Cardiotoxicity of concomitant radiotherapy and trastuzumab for early breast cancer

Abstract: Background. Trastuzumab therapy given in combination with one of several chemotherapy regimens is currently considered the standard of care for the treatment of early-stage, human epidermal growth factor receptor-2 (HER2) -positive breast cancer. The treatment with trastuzumab is due to a significant impact on the survival part of the standard adjuvant treatment of patients with HER2-positive breast cancer. Patients treated with postoperative breast or chest wall irradiation receive trastuzumab concomitant with radiotherapy. In a small proportion of patients trastuzumab causes cardiotoxicity. Preclinical findings indicate a radiosensibilizing effect of trastuzumab in breast cancer cells, but it is not yet clear whether it radiosensibilizes cells of healthy tissues too.Conclusions. Special attention is required when left breast or left thoracic wall is irradiated in patient receiving trastuzumab, because long-term effects of the concurrent treatment with trastuzumab and radiotherapy are not yet known. In an era where more patients are surviving a diagnosis of breast cancer, better understanding and earlier detection of therapy-induced cardiac toxicity will be of paramount importance.

Incidental uptake of 18F-fluorocholine (FCH) in the head or in the neck of patients with prostate cancer

Abstract: Background. Positron emission tomography-computed tomography (PET/CT) with 18F-fluorocholine (FCH) is routinely performed in patients with prostate cancer. In this clinical context, foci of FCH uptake in the head or in the neck were considered as incidentalomas, except for those suggestive of multiple bone metastases. Case reports. In 8 patients the incidental focus corresponded to a benign tumour. The standard of truth was histologyin two cases, correlative imaging with MRI in four cases, 99mTc-SestaMIBI scintigraphy, ultrasonography and biochemistry in one case and biochemistry including PTH assay in one case. The final diagnosis of benign tumoursconsisted in 3 pituitary adenomas, 2 meningiomas, 2 hyperfunctioning parathyroid glands and 1 thyroid adenoma. Malignancy was proven histologically in 2 other patients: 1 papillary carcinoma of the thyroid and 1 cerebellar metastasis.Conclusions. To the best of our knowledge, FCH uptake by pituitary adenomas or hyperfunctioning parathyroidglands has never been described previously. We thus discuss whether there might be a future indication for FCH PET/CT when one such tumour is already known or suspected: to detect a residual or recurrent pituitary adenoma aftersurgery, to guide surgery or radiotherapy of a meningioma or to localise a hyperfunctioning parathyroid gland. Inthese potential indications, comparative studies with reference PET tracers or with 99mTc-sestaMIBI in case of hyperparathyroidism could be undertaken.

Consequence of the introduction of routine FCH PET/CT imaging for patients with prostate cancer : a dual centre survey

Abstract: Background. Fluorocholine(18F) (FCH) was introduced at the beginning of April 2010 in France, Slovenia and three other EU member states for the localisation of bone metastases of prostate cancer with PET. The aim of the study was to compare the evolution of diagnostic imaging in patients with prostate cancer using a new radiopharmaceutical FCH, observed in France and in Slovenia, and to quantify the consequence of the results of new imaging modality on the detection rate of abnormal metastases and recurrences of prostate cancer.Patients and methods. In two centres (France/Slovenia), a survey of the number of nuclear medicine examinations in patients with prostate cancer was performed, covering 5 quarters of the year since the introduction of FCH. For each examination, the clinical and biological circumstances were recorded, as well as the detection of bone or soft tissue foci.Results. Six hundred and eighty-eight nuclear medicine examinations were performed impatients with prostate cancer. Nuclear medicine examinations were performed for therapy monitoring and follow-up in 23% of cases. The number of FCH PET/CT grew rapidly between the 1st and 5th period of the observation (+220%), while the number of bone scintigraphies (BS) and fluoride(18F) PET/CTs decreased (-42% and -23% respectively). Fluorodeoxyglucose(18F) (FDG) PET/CT remained limited to few cases of castrate-resistant or metastatic prostate cancer in Paris. The proportion of negative results was significantly lower with FCH PET/CT (14%) than with BS (49%) or fluoride(18F) PET/CT (54%). For bone metastases, the detection rate was similar, but FCH PET/CT was performed on average at lower prostate-specific antigen (PSA) levels and was less frequently doubtful (4% vs. 28% for BS). FCH PET/CT also showed foci in prostatic bed (53% of cases) or in soft tissue (35% of cases).Conclusions. A rapid development of FCH PET/CT was observed in both centres and led to a higher detection rate of prostate cancer lesions.

6.
Hardware–software co-design of an audio feature extraction pipeline for machine learning applications

Abstract: Keyword spotting is an important part of modern speech recognition pipelines. Typical contemporary keyword-spotting systems are based on Mel-Frequency Cepstral Coefficient (MFCC) audio features, which are relatively complex to compute. Considering the always-on nature of many keyword-spotting systems, it is prudent to optimize this part of the detection pipeline. We explore the simplifications of the MFCC audio features and derive a simplified version that can be more easily used in embedded applications. Additionally, we implement a hardware generator that generates an appropriate hardware pipeline for the simplified audio feature extraction. Using Chisel4ml framework, we integrate hardware generators into Python-based Keras framework, which facilitates the training process of the machine learning models using our simplified audio features.

Seismogenic depth and seismic coupling estimation in the transition zone between Alps, Dinarides and Pannonian Basin for the new Slovenian seismic hazard model

Abstract: Seismogenic depth and seismic coupling are important inputs into seismic hazard estimates. Although the importance of seismic coupling is often overlooked, it significantly impacts seismic hazard results. We present an estimation of upper and lower seismogenic depth and expected hypocentral depth and seismic coupling in the transition zone between the Alps, Dinarides and Pannonian Basin, characterized by a complex deformation pattern, highly variable crustal thickness, and moderate seismic hazard, supporting the development of the 2021 seismic hazard model of Slovenia. The hazard model was based on three seismic source models: area source model, fault source model and smoothed seismicity (point) source model. We estimated the lower seismogenic depth using seismological and geological data and compared them. The seismological estimate was based on two regional earthquake catalogues prepared for this study. In the area source model, estimates of lower seismogenic depth from seismological data are deeper or equal to the ones derived from geological data, except in one case. In the fault source model, we analysed each fault individually and chose seismological lower depth estimates in 12 among 89 faults as more representative. The seismogenic thickness for each individual fault source was determined for seismic coupling determination. The seismic coupling was assessed by two approaches, i.e. we chose the most trusted value from the literature, and the value determined for each fault individually by using the approach based on the updated regional fault and earthquake data sets. The final estimate of seismic coupling ranges from 0.77 to 0.38. We compared the tectonic moment rate based on long-term slip rate using different values of seismic coupling with the seismic moment rate obtained from the earthquake catalogue. The analysis is done for the whole area, as well as for the individual area zones. The analysis of N–S components of estimated slip for the largest faults in the area of west Slovenia shows that the regional geological and geodetic shortening rates are comparable. The total activity rate of three global seismic source models is compared, which gives up to a 10 % difference. Our results contribute to a better understanding of the seismic activity in the region. The presented approach for seismic coupling estimation can be applied in cases where the total slip rate is given instead of its seismic part and can be used at regional or national level. The approach is also suitable for the cross-border harmonization of the European seismic hazard modelling data.

Reflexivity of the space of transversal distributions

Abstract: For any smooth, Hausdorff and second-countable manifold $N$ one can define the Fréchet space ${\mathcal C}^{\infty}(N)$ of smooth functions on $N$ and its strong dual ${\cal E}'(N)$ of compactly supported distributions on $N$. It is a standard result that the strong dual of ${\cal E}'(N)$ is naturally isomorphic to ${\mathcal C}^{\infty}(N)$, which implies that both ${\mathcal C}^{\infty}(N)$ and ${\cal E}'(N)$ are reflexive locally convex spaces. In this paper we generalise that result to the setting of transversal distributions on the total space of a surjective submersion $\pi : P\to M$. We show that the strong ${\mathcal C}^{\infty}_c(M)$-dual of the space ${\cal E}'_{\pi} (P)$ of $\pi$-transversal distributions is naturally isomorphic to the ${\mathcal C}^{\infty}_c(M)$-module ${\mathcal C}^{\infty}(P)$.

Locally convex bialgebroid of an action Lie groupoid

Abstract: Action Lie groupoids are used to model spaces of orbits of actions of Lie groups on manifolds. For each such action groupoid $M\rtimes H$ we construct a locally convex bialgebroid $\mathord{\mathrm{Dirac}}(M\rtimes H)$ with an antipode over $\mathord{\mathcal{C}^{\infty}_{c}}(M)$, from which the groupoid $M\rtimes H$ can be reconstructed as its spectral action Lie groupoid $\mathord{\mathcal{AG}_{\mathit{sp}}}(\mathord{\mathrm{Dirac}}(M\rtimes H))$.
