Kirurško zdravljenje melanoma

Abstract: Kirurško zdravljenje je najstarejši in še danes osnovni način zdravljenja kožnega melanoma. Pri bolnikih z melanomom tanjšim od 0,8 mm je ekscizija v lokalni anesteziji edino potrebno zdravljenje z več kot 95% ozdravitvijo. Pri bolnikih z melanomo debelejšim od 1 mm napravimo poleg ekscizije primarnega melanoma še biopsijo varovalne bezgavke, ki je najpomembnejši prognostični dejavnik. Bolniki z klinično ugotovljenimi zasevki v regionalnih bezgavkah potrebujejo disekcijo regionalne bezgavčne lože. V bližnji prihodnosti bomo ta poseg najverjetneje opravljali po neoadjuvantnem sistemskem zdravljenju. In transit zasevke omejene na ekstremitete lahko zdravimo glede na breme bolezni z elektrokemoterapijo ali hipertermično izolirano ekstremitetno perfuzijo. Pri oligometastaski bolezni ali pri bolnikih z rezistenco na sistemsko zdravljenje omejeno na posamične zasevke je smiselno opraviti metastazektomijo.

Bryophyte diversity, composition and functional traits in relation to bedrock and tree species composition in close-to-nature managed forests

Abstract: Bryophytes respond to a variety of environmental factors, including tree species composition and geological conditions, which are influenced by both natural and anthropogenic factors. Bryophytes growing on a variety of substrates were identified in ICP Forests monitoring plots in a wide range of close-to-nature managed forests, from lowland to high mountain areas in Slovenia. Based on data on tree layer composition (broadleaves vs. conifers) and on bedrock and soil type (calcareous vs. siliceous bedrock), all study plots were classified into five groups representing forest types. Different ordination analyses and statistical tests were used to represent differences between forest types in bryophyte composition, richness and functional traits. Almost 200 bryophyte species were identified in managed forests, with the most common being Hypnum cupressiforme, Brachytheciastrum velutinum and Polytrichum formosum. Bryophyte species composition varied considerably along the tree species composition and edaphic gradients represented by the selected forest types. Bryophyte species richness was significantly higher on calcareous bedrock than on siliceous bedrock. The higher occurrence of rocks on the ground surface may significantly increase the availability of various microhabitats for bryophytes. Bedrock had a significant influence on the functional diversity and composition of bryophytes, while tree species composition had less influence. Bedrock and tree species composition are important drivers of bryophyte species composition and diversity at taxonomic and functional trait levels. The high diversity of bryophyte species in Slovenian forests can also be attributed to close-to-nature and sustainable forest management practices that preserve favourable microclimatic conditions in forest stands and maintain diverse substrates.

The effect of rapid weight loss on the handgrip strength of national-level wrestlers

Abstract: The effects of rapid weight loss (RWL) in combat sports athletes is an area that is not yet fully discovered. Therefore, the goal of this study was to determine the impact of weight loss on HGS (handgrip strength) in Greco-Roman wrestlers.This cross-over study included ten athletes examined for HGS during three time points for each hand. The first measurement – baseline (BL), was performed before we-ight reduction. The second measurement ‒ phase 1 (P1), was taken after high-intensity sports specific training (HISST) combined with RWL. In this phase, participants had to lose 5% of their body mass within three days, after which HISST was carried out (on day 3). The third measurement ‒ phase 2 (P2), was performed seven days after P1, which included HISST with no RWL.When comparing the HGS values, significantly higher values were observed only when P1 was compared with P2 for the left hand (p=0.039). This means that the grip was significantly stronger after RWL was conducted together with HISST.This research could be of great importance to combat sports coaches and athletes as it evaluates the effect of RWL on performance parameters. Therefore, the results of our study could serve to improve the wrestlers’ weight-reduction plan.

Novosti na področju imunoterapije rakov spodnjih prebavil

Abstract: Rak debelega črevesa in danke (RDČD) v svetovnem pogledu predstavlja drugi najpogostejši vzrok smrti in po incidenci sodi na tretje mesto med vsemi raki. V zadnjih 10-15. letih smo lahko priča velikim napredkom v sistemskem zdravljenju RDČD in to je poleg uspešnih preventivnih programih glavni razlog da v tem trenutnku RDČD spada med bolezni z dobrim preživetjem. V mednarodni raziskavi Concord-3 je bilo ugotovjeno, da petletno čisto preživetje bolnikov v več kot polovici evropskih držav že presega 60 %. Najpomembnejši prognostični dejavnik je stadij ob diagnozi, saj je pet-letno čisto preživetje bolnikov z omejenim stadijem že več kot 90 %, medtem ko je preživetje tistih z razsejano boleznijo pa komaj 9 %. Pri 50% bolnikov z N+ boleznijo ob ugotovitvi bolezni v poteku njihove bolezni le-ta postane metastatska. V nasprotju z splošnim prepričanjem je tudi RDČD imunogena maligna bolezen, vendar le v primeru ko gre za MSI-H (mikrosatelitno nestabilno) obliko bolezni. Tovrstni tumorji imajo namreč preko 1500 mutacij na tumor, kar je napovedni dejavnik za učinkovitost zdravljenja z imunoterapijo (IT). Tako ni presenečenje, da je IT lahko zelo učinkovita možnost zdravljenja pri bolnikih z metastatskim RDČD in je tudi že del mednarodnih in slovenskih smernic zdravljenja RDČD.
