Magnetic resonance spectroscopy: an overview of the method and its applicationin clinical neuroradiology

Povzetek: Background. Magnetic resonance spectroscopy (MRS) is a comparatively new diagnostic method. Brain tissue is very suitable for MRS analysis. In practice, only a small number of compounds present in the brain may be analysed by MRS. The utility of MRS in neuroradiology and clinical practice isconstantly growing since the investigation is of help in the differential diagnosis of pathological processes as well as in assessing the progress of a disease and evaluating the outcome of treatment. In analysing the data obtained, a number of factors that may influence the objectivity of the resultmust be taken into account. The magnetic resonance scanner located at the Institute of Radiology, University Medical Centre Ljubljana, utilises modern MRS protocols and has proved a valuable tool in the diagnostic evaluation of neurologic diseases. Conclusions. MRS provides spectral analysisof substance in a selected volume of tissue, thereby offering an insight into the metabolic state of the tissue.

Bifocal primary intracranial germinoma in a child. Case report

Povzetek: Background. Bifocal primary intracranial germinal tumors are rare. Only 5-10% of all germ cell tumors are found both in the suprasellar and pineal region. Case report. In presented patient we found two primary intracranial germinomasin pituitary and pineal gland that were successfully operated. Radiological properties of germinomas and differential diagnosis are discussed. Conclusions. Although the definite histological diagnosis cannot beachieved by computer tomography and/or magnetic resonance images alone, a detailed analysis of neuroradiological images is useful for predicting the histologieal diagnosis.

Ultrasonographic diagnnosis of obstructive ileus in a patient with Meckel's diverticulum

Povzetek: Introduction. Despite the use of modern imaging techniques, the reliable preoperative assessment of Meckel's diverticulurn and related complications with this rare congenital anomaly of the gastrointestinal tract in adults is uncomrnon. Case presentation. This report presents the case of a 25 year old man who presented with a sudden onset of pain in the right lower abdomen and vomiting. On clinical examination the affected area was tender to palpation which revealed an elastic cylindrical formation situated deeply in the abdomen. Blumberg's sign was positive while the laboratory findings were stillwithin normal limits. An ultrasonography of the abdomen revealed an ileocaecal fluid collection containing thicker residue, which was suspicious for Meckel's diverticulum or a duplication cyst and ileus of the small bowel proximally from the formation described. Native radiogram of the abdomen in supine position has confirmed the presence of obstructive ileus at the level of the distal part of the small bowel. Surgery revealed an ileus and compression of the distal part of the small bowel due to the presence of an edematous Meckel's diverticulum. Conclusion. In view of the frequent use of ultrasonography in the evaluation of acute ahdomen, the diagnostic procedures could be rationalized and the time to surgery reduced if possible complicatioris due to Meckel's diverticulum would he considered in the differential diagnosis.
