Carrier screening and pregnancy

Povzetek: Recessive genetic conditions impose a significant burden, often leading to severe childhood disorders, many of which remain untreatable. It is estimated that 1–2 % of couples are at risk of having an affected child in the general population, with the risk being significantly higher in consanguineous couples. Understanding the increased risk of having a child with a recessive disorder empowers prospective parents to make informed reproductive choices. With technological advancements, genetic screening has evolved beyond identifying only a few common conditions. Expanded carrier screening (ESC) now offers a single test that covers a comprehensive list of recessive disorders, addressing those that contribute most significantly to the burden of these conditions within specific populations. ESC is recommended for all couples planning a pregnancy, with particular emphasis on consanguineous couples or those who are subfertile. To ensure responsible use of ESC, clinical service delivery should adopt a multidisciplinary approach, providing couples with the information they need to make voluntary, informed decisions. This includes access to high-quality genetic testing, genetic counseling, and psychosocial support. National professional societies and governments play a crucial role in shaping guidelines, policies, oversight, and funding to guarantee equitable access to high-quality ESC services.

Influence of selected cognitive performances on musculoskeletal injury occurrence in adult male professional Slovenian PrvaLiga football players in a prospective cohort study

Povzetek: Evidence suggests athletes with lower baseline cognitive performance are at higher risk of musculoskeletal injuries. This prospective cohort study investigates basic and executive cognitive functions in predicting injuries in 78 professional male football (soccer) players from four Slovenian f irst league teams. Data were collected during the 2018/2019 winter break, and injuries recorded in the second half of that and the 2019/2020 season. Cognitive functions assessed by computerized tests (PsyToolkit) included psychomotor vigilance (simple, choice reaction time) and visuospatial memory (Corsi-block-tapping-test), while pen-and-paper tests assessed motor speed, visual scanning, and executive functions (TMT; Delis-Kaplan-Executive-Function-System). Forty-two players sustained at least one musculoskeletal injury (9 contact injuries), 36 remained injury-free. Logistic regression analyses indicated that none of the cognitive measures significantly influenced injury occurrence (p > 0.05). However, non-significant trends were observed for the TMT ratio score (p = 0.065, OR = 1.64), reflecting cognitive flexibility, and for TMT-A (p = 0.05, OR = 0.60), reflecting visual scanning. Specifically, players with lower cognitive flexibility showed a 64% increase in the odds of sustaining an injury, while better visual scanning performance was associated with a 40% reduction in injury odds. No significant association was found between basic or executive cognitive functions and musculoskeletal injuries in professional male football players. However, a non-significant trend suggested that lower cognitive flexibility may be associated with increased injury risk. These findings underscore the need for larger studies to better clarify the role of executive functions in assessing injury risk in football.

Breast cancer risk assessment and risk distribution in 3,491 Slovenian women invited for screening at the age of 50 : a population-based cross-sectional study

Povzetek: Background. The evidence shows that risk-based strategy could be implemented to avoid unnecessary harm in mammography screening for breast cancer (BC) using age-only criterium. Our study aimed at identifying the uptake of Slovenian women to the BC risk assessment invitation and assessing the number of screening mammographies in case of risk-based screening.Patients and methods. A cross-sectional population-based study enrolled 11,898 women at the age of 50, invited to BC screening. The data on BC risk factors, including breast density from the first 3,491 study responders was col-lected and BC risk was assessed using the Tyrer-Cuzick algorithm (version 8) to classify women into risk groups (low, population, moderately increased, and high risk group). The number of screening mammographies according to risk stratification was simulated. Results. 57% (6,785) of women returned BC risk questionnaires. When stratifying 3,491 women into risk groups, 34.0% were assessed with low, 62.2% with population, 3.4% with moderately increased, and 0.4% with high 10-year BC risk. In the case of potential personalised screening, the number of screening mammographies would drop by 38.6% com-pared to the current screening policy. Conclusions. The study uptake showed the feasibility of risk assessment when inviting women to regular BC screen-ing. 3.8% of Slovenian women were recognised with higher than population 10-year BC risk. According to Slovenian BC guidelines they may be screened more often. Overall, personalised screening would decrease the number of screening mammographies in Slovenia. This information is to be considered when planning the pilot and assessing the feasibility of implementing population risk-based screening.
