Analysis of ATP7A expression and ceruloplasmin levels as biomarkers in patients undergoing neoadjuvant chemotherapy for advanced high-grade serous ovarian carcinomaDavid Lukanović, Sara Polajžer, Miha Matjašič, Borut Kobal, Katarina Černe, 2024, izvirni znanstveni članek Povzetek: Ovarian cancer (OC), particularly high-grade serous carcinoma (HGSC), is a leading cause of gynecological cancer mortality due to late diagnosis and chemoresistance. While studies on OC cell lines have shown that overexpression of the ATP7A membrane transporter correlates with resistance to platinum-based drugs (PtBMs) and cross-resistance to copper (Cu), clinical evidence is lacking. The functionality of ceruloplasmin (CP), the main Cu-transporting protein in the blood, is dependent on, among other things, ATP7A activity. This study investigated ATP7A expression and CP levels as potential biomarkers for predicting responses to PtBMs. We included 28 HGSC patients who underwent neoadjuvant chemotherapy (NACT). ATP7A expression in ovarian and peritoneal tissues before NACT and in peritoneal and omental tissues after NACT was analyzed via qPCR, and CP levels in ascites and plasma were measured via ELISA before and after NACT. In total, 54% of patients exhibited ATP7A expression in pretreatment tissue (ovary and/or peritoneum), while 43% of patients exhibited ATP7A expression in tissue after treatment (peritoneum and/or omentum). A significant association was found between higher ATP7A expression in the peritoneum before NACT and an unfavorable CA-125 elimination rate constant k (KELIM) score. Patients with omental ATP7A expression had significantly higher plasma mean CP levels before NACT. Plasma CP levels decreased significantly after NACT, and higher CP levels after NACT were associated with a shorter platinum-free interval (PFI). These findings suggest that the ATP7A transporter and CP have the potential to serve as predictive markers of chemoresistance, but further research is needed to validate their clinical utility. 2. Tumor-derived extracellular vesicles (EVs) that can be isolated from ascites and blood of HGSC patients are such promising biomarkers. Epithelial cell adhesion molecule (EpCAM) expression is upregulated in most epithelium-derived tumors; however, studies on prognostic value of EpCAM overexpression in ovarian carcinoma have shown contradictory results. The aim of our study was to evaluate the potential of total and EpCAM-positive EVs as prognostic and predictive biomarkers for advanced HGSC. Flow cytometry was used to determine the concentration of total and EpCAM-positive EVs in paired pretreatment ascites and plasma samples of 37 patients with advanced HGSC who underwent different first-line therapy. We found that higher EpCAM-positive EVs concentration in ascites is associated with shorter progression-free survival (PFS) regardless of treatment strategy. We also found a strong correlation of EpCAM-positive EVs concentration between ascites and plasma. Our findings indicate that EpCAM-positive EVs in ascites of patients with advanced HGSC have the potential to serve as prognostic biomarkers for predicting early recurrence and thereby likelihood of more aggressive tumor biology and development of chemoresistance. 4. It usually presents late with non-specific symptoms and most cases are diagnosed at an advanced stage, with 5-year overall survival being around 40%. Biomarkers for screening and early diagnosis of this aggressive disease are, thus, a research priority. Extracellular vesicles (EVs) that reflect the cell of origin and that can be isolated from local fluid and plasma by minimally invasive liquid biopsy are such promising biomarkers. Besides EV concentration and molecular profile, which have been the main focus of research for many years, recent studies have also called attention to EV size distribution. The aim of our study was to evaluate the potential of EV concentration and size distribution in local fluid and plasma as diagnostic biomarkers for HGSC. Methods: Paired pretreatment ascites and plasma samples from 37 patients with advanced HGSC and paired pretreatment free peritoneal fluid (FPF) and plasma samples from 40 controls with benign ovarian pathology (BOP) were analyzed using nanoparticle tracking analysis (NTA). Results: We observed a significant difference in EV concentration in local fluid, but not in plasma, between HGSC patients and the control group. We also found a significant difference in EV size distribution in both local fluid and plasma between HGSC patients and the control group. The receiver operating characteristics (ROC) curve analysis of EV characteristics showed excellent diagnostic performance for the mode, D10, and D50 in local fluid and acceptable diagnostic performance for EV concentration and mean EV size in local fluid, as well as for the mode and D10 value in plasma. Conclusions: The results of our study show that EV concentration in local fluid and more importantly EV size distribution in both local fluid and plasma are significantly changed in the presence of HGSC. Future research of size-dependent molecular profiling of EVs could help identify novel diagnostic biomarkers for HGSC. The KELIM model, derived from CA-125 kinetics, is a promising biomarker for predicting chemosensitivity. Thymidine kinase 1 (TK1), a proliferation marker, has shown relevance in various cancers but its role in chemotherapy response for OC is unclear.Methods: In this retrospective study, we assessed the association between TK1 protein (TK1p) and enzymatic activity (TK1a) and chemosensitivity (KELIM), platinum-free interval (PFI), and chemotherapy response score (CRS) in 28 patients with epithelial OC treated with platinum-based chemotherapy. Biomarker dynamics were measured at multiple timepoints. KELIM was calculated using CA-125 kinetics; relationships with CRS and PFI were evaluated.Results: KELIM demonstrated robust predictive performance (correlating with favorable CRS [rho = 0.731, p = 0.011] and longer PFI [rho = 0.437, p = 0.007]). TK1p and TK1a showed no significant correlations with KELIM, PFI, or CRS. ROC analysis for preoperative TK1p yielded an AUC of 0.6941, indicating moderate discriminative potential. TK1a increased postoperatively but lacked predictive value for chemoresistance.Conclusion: Our findings reinforce the value of KELIM as a reliable predictor of platinum sensitivity in OC. TK1 dynamics reflect tumor proliferation but did not significantly predict chemotherapy response. Larger cohorts and further research are required to explore whether TK1 can complement established biomarkers. 6. A novel serum-based steroid-protein panels for differentiating ovarian cancer from non-malignant adnexal massesMarija Gjorgoska, Boštjan Pirš, Špela Smrkolj, Tea Lanišnik-Rižner, 2025, izvirni znanstveni članek Povzetek: Background: Ovarian cancer is the deadliest gynecological malignancy, largely due to the advanced stage at diagnosis in most patients. This study investigates whether systemic steroids can serve as biomarkers to distinguish malignant ovarian tumors from non-malignant adnexal masses. Methods: This prospective, single-center observational study included 99 women with adnexal masses who underwent surgery between December 2021 and February 2025. Preoperative serum levels of 17 steroid hormones, including androgens, 11-oxyandrogens, glucocorticoids, and mineralocorticoids, were quantified using liquid chromatography-tandem mass spectrometry (LC-MS/MS). Machine learning was employed to assess the diagnostic potential of these steroids in distinguishing ovarian cancer (n = 43) from non-malignant adnexal masses (n = 56). Results: Patients with ovarian cancer had lower levels of 11β-hydroxy-testosterone (11OHT), 11-keto-testosterone (11KT), and testosterone compared to controls. Using stepwise feature selection, we developed two diagnostic models incorporating three 11-oxyandrogens (11KT, 11OHT, and 11β-hydroxy-androstenedione), patient age, and either cancer antigen 125 (CA-125) or human epididymis protein 4 (HE4) for distinguishing malignant from non-malignant adnexal masses. The model including CA-125 achieved AUC of 0.907, 88.9% sensitivity and 82.0% specificity, while the model including HE4 achieved AUC of 0.911, 94.4% sensitivity and 77.3% specificity as evaluated by cross-validation. Both models significantly outperformed CA-125, HE4, and the Risk of Ovarian Malignancy Algorithm (ROMA) index alone. Conclusion: Patients with ovarian cancer exhibit distinct steroid profiles compared to those with non-malignant adnexal masses. If validated, the models could enhance diagnosis, reducing unnecessary surgeries for benign conditions while ensuring timely treatment for ovarian cancer, particularly when conventional biomarkers are inconclusive. However, it is also an endocrine-disrupting chemical that can negatively affect human health. It is not yet known whether it is associated with the development of epithelial ovarian cancer (EOC), a severe and highly fatal human disease. Therefore, the purpose of this study was to determine the concentrations of BPA in the urine of women with EOC or epithelial borderline ovarian tumors (EBOTs) using gas chromatography tandem mass spectrometry (GC-MS/MS) and find their possible associations with kidney function at the molecular level, urine and blood biochemical parameters related to metabolism, anti-Müllerian hormone (AMH) (a marker of ovarian reserve/fertility), and lifestyle habits determined via a questionnaire in comparison to healthy controls. The results suggest that the unadjusted or urine-specific-gravity-adjusted BPA levels were significantly increased in women with EOC/EBOT. The unadjusted BPA was significantly positively associated with urinary creatinine (p = 0.007) in all women with EOC/EBOT after adjustment for age, body mass index, and pregnancy using multiple linear regression analysis. This may be related to kidney injury. However, no association was found between urinary BPA and serum AMH levels in women. Women with ovarian malignancies were more exposed to plastic products for storing foods and drinks. Some lifestyle habits, including refilling plastic bottles, correlate with higher urinary BPA levels across the entire cohort of women. When considering EOC or EBOT, it is necessary to consider the potential higher exposure of women to BPA, as reflected in their urine and lifestyle habits. Thisstudy employed a high-resolution mass spectrometry-basednontargeted screening approach, integrating polar (HILIC) andreversed-phase separations to expand the chemical space coverageand, supported by open-science tools and resources, evaluatedurinary chemical profiles to assess internal EDC exposure. Among106 annotated biomarkers of exposure, six exhibited significantlyhigher normalized intensities in patients with ovarian malignanciescompared to healthy controls (p < 0.05). This suggests theirgreater exposure to phthalates (diethylhexyl phthalate and diethylphthalate), pesticides (metolachlor metabolite and 4-nitrophenol), a UV filter (benzophenone-1), and an industrial byproduct (4-methyl-2-nitrophenol). These compounds may interfere with hormonal regulation, potentially contributing to cancer development.While these findings highlight potential differences in internal EDC exposure, the study primarily demonstrates the applicability ofnontargeted urinary profiling for chemical exposure assessment. In advanced epithelial ovarian cancer (EOC), the peritoneum is the primary site of disease recurrence which occurs in >75% of patients despite complete cytoreductive surgery (CRS) and chemotherapy. Macroscopically undetectable remaining cancer cells are deemed to be a source for recurrent disease. We investigated characteristics of occult disease in biopsies of macroscopically normal peritoneum during CRS.
Materials and methods
We included 14 patients with advanced stage high grade serous ovarian cancer (HGSOC). Eleven patients had received neoadjuvant chemotherapy (NACT) and three patients were chemotherapy naïve. Each patient underwent three study-related peritoneal biopsies: 1) of a metastasis, 2) adjacent to a metastasis and 3) at distance from metastases. Cryostat sections were immunohistochemically stained for PAX8 and PanCK as markers of EOC cells and for CD31 as a marker for vascular and lymphatic endothelium. The sections were analyzed semi-quantitatively.
Results
Macroscopically normal peritoneum showed solitary PAX8-positive cells adjacent to and at distance from metastases in all patients. Thirteen percent of these PAX8-positive cells were found to be attached to the mesothelium and are presumably spread through intra-abdominal fluid. Eighty-seven percent of the solitary PAX8-positive cells were found in the stroma underneath the mesothelium, of which 59% were firmly attached to endothelium and 33% were found in the stroma. In most cases, no sign of proliferation of the solitary cells was observed. Only a few clusters of PAX8-positive cells were found. Chemotherapy did not affect these results.
Conclusions
Solitary PAX8-positive cells are present in the macroscopically healthy-looking peritoneum of all EOC patients investigated, irrespective of the distance to macroscopically-visible metastases and of previous treatment. The majority of these solitary cancer cells were attached to endothelium of capillaries, venules or lymphatic vessels. Their solitary character and lack of proliferation suggests a dormant state, which could explain why these cells are unaffected by neo-adjuvant chemotherapy.
