1. Fetal magnetocardiography using optically pumped magnetometers : a literature reviewRok Hren, Urban Marhl, Tamás Dóczi, Erika Országh, Vojko Jazbinšek, Tilmann Sander, 2026, pregledni znanstveni članek Povzetek: Fetal magnetocardiography (fMCG) provides direct non-invasive assessment of fetal cardiac electrophysiology, enabling detailed evaluation of cardiac rhythm, conduction, and repolarization. However, the clinical adoption of conventional fMCG has been limited by its reliance on superconducting quantum interference device (SQUID) systems, which require cryogenic cooling and specialized infrastructure. Optically pumped magnetometers (OPMs) have emerged as a promising cryogen-free alternative with the potential to broaden access to fetal electrophysiological assessment. This review summarizes the technological evolution and early clinical evaluation of OPM-based fMCG through an analysis of original in vivo human studies published up to June 2026. Twelve eligible studies were identified and synthesized narratively. Advances in sensor design, magnetic shielding, acquisition strategies, and signal-processing algorithms have enabled SQUID-comparable signal quality and cardiac interval measurements while substantially reducing cryogenic and infrastructure requirements. OPM-fMCG has demonstrated the potential to assess fetal cardiac time intervals, heart rate variability, fetal movement, and clinically important arrhythmias, including congenital long QT syndrome, atrioventricular block, and supraventricular and ventricular tachyarrhythmias. However, the available evidence remains dominated by small, single-centre studies, with relatively few fetuses affected by clinically significant arrhythmias. Prospective multicenter clinical validation, protocol standardization, independent replication, and regulatory evaluation are therefore required before OPM-fMCG can be integrated into routine diagnostic pathways for pregnancies requiring advanced fetal electrophysiological assessment. Robson, 2024, izvirni znanstveni članek Povzetek: The pattern electroretinogram (PERG) is a localized retinal response evoked by a contrast-reversing pattern, usually a black and white checkerboard, which provides information about macular and retinal ganglion cell function. This document, from the International Society for Clinical Electrophysiology of Vision (ISCEV; www.iscev.org) presents an updated and revised Standard for clinical PERG testing. This replaces the 2013 and all earlier versions. Minimum protocols for basic PERG stimuli, recording methods and reporting are specified, to promote consistency of methods for diagnosis and monitoring purposes, while responding to evolving clinical practices and technology. The main changes in the updated ISCEV Standard for clinical PERG include expanded guidance about large stimulus fields, stimulus parameters for simultaneous PERG and pattern visual evoked potential recording, baseline drift correction, and use of consistent ambient room lighting. Robson, 2025, pregledni znanstveni članek Povzetek: Visual evoked potentials (VEPs) are electrophysiologic responses to pattern or flash stimuli, recorded over the occiput. VEPs can provide information regarding the function of the visual system and are valuable in the diagnosis and investigation of optic nerve disease or post-retinal visual pathway dysfunction. The ISCEV VEP Standard specifies stimulus and recording conditions for three basic types of recording: (1) Pattern-reversal VEPs elicited by checkerboard stimuli with large 1° (degree) and small 0.25° check widths. (2) Pattern onset/offset VEPs elicited by checkerboard stimuli with large 1° and small 0.25° check widths. (3) Flash VEPs elicited by a flash which subtends a visual field of at least 20°. The ISCEV VEP Standard protocols are defined for a single recording channel with a midline occipital active electrode. Multi-channel VEPs for evaluation of chiasmal and post-chiasmal lesions, together with protocols specific for pediatric populations, are also described in this document as non-standardized additions. The main changes in the updated ISCEV Standard for clinical VEP include an option to perform a simultaneous pattern electroretinogram (PERG) and pattern-reversal VEP recording, a revised definition of the origin and the analysis of the most prominent VEP components, and more precise descriptions of non-standard multi-channel and pediatric VEP recordings, intended to encourage convergence of widely used non-standard methods. These changes aim to provide a clinically relevant document about current practice which will facilitate good quality recordings and inter-laboratory comparisons. 5. Midseptal and anteroseptal accessory pathway ablation in childrenLászló Környei, Matevž Jan, Mohammad Ebrahim, Vjekoslav Radeljić, Mirta Rode, 2024, izvirni znanstveni članek Povzetek: Objectives: The goal of this study is to document outcomes of ablation for high-risk accessory pathways in paediatrics using 3D mapping systems with minimal to zero fluoroscopy. Methods: It is a cross-sectional, multicentre study, conducted between 2013 and 2023, and involving four different centres in Hungary, Croatia, Kuwait, and Slovenia. Results: A total of 128 procedures were performed on 111 patients. The cohort included 57.8% anteroseptal (AS) pathways and 42.2% midseptal (MS) pathways. The mean follow-up time was 2.0 ± 2.1 years. Cryoablation was used in 72.7% of the cases, and radiofrequency ablation was used in 27.3%. The EnSite Precision™ Cardiac Mapping System was the predominant system used. The overall acute success rate was 89.1%, with recurrence rates at 17.2% with similar results regardless of the type of energy used. The success rate was not significantly different between AS and MS substrates. The age and weight of the patient had no bearing on the outcomes (median age and weight were 13 years and 52 kg, respectively). The complications rate was at 4.69% and included transient AV block (three patients), hematoma (one patient), right bundle branch block (one patient), and possible permanent complete AV block (one patient). Fluoroscopy was utilized in 18 cases, with a fluoroscopy time mean of 3 min and 45 s. Conclusions: MS and AS AP in paediatric patients can be treated effectively with either RF or cryoablation and with a low dose of radiation using 3D mapping systems, with excellent acute success rates and low complication rates. 6. Methods: We provide detailed analysis of the clinical examinations of a male patient with bilateral optic neuropathy from the acute stage to 8 years of follow-up. Complete ophthalmological exam, electrophysiology and optical coherence tomography (OCT) segmentation were performed. The genotype analysis was performed with a complete screening of the mitochondrial genome. Furthermore, proteomic analysis of the protein structure and function was performed to assess the pathogenicity of a novel variant of unknown significance. Mitochondrial function analysis of the patient’s peripheral blood mononuclear cells (PBMCs) was performed with the objective of evaluating the mutation effect on mitochondrial function using flow cytometry and high-resolution respirometry. Results: The patient had a profound consecutive bilateral visual loss at 19 years of age due to optic neuropathy with characteristics of LHON; however, unlike patients with typical LHON, the patient experienced a fluctuation in visual function and significant late recovery. He had a total of three visual acuity deteriorations and improvements in the left eye, with concomitant visual loss in the right eye and a final visual acuity drop reaching nadir 9 months after onset. The visual loss was characterized by centrocecal scotoma, abnormal color vision and abnormal VEP, while deterioration of PERG N95 followed with a lag of several months. The OCT examination showed retinal nerve fiber layer thinning matching disease progression. Following a two-year period of legal blindness, the patient’s visual function started to improve, and over the course of 5 years, it reached 0.5 and 0.7 Snellen (0.3 and 0.15 LogMAR) visual acuity (VA). Mitochondrial sequencing identified a presumably pathogenic variant m.15309T>C in the MT-CYB gene at 65% heteroplasmy, belonging to haplogroup K. Mitochondrial function assessment of the patient’s PBMCs showed a lower respiration rate, an increase in reactive oxygen species production and the presence of mitochondrial depolarization, compared to an age- and sex-matched healthy control’s PBMCs. Conclusions: A novel variant in the MT-CYB:m.15309T>C (Ile188Thr) gene was identified in a patient with optic nerve damage and the LHON phenotype without any additional systemic features and atypical presentation of the disease with late onset of visual function recovery. The pathogenicity of the variant is supported by proteomic analysis and the mitochondrial dysfunction observed in the patient’s PBMCs. This study aimed to test how a longer disuse period affects MU properties, NMJ transmission, and NMJ morphology remodeling. Nine young healthy males (age: 18‐29 years) underwent 21 days of horizontal bed rest. Pre‐ (BR0) and post‐bed rest (BR21), quadriceps maximal voluntary contraction (MVC), and size were assessed. We combined intramuscular electromyography (iEMG) and high‐density surface electromyography (HDsEMG) recordings on the vastus lateralis to assess MU properties at 25% and 50% of MVC. Muscle biopsies and blood samples were also collected. Quadriceps MVC and size decreased at BR21. We found alterations in MU properties at both contraction intensities, including reduced discharge rate, MU potential area changes, and increased complexity. NMJ transmission was found to be reduced at BR21 at 25% MVC. This functional NMJ impairment was biochemically corroborated by an increase in serum C‐terminal agrin fragment concentration, a biomarker of NMJ instability. In addition, a direct assessment of NMJ morphology revealed the presence of some denervated NMJs exclusively at BR21. In conclusion, 21‐day bed rest altered MU properties across different contraction intensities and impaired NMJ transmission with initial signs of remodeling/denervation. Disuse duration appears to be a critical factor, as previous shorter studies failed to detect some of these changes. We believe these findings are clinically relevant for disuse after trauma, surgery, or illness, and may support the development of effective countermeasures.
