Long term follow-up after radiosynovectomy with yttrium 90 in patients with different rheumatic diseases

Abstract: The aim of the retrospective study was to evaluate the efficacy of radiosynovectomy (with yttrium 90) mainly in patients with rheumatoid arthritis, less with some other rheumatic diseases. The evaluation period varied from half to nine years. The procedure was performed in 273 patients (225 females, 48 males) or in 463 joints (402 knees, 61 shoulders and ankles).The effects was evaluated by change in degree of morning stiffiness, pain and swelling (score from 0 to 9). Very good results were obtained in 69 (15%), good in 142 (30.5%), moderate in 197 (42.5%) and no effect in 55 (12%) joints. Six months after the procedure 38 joints (8%), half to two years after221 joint (48%) were in good remission, after 3 to 4 years 95 joints (20%), after 5 to 6 years 57 joints (12%) were well, 7 to 9 years later 52 joints (11%) showed no signs of arthritis. Joint pain and swelling were the most frequent procedure complications (5.6%). In two patients with additional immunomodulating therapy chronic myeloid and lymphocityc leukaemia were diagnosed. Radiosynovectomy is considered to be an effective and safe treatment for synovitis indifferent rheumatic diseases.

Cytology of mediastinal tumors

Abstract: Our experience with cytological examinations of tumorous mediastinal lesions is evaluated. A group of 117 patients with mediastinal tumor have been included into the study. Among them carcinomas prevailed (60,7%), followed by lymphomas (18,8%), other tumors (15.4%) and thymic neoplasms (5.1%). Malignantor suspicious cells were found in 77.4% of patients with carcinoma. The cells indicating a possibility of non-Hodgkin's lymphoma were found in 9 out of 14 patients. In 5 out of 6 thymic neoplasms the cytological pattern wasconsistent with the diagnosis of thymic neoplasm. One case of thymoma was cytologically falsely diagnosed as malignant lymphoma. One case of neurofibroma was falsely diagnosed as adenocarcinoma. The sensitivity of cytological examinations was 67.5%. If 18 patients with diagnostically unsatisfactory material were excluded from the analysis, the sensitivity wouldincrease to 80.8%. Owing to the wide variety of primary and metastatic tumors that can occur in the mediastinum, apart from the routine cytological techniques, additional staining methods should be used. For final cytological diagnosis the integration of cytological findings with clinical and radiological data is often required. Owing to the characteristics of the obtained material and biological behaviour of some mediastinal tumors, some tumors cannot be definitively diagnosed by cytological examinations alone.

A role of gender in the occurrence of dimethylhydrazine induced colorectal tumors in Wistar rats

Abstract: Human colorectal carcinoma appears more frequently in males. The aim of our study was to evaluate the influence of gender on the induction of colorectal carcinoma by 1,2-dimethylhydrazine (DMH) in Wistar rats. Sixty Wistar rats (30males, 30 females) were subjected to weekly subcutanous injections of DMH (20 mg/kg) for 15 weeks. After 25 weeks from the beginning of the experiment the animals were sacrificed and autopsied. All macroscopical lesions were evaluated histologicaly. Induction of colorectal tumors succeeded in 37% of males and 17% of females. There were 21 tumors of the large bowel found, of these 15 in males and 6 in females. Histologically, males had 11 adenomas, 2 signet-cell carcinomas and 2 adenocarcinomas, while females had 4 signet-cell carcinomas and 2 adenomas. We also found extracolonic tumors, mainly those of the small intestine and of the Zymbal glands. Wistar rats showed lower incidence of DMH-induced colorectal tumors in comparison with other strains ofrats. The gender-dependent difference in the incidence of colorectal tumors was found to be statistically marginally significant (p<0.08), whereas the difference in the incidence of all induced tumors between genders was significant (p<0.02). Males showed a greater incidence of coloreactal tumors and also a greater histological resemblance to human colorectal tumors than females. That is why we recommend Wistar males rather than females for research on colorectal tumors.

Interferon alpha (IFN-[alpha]) in treatment of malignant diseases

Abstract: There are several natural varieties of IFN-alpha in clinical use. Although IFNs were the first cytokines in use as a new treatment modality, we still know little about their mode of action in malignancies. IFN-alpha can inhibit cell growth, but it has been shown only recently, on malignant cells from patients with multiple myeloma, that IFN-alpha can exert a direct cytotoxic effect on tumor cells. The treatment with natural leucocyte IFN-alpha in dosesused in the early clinical trials, are still most widely used. It has been shown that through a high local concentration by local application in malignant melanoma, basalioma, pleural carcinosis, glioblastoma, total local tumor control may be achieved. it is thys not known what the optimal doses of IFN-alpha are, and they may vary by the tumor type, as well as from patient topatient. The neccessary duration of treatment also remains an open question.IFNs are a tool for treatment of malignant tumors. Their cytotoxic effect, however, is not strong enough and does not last. At present, there is a vast field of investigative work stil open-but in the meantime it seems necessary to proceed with clinical trials of different types of IFN, the natural IFN-alpha again beeing the most interesting one.

Diagnostic value of planar myocardial perfusion scintigraphy in patients with coronary artery disease

Abstract: Patients with suspected or proven coronary artery disease are investigated using noninvasive and invasive diagnostic methods. Noninvasive myocardial perfusion scintigraphy provides data on myocardial perfusion during stress andat rest. Coronary angiography is invasive morphologic method, performed before coronary artery dilatation or surgery. Aim of our retrograde analysis of planar thallium myocardial perfusion scintigrams and coronary angiograms was to assess sensitivity and specificity of rnyocardial perfusion planar scanning and to evaluate causes of possible disagreement. Original readings ofmyocardial perfusion scans and coronary angiograms of 156 patients with coronary artery disease were compared. When results of both investigations were partially concordant or discordant, the original studies were reviewed. Concordant results of both examinations were found in 62% of patients. In only3% (5 patierrts) the results were discordant and the reason for disagreement of results of both studies could not be detected. Most of the remaining 55 patients had more pronounced myocardial perfusion defects than was the estinrated severity of cororsary artery stenosis, attributed to the different nature of both irtvestigations. Anonralous coronary artery was foundirt 3% of all patients, tortuous coronary arteries with slow ftow of coritrast media in 9 patients (6% of all) and arterial hypertension with extreme left ventricular wall hypertrophy in one patient. Serrsitivity of the rnyocardial perfusion scintigraphy was l00% and specificity 50%. Positive predictive valrre for coronary artery disease was 96% and negative predictive value was 100 %. We cconclude that myocardial perfusion scintigraphy has a definite role in: diagnosis and follow-up of patients with suspected or provencoronary artery disease. The urokinase-type plasminogen activator, its inhibitors and its receptor - the new prognostic factors in solid cancers

Abstract: The urokinase-type plasminogen activator (uPA), its inhibitors (PAI-1 and PAI-2) and its receptor (uPAR) play nan important role in the degradation of the intercellular tissue, the process which affects the ability of cancer cells to invade to surrounding tissue and to metastasize. The results of clinical studies performed in the past few years point out a significant influence of uPA, PAI-1, PAI-2 and uPAR on the course of the disease and survival of patients with solid tumours, particularly breast cancer. Hopefullythe categorization of patients according to the content of the serineproteases and its inhibitors in tumour tissues could provide a basis formore rational treatment planning and thus improving patients' survival.
