitamin D intake, ΔMI, and protective behavior against the sun. This conclusion questions the usefulness of generalized population-level recommendations since personal factors are a major predictor of vitamin D status

Title:Rezultati longitudinalne raziskave Nutri-D: dejavniki vpliva na zimsko in poletno preskrbljenost z vitaminom D v Kavkazijski populaciji
Abstract:Ustrezna preskrbljenost z vitaminom D je svetovni javno-zdravstveni problem. Pri ljudeh na preskrbljenost z vitaminom D vplivata prehrana, še bolj pa izpostavljenost ultravijolični B (UVB) svetlobi in posledična endogena sinteza. Na endogeno sintezo vplivajo različni osebni in okoljski dejavniki, kar smo raziskovali v prospektivni longitudinalni kohortni raziskavi s poletnim in zimskim opazovalnim obdobjem. Metode: Končni vzorec je vključeval 292 odraslih oseb, od tega 111 (38 %) moških in 181 (62 %) žensk. Udeleženci so bili iz Slovenije, povprečne starosti 38,2 (±11,8) let, in niso dodajali vitamina D. Koncentracije 25-hidroksivitamina D [25(OH)D] so bile merjene v zimskem in poletnem obdobju: v slednjem smo določali tudi prehranski vnos vitamina D, indeks telesne mase (ITM) in zaščitna vedenja pred soncem. Drugi merjeni parametri so vključevali meritve konstitutivno barvo kože s pomočjo objektivnega individualnega tipološkega kota (ITA) in razlike v indeksu melanina (ΔMI), s katerimi smo objektivno izpolnjevali izpostavljenost soncu. Rezultati: Pozimi smo opazili visoko prevalenco (63,4 %) pomanjkanja vitamina D (<50 nmoL/L) s povišanim tveganjem za pomanjkanje pri osebah z višjim ITM in zelo svetlo in na sonce občutljivo/reaktivno kožo. Poleti je bila prevalenca pomanjkljivosti nizka (5,5 %), vendar je polovica udeležencev (50,0 %) imela neoptimalno koncentracijo 25(OH)D (<75 nmol/L). Poleti so OR za neoptimalno preskrbljenost z vitaminom D višji pri tistih z debelostjo, nižjim ΔMI, svetlim ITA, nizkim prehranskim vnosom vitamina D in tisti, ki za zaščito pred soncem dosledno uporabljajo oblačila, ki pokrivajo večino telesa. Hkrati nismo opazili povezave z uporabo s kozmetičnimi izdelki za zaščito pred soncem. Zaključek: Z uporabo različnih meritev smo pokazali, da na preskrbljenost z vitaminom D močno vpliva več osebnih dejavnikov, kot so ITM, ITA, prehranski vnos vitamina D, ΔMI in zaščitno vedenje pred soncem. Zaradi navedenega je vprašljiva koristnost splošnih priporočil na ravni populacije, saj na preskrbljenost z vitaminom D zelo vplivajo osebni dejavniki.
