The contribution of new US technologies to US differential diagnosis of nonpalpable lesions

Povzetek: Background. Gray-scale ultrasound (US) patterns are still the best indicators of the risk of malignancy and correlation with mammography and guidance to bioptical procedures are still the gold standard in breast diagnosis. But recent technological advancements in ultrasound offer new diagnostic capabilities that integrate conventional US imaging: 3D, CAD, perfusion imaging and elastography. Conclusions. The US technologies allow to differentiate and grade the vascularity of breast lesions (both with conventional technologies and with contrast enhancers) and to evaluate the elastic properties of the normal and pathologic tissues (elastography). Both these technologies are on the way of becoming commer cially available on medium and high-end US instruments. But they must still be considered as research tools because their diagnostic efficacy requires more widely clinicaltesting.

Povzetek: A well-defined terminology of fire-related variables is important for correct analyses and supporting knowledge-based decisions regarding the evaluation of building fires at the European level. After developing an overview of current practices for fire statistics in Part I, the terminology used and the data collected by the EU Member States and eight other countries regarding fire incidents, property damage and human losses were mapped to increase awareness of their practice and support a comprehensive assessment of several fire statistical datasets. A questionnaire was distributed to relevant authorities responsible for the collection, elaboration/analysis, and fire statistical data publications to define and select the essential variables for an appropriate fire assessment and fire incident description. Based on the results of the questionnaire able to identify the essential fire statistical variables and a detailed analysis of current definitions adopted in the fire statistics of the EU Member States and other countries, a common terminology is proposed to collect the necessary data in the EU Member States and obtain meaningful datasets based on standardised terms and definitions. The results will generate essential outputs to move towards harmonised fire statistics at the EU level and contribute to an appropriate analysis able to improve fire prevention and fire mitigation in building fires.

Povzetek: The analysis of the current state of fire statistics and data collection in Europe and other countries is needed to increase awareness of how fire incidents affect buildings and to support pan-European fire prevention and fire mitigation mea- sures. The terminology and data collected regarding fire incidents in buildings in the EU Member States were mapped to obtain meaningful datasets to determine common terminology, collection methodology, and data interpretation system. An extensive literature review showed that fire data collection systems have been instrumental in informing firefighting strategies, evidence-based planning, prevention, and educational programmes. Differences and similarities between fire data collection systems were also investigated. The amount and quality of the information in fire statistical recording systems appear to be influenced by the complexity and structure with which the data are collected. The analysis also examined the existing fire statistics in the EU Member States and a few other countries. Finally, a detailed investigation of the number of fires, fire deaths, and injuries from 2009 to 2018 in several countries was examined based on data from a report by CTIF. The trends showed differences attributable to the existing fire statistical practices in terms of terminology and data collection, and interpretation. Part II proposes a common terminology for selected fire statistical variables. The results provide relevant information regarding fire safety at the European level and should be used to guide the development of more uniform fire statistics across Europe.

Effects of specific parameters on simulations of energy use and air temperatures in offices equipped with radiant heating/cooling panels

Povzetek: When creating a simulation model to assess the performance of buildings, there is usually a lack of feedback information. Only in the case of measurements of a real building is a direct comparison of the measured values and simulated results possible. Parameter data related to users’ behavior or other events can also be obtained. Their evaluated frequency, magnitude and duration, along with boundary conditions, are crucial for the results. It is clear that none of them can be predicted very accurately. Most of them, however, are needed for computer modeling. In this paper we analyzed the well-defined TRNSYS simulation model of offices equipped with radiant ceiling panels for heating and cooling. The model was based on real case offices and was validated based on measurements for 1 year. The analysis included simulations in order to define what effect the parameters related mainly to users have on the energy use and the indoor air temperatures. The study confirmed that specific human activities influence the annual energy use to a relatively small degree and that their effects often counteract. It also confirmed the even more important fact that although small, these activities can influence the thermal comfort of users. It is believed that despite the fact that this research was based on an analysis of offices equipped with radiant ceiling panels, most of the results could be applied generally.

Influence of the size and type of pores on brick resistance to freeze-thaw cycles

Povzetek: This paper estimates the frost resistance of bricks using the ratio of compressive strength before freezing to compressive strength after freezing to describe the damage degree of bricks being exposed to freeze-thaw cycles. In an effort to find the ratio that clearly distinguishes resistant bricks from non-resistant bricks, the authors attempted to establish the correlation between the ratio and Maage factor as a recognized model for assessing brick resistance. To clarify the degree of damage of individual bricks, the pore size distribution has been investigated by means of mercury porosimetry. Additionally, micro computed X-ray tomography (micro-CT) has been employed to define the influence of the type of pores (open or closed) and their connectivity on the frost resistance of bricks. According to the results, it can be concluded that there is a good correlation between the Maage factor and the ratio of pre- to post-freeze-thaw cycle compressive strengths, and that the latter ratio strongly correlates with the percentage of large pores (≥3 mm) in the brick. If such a correlation could be confirmed in a larger sample, then the ratio of pre- to post-freeze-thaw cycle compressive strengths could be used as a new method for assessing brick resistance to freeze-thaw cycles and it would be possible to determine the minimum percentage of large pores required to ensure the overall resistance of brick to freeze-thaw conditions. The complexity of the problem is, however, evidenced by the fact that no clear connection between the type (open versus closed) or connectivity of pores and the frost resistance of bricks could be revealed by micro-CT.

Povzetek: Leta 2019 so bila objavljena Priporočila za obravnavo bolnikov s pljučnim rakom, ki so v slovenski prostor vnesla prepotrebno poenotenje diagnostike in zdravljenja z namenom izboljšanja preživetja bolnikov s pljučnim rakom. Posodobitev Priporočil tri leta po izidu izvirnika prinaša največ novosti v poglavju o sistemskem zdravljenju bolnikov s pljučnim rakom. To kaže na izjemen napredek na področju razumevanja onkogeneze in biologije pljučnega raka ter s tem razvoja novih zdravil. Breme pljučnega raka ostaja veliko, saj je pljučni rak pri nas in v svetu še vedno najpogostejši vzrok smrti zaradi raka. Za vsako peto smrt zaradi raka je odgovoren pljučni rak. Skoraj tretjina bolnikov s pljučnim rakom ne prejme specifičnega onkološkega zdravljenja, bodisi zaradi slabega stanja zmogljivosti, spremljajočih bolezni ali obsega bolezni. Polovica bolnikov ima ob diagnozi razsejano bolezen, zaradi česar izboljšanje preživetja z malimi koraki sledi napredku v zdravljenju bolnikov s pljučnim rakom. Ti podatki nas opominjajo, da se bomo morali za velike premike v obravnavi bolnikov s pljučnim rakom lotiti drugačnih pristopov. Kot najbolj obetavno se ponuja zgodnje odkrivanje bolezni, ko so možnosti ozdravitve pljučnega raka najboljše. Zapisana Priporočila so usmeritev za obravnavo bolnikov s pljučnim rakom. Le s sodobnim multidisciplinarnim pristopom obravnave lahko bolniku ponudimo zdravljenje, ki mu omogoča najboljši izhod prognostično neugodne bolezni.

Fizikalni modeli imunoterapije in radioterapije

Povzetek: V prispevku bomo predstavili rezultate fizikalnih modelov imunoterapije, ki smo jih razvili v programski skupini Medicinska fizika na Fakulteti za matematiko in fiziko Univerze v Ljubljani. Glavni cilj raziskav je bila prepoznava bioloških značilnosti tumorjev, od katerih je odvisen odziv na zdravljenje s protitelesi receptorja programirane celične smrti l (angl. anti-programmed-death-1 (anti-PD-1)). Posebno pozornost smo namenili meritvam modelskih parametrov ter preverbi rezultatov modeliranja, kar je eden izmed predpogojev za uspešno translacijo modeliranja v rutinsko predklinično in klinično prakso. Predstavili bomo tudi načrte za prihodnost, tj. modeliranje kombinacije anti-PD-1 in radioterapije.
