Induction chemotherapy and concurrent chemoradiotherapy for larynx preservation in laryngeal and hypopharyngeal cancer

Abstract: To test the hypothesis that clinical tumor response after a single cycle of induction chemotherapy (ICT) can reliably differentiate between chemo-/radiosensitive and resistant tumors in the larynx preservation setting. Patients and methods Treatment consisted of docetaxel/cisplatin/5-fluorouracil (TPF) ICT followed by concurrent chemoradiotherapy (cCRT) with weekly cisplatin. The response of the primary tumor was assessed by transnasal endoscopy after the first ICT cycle. Results 37/39 (95%) patients with laryngeal (46%) or hypopharyngeal (54%) carcinoma responded to one cycle of ICT, and two patients were referred for salvage surgery. Laryngectomy-free survival at 2 and 5 years was 87% and 75%, respectively. The corresponding rates for locoregional control (and also for disease-free survival) were 79% and 70% and for overall survival 92% and 82%. Conclusions Clinical assessment of tumor response to one cycle of TPF ICT serves as a valid and easy-to-use predictor of tumor sensitivity to platinum-based cCRT.

Uvealni melanom : priporočila zdravljenja

Abstract: Uvealni melanom je najpogostejši primarni maligni znotrajočesni tumor pri odraslih. Med uvealne melanome uvrščamo melanom žilnice (90 %), ciliarnika (6 %) in šarenice (4 %). Med leti 2017 in 2021 je za uvealnim melanomom v Sloveniji letno zbolelo povprečno 23 oseb, kar predstavlja 3,7 % vseh melanomov. Prva slovenska celostna in multidisciplinarna priporočila zdravljenja vključujejo melanome žilnice in ciliarnika in so povzeta po ameriških smernicah NCCN. Priporočila celostno predstavijo tako dejavnike tveganja, kot klinično sliko in možnosti lokalnega zdravljenja tega raka, kot tudi sodobne načine multidisciplinarnega zdravljenja oddaljenih zasevkov vključno z možnostjo genotipizacije polne krvi na prisotnost človeškega levkocitnega antigena (HLA)-A*02:01). S priporočili želimo specialistom različnih strok olajšati tako pogovor z bolnikom in svojci, kot izbiro ustreznega zdravljenja.

Post-radiation xerostomia therapy with allogeneic mesenchymal stromal stem cells in patients with head and neck cancer : study protocol for phase I clinical trial

Abstract: Background: Xerostomia is a common side effect of radiotherapy in patients with head and neck tumors that negatively affects quality of life. There is no known effective standard treatment for xerostomia. Here, we present the study protocol used to evaluate the safety and preliminary efficacy of allogeneic mesenchymal stromal stem cells (MSCs) derived from umbilical cord tissue. Methods: Ten oropharyngeal cancer patients with post-radiation xerostomia and no evidence of disease recurrence 2 or more years after (chemo)irradiation (intervention group) and 10 healthy volunteers (control group) will be enrolled in this nonrandomized, open-label, phase I exploratory study. MSCs from umbilical cord tissue will be inserted under ultrasound guidance into both parotid glands and both submandibular glands of the patients. Toxicity of the procedure will be assessed according to CTCAE v5.0 criteria at days 0, 1, 5, 28, and 120. Efficacy will be assessed by measuring salivary flow and analyzing its composition, scintigraphic evaluation of MSC grafting, retention, and migration, and questionnaires measuring subjective xerostomia and quality of life. In addition, the radiological, functional, and morphological characteristics of the salivary tissue will be assessed before, at 4 weeks, and at 4 months after the procedure. In the control group subjects, only salivary flow rate and salivary composition will be determined. Discussion: The use of allogeneic MSCs from umbilical cord tissue represents an innovative approach for the treatment of xerostomia after radiation. Due to the noninvasive collection procedure, flexibility of cryobanking, and biological advantages, xerostomia therapy using allogeneic MSCs from umbilical cord tissue may have an advantage over other similar therapies.

Abstract: V zborniku je celostno predstavljena problematika rakov glave in vratu, kot jo dojemajo strokovnjaki, ki se vsakodnevno srečujejo z bolniki s temi raki. Osvetljeni so vzroki za nastanek rakov glave in vratu, incidenca, mortaliteta in sodobni epidemiološki trendi, znaki in simptomi, ki jih povzročajo, načela sodobne diagnostike in zdravljenja s posledično morbiditeto ter možnosti podporne terapije in rehabilitacije. Poseben poudarek je namenjen obravnavi teh rakov v slovenskem prostoru in nacionalnim smernicam.

Vloga radioterapije pri zdravljenju očesnega melanoma

Abstract: Očesni melanom je redek rak z incidenco 5/ milijon prebivalcev in vrhom pojavnosti okrog 60. leta starosti. Uvealni melanom predstavlja več kot 90% primerov. Enukleacija je bila primarna metoda zdravljenja več kot stoletje, vendar je privedla do izgube funkcionalnega očesa. Danes je radioterapija, primarno brahiterapija, metoda izbora za zdravljenja uvealnega melanoma. Za izvedbo brahiterapije je potrebna kirurška vstavitev radioaktivne ploščice v bližino tumorja, primerna pa je za tumorje debeline 8- 10 mm in premera do 16 mm. Pri večjih tumorjih se uporablja zunanje obsevanje, pri čemer tako obsevanje s protonskim snopom (PBT) kot tudi stereotaktična radioterapija (SRT) kažeta dobre in primerljive rezultate. Radioterapija lahko povzroči neželene učinke, kot so katarakta, radiacijska retinopatija in neovaskularni glavkom, vendar koristi ohranjanja vida pogosto pretehtajo tveganja. Radioterapija je učinkovita možnost zdravljenja uvealnega melanoma, ki dosega primerljive izide z enukleacijo in omogoča ohranitev vida v večini primerov.

Etiološki vzroki za nastanek raka glave in vratu ter možnosti za preventivno ukrepanje

Abstract: Najpomembnejša etiološka vzroka za nastanek raka glave in vratu sta tobak in alkohol, ki sta skupaj udeležena pri nastanku več kot 70 % rakov glave in vratu, v ospredju pa je v zadnjih letih tudi okužba s humanim papilomavirusom (HPV). Kot najbolj učinkovit ukrep za zmanjševanje uživanja škodljivih substanc se je izkazala primarna preventiva, za preprečevanje rakov, povzročenih s HPV, pa cepljenje.
