Mesenteric ischemia after capecitabine treatment in rectal cancer and resultant short bowel syndrome is not an absolute contraindication for radical oncological treatment

Povzetek: Thrombotic events, arterial or venous in origin, still remain a source of substantial morbidity and mortality in cancer patients. The propensity for their development in oncology patients is partially a consequence of the disease itself and partially a result of our attempts to treat it. One of the rarest and deadliest thromboembolic complications is arterial mesenteric ischemia. The high mortality rate is caused by its rarity and by its non-specific clinical presentation, both of which make early diagnosis and treatment difficult. Hence, most diagnoses and treatments occur late in the course of the disease. The issue survivors of arterial mesenteric ischemia may face is short bowel syndrome, which has become a chronic condition after the introduction of parenteral nutrition at home. We present a 73-year-old rectal cancer patient who developed acute arterial mesenteric thrombosis at the beginning of the pre-operative radiochemotherapy. Almost the entire length of his small intestine, except for the proximal 50 cm of it, and the ascending colon had to be resected. After multiorgan failure his condition improved, and he was able to successfully complete radical treatment (preoperative radiotherapy and surgery) for the rectal carcinoma, despite developing short bowel syndrome (SBS) and being dependent upon home-based parenteral nutrition to fully cover his nutritional needs. Mesenteric ischemia and resultant short bowel syndrome are not absolute contraindications for radical oncological treatment since such patients can still achieve long-term remission.

Capecitabine in adjuvant radiochemotherapy for gastric adenocarcinoma

Povzetek: Background. In patients with non-metastatic gastric cancer surgery still remains the treatment of choice. Postoperative radiochemotherapy with 5-fluorouracil and leucovorin significantly improves the treatment outcome. The oral fluoropyrimidines, such as capecitabine, mimic continuous 5-fluorouracil infusion, are at least as effective as 5-fluorouracil, and such treatment is more comfortable for the patients. Patients and methods. In the period from October 2006 to December 2009, 101 patients with gastric cancer in stages Ib-IIIc were treated with postoperative chemoradiation with capecitabine. Distal subtotal resection of the stomach was performed in 46.3%, total resection in 50.5% and multivisceral resection in 3.2% of patients. The main endpoints of this study were loco-regional control (LRC), disease-free survival (DFS), disease-specific survival (DSS) and overall survival (OS). The rates of acute side-effects were also estimated. Results. Seventy-seven percent of patients completed the treatment according to the protocol. The median followup time of all patients was 3.9 years (range: 0.4-6.3 years) and in survivors it was 4.7 years (range: 3.2-6.3 years). No death occurred due to the therapy. Acute toxicity, such as nausea and vomiting, stomatitis, diarrhoea, hand-foot syndrome and infections of grade 3 or 4, occurred in 5%, 1%, 2%, 8.9% and 18.8% of patients, respectively. On the close-out date 63.4% patients were still alive and with no signs of the disease. The 4-years follow-up survey showed that LRC, DFS, DSS and OS were 95.5%, 69.2%, 70.7%, and 66.2%, respectively. Higher pN-stage and splenectomy were found to be independent prognostic factors for all four types of survival and perineural invasion and lower treatment intensity for DFS, DSS and OS.

Treatment of condensation in sandwich panels without known vapour resistance of sealant

Povzetek: In order to calculate amount of interstitial condensation in a building envelope, water vapour resistance of each layer is of importance. Once having it, 1D calculation according to ISO 13788 with monthly average vapour pressures can be applied. In EN 14509 sandwich panels are considered to be impermeable for water vapour, thus (according to the standard) water vapour cannot enter from outside and condensate in the panels. But it is not always true for real sandwich panels, because joints between neighbouring panels can cause non-neglecting water vapour bridges. Although in measurements of linear water vapour transmittance of the joints (Ψv) stationary boundary vapour pressures can be applied, the measurements can be long lasting. We shortened time needed to get Ψv performing simulations in Delphin6.0. We simulated panels and steel sheets with joints using constant boundary vapour pressures and compared the results with the results of measurements on the equivalent systems. In systems under consideration a sealant in built-in-state, located at a joint of a sandwich panel, is a compressed EPDM tube. It is impossible to directly measure its effective μ according to ISO 12572. In the paper we study to which precision it is possible to determine it using measurements and simulations. Once having effective μ of the sealant (if all other necessary material parameters available) one can simulate condensation in envelopes including sandwich panels in 2D according to EN 15026 using hourly climatic data. Another goal of the study was determination of differences in resulting Ψv values when varying narrowest part of the gap dGAP at the joint in the panels without any sealant. Results confirm significant sensibility of Ψv to variations of dGAP.

Reviewing the modeling aspects and practices of shallow geothermal energy systems

Povzetek: Shallow geothermal energy systems (SGES) may take different forms and have recently taken considerable attention due to energy geo-structures (EGS) resulting from the integration of heat exchange elements in geotechnical structures. Still, there is a lack of systematic design guidelines of SGES. Hence, in order to contribute towards that direction, the current study aims at reviewing the available SGES modeling options along with their various aspects and practices. This is done by first presenting the main analytical and numerical models and methods related to the thermal behavior of SGES. Then, the most important supplementary factors affecting such modeling are discussed. These include: (i) the boundary conditions, in the form of temperature variation or heat flow, that majorly affect the predicted thermal behavior of SGES; (ii) the spatial dimensions that may be crucial when relaxing the infinite length assumption for short heat exchangers such as energy piles (EP); (iii) the determination of SGES parameters that may need employing specific techniques to overcome practical difficulties; (iv) a short-term vs. long-term analysis depending on the thermal storage characteristics of GHE of different sizes; (v) the influence of groundwater that can have a moderating effect on fluid temperatures in both heating and cooling modes. Subsequently, thermo-mechanical interactions modeling issues are addressed that may be crucial in EGS that exhibit a dual functioning of heat exchangers and structural elements. Finally, a quite lengthy overview of the main software tools related to thermal and thermo-hydro-mechanical analysis of SGES that may be useful for practical applications is given. A unified software package incorporating all related features of all SGES may be a future aim.

CT-guided percutaneous transthoracic needle biopsy of lung lesions - 2-year experience at the Institute of radiology in Ljubljana

Povzetek: Background. In 1883, Leyden described percutaneous lung biopsy, but it was notuntil 1970s that image guided fine needle chest biopsy gained widespread acceptance. Haaga and Alfidi reported CT-guided thoracic biopsy in 1976. Currently, tissue sampling of a thoracic lesion is indicated when the diagnosis is not obtained by the endobronchial technique and when the cytological diagnosis will modify the stage of the disease or influence the therapeutic strategy. Cytology obtained by small-gauge needle aspiration biopsy confirms the nature of the lesion in 80 - 95% of cases and carry a low incidence of major complications according to the literature. The purpose of this retrospective analysis was to provide basic data about diagnostic accuracy and incidence of pneumothorax and chest tube insertion with respect to percutaneous transthoracic CT-guided needle biopsy of lung lesions. Methods. After positioning of the patient we performed a spiral CT of the thorax with the accordingly placed metal mark, which helped us to set the optimal cutaneous entry point. After that we re-checked the localisation of the lesion and marked the entry point with a pen and clean the surface to keepit sterile. After we applied local anaesthetic subcutaneously, we used coaxial 18G Gallini aspiration biopsy needles with cutting tip for CT- guided aspiration cytologic examination. The length of the needle was chosen according to the distance of the targeted lesion. Results. From January 2005 to January 2007 forty-three patients - 24 men and 19 women who were 26-79 years old (mean +/- SD, 59.8 +/- 10 years) were referred to the Institute of Radiology to undergo the PTNB. One patient was referred twice. Consequently, the hospital records and images of 44 consecutive cases of percutaneous transthoracic fine needle aspiration biopsy procedure were retrospectively analysed. Radiofrequency ablation of lung tumours - new perspective in treatment of lung neoplasms

Povzetek: Background. Percutaneous radiofrequency ablation (RFA) is a minimally invasivetechnique used to treat solid tumours. Because of its ability to produce large volume of coagulation necrosis in controlled fashion this technique has been progressively tested as a possible treatment of lung malignancies. Recent clinical studies have shown that RFA enables successful treahnent of relatively small lung malignancies with high rate of complete response and acceptable morbidity and have suggested that the technique could represent a viable alternate or complementary method for patients with non-small cell lung cancer or lung metastases of favourable histotypes who arenot candidates for surgical resection. Conclusions. Initial intenational studies as well as the clinical experience of Institute of Radiology in Clinical Center Ljubljana, although limited, indicated that RFA is mostly welltolerated by patients and also, that it can result in complete necrosis oftargeted lesion. Pneumothorax is most common procedure related complication,occurring in up to 40% of cases, with approx. half of them requiring drainage.
