Stratigos, 2023, izvirni znanstveni članek Povzetek: Background: The public perception of dermatology has been poorly investigated in Europe. Objective: To determine the general public's perception of dermatologists in Europe. Methods: This multinational, cross-sectional study was conducted within the framework of the EADV population-based survey on the 'Burden of skin diseases in Europe'. Data were collected using a web-based online survey on a representative sample of the general populations aged 18 years or more of 27 European countries. Proportional quota sampling with replacement design was used for each country. Results: A total of 44,689 adult participants responded to the questionnaire, of whom 18,004 visited a dermatologist in the preceding 3 years. The dermatologist was the second most often visited specialist among all medical specialties, with 69.7%, 72.1%, 42.1% and 78.1% of participants in Western Europe (WE), Eastern Europe (EE), Northern Europe (NE) and Southern Europe (SE), respectively, having consulted a dermatologist over the past 24 months. Most participants across all regions agreed that the dermatologist was the first healthcare provider for chronic skin diseases (61.9% in WE, 69.8% in EE, 45.7% in NE and 60.4% in SE) and for skin cancers (65.5% in WE, 67.6% in EE, 42.4% in NE and 63.0% in SE). The five most common reasons for visiting a dermatologist among all participants were: naevi check-up or skin cancer screening (20.2%), chronic skin diseases (16.5%), acute skin diseases (12.4%), cosmetic advice or procedure (10.2%), hyperpigmentation or congenital lesions (9.1%) and hair or nail disorder (7.7%). Most participants (84.6% in WE, 82.5% in EE, 78.3% in NE and 82.8% in SE) were very swatisfied/somewhat satisfied with the service provided. Conclusion: Our study findings underscore the central role of dermatologists in skin health and highlights them as valued and trusted care providers across Europe. Understanding the perceived position of the dermatologist is the first step in improving health policy development and implementation. Notably, access to a dermatologist was lowest in NE, probably reflecting differences in healthcare organizational structures or possibly cultural differences in healthcare seeking behaviour. 2. Assessing Europe’s policy readiness to confront the MASLD/MASH public health threat : Chronic liver disease in EuropeJeffrey Victor Lazarus, Trenton M. White, Leire Agirre-Garrido, Kushala W. M. With an estimated 30.4% prevalence among the adult population in the European Union and United Kingdom, it is the most common chronic liver disease and is closely linked to obesity and type 2 diabetes, which can be considered as in- dicator conditions. Despite the scale of the burden, the spectrum of MASLD, including its advanced form, metabolic dysfunction-associated steatohepatitis (MASH), is weakly represented—or even absent—from health policy and development agendas. Further, the extent to which regional and national clinical practice guidelines (CPGs) on other liver diseases and related fields incorporate information on MASLD/MASH prevention or management is unclear. In this Series paper, we reviewed policy attention to MASLD/MASH across the European Union and the United Kingdom. We report low policy readiness across European countries to address MASLD/MASH. Only two (7.1%), Romania and Sweden, had a subnational strategy, while 13 (46.4%) referenced MASLD/MASH in other national or subnational action plans, predominantly those for liver disease and obesity. No country included MASLD/MASH in its priority non-communicable disease list. Additionally, 15 (53.6%) countries had national MASLD/MASH CPGs, while 25 (89.3%) mentioned MASLD/MASH in other national CPGs, most often those for obesity, diabetes, cirrhosis, liver transplantation, and primary care, respectively. We conclude that MASLD/MASH remains insuffi- ciently addressed from a policy perspective, which contributes to low readiness to address this public health threat. 3. Spiroplasma ixodetis, an emerging tick-associated pathogen previously linked mainly to congenital cataracts, has not been well characterised in adults. We investigated the prevalence and clinical features of S. ixodetis infection in adults with acute febrile illness without localisation. Methods: Shotgun metagenomic sequencing identified S. ixodetis in the initial 209 patient cohort and the sequences were used to developed a novel real-time PCR assay targeting the 23S rRNA gene. Initial cohort screening was followed by testing 128 patients from an additionally selected targeted cohort. Positive results were confirmed by sequencing of 16S and 23S rRNA genes. Results: S. ixodetis DNA was confirmed in 7.2% patients from the initial and in 35.2% patients from the additional cohort (60 in total). All were identified in the period from April to October and 57% reported a recent tick-bite. Clinical presentation was homogenous, characterised by fever, headache, bicytopenia and liver enzyme abnormalities. Outcomes were favourable, with 15% requiring hospitalisation. Conclusion: This study identifies S. ixodetis as a previously unrecognised cause of adult febrile illness without localisation, bridging the gap between previously published data between tick studies and isolated human case reports. 5. Population-based screening for cardiovascular diseases and type 2 diabetes risk among migrant origin and ethnic minority groups in Europe : a scoping reviewSinna M. H. Although some migrant origin groups in Europe show elevated risk for cardiovascular diseases (CVDs) and type 2 diabetes (T2D), information is sparse and fragmented. This study examines availability and possibilities for harmonization of health examination survey (HES) data on major determinants of CVD and T2D among migrant origin and ethnic minority groups in EU Member states and EU4Health associated countries (Norway, Iceland, Ukraine, Moldova, Montenegro).
Methods
We conducted a scoping review, following PRISMA-ScR in PubMed and Web of Science, supplemented by targeted grey literature searches of national health institutes, to identify HESs covering core risk factors for CVD and T2D among migrant origin and ethnic minority groups, older than 18 years and living in EU Member states and selected associated countries. Studies were published between 2014 and 2026. Altogether 2537 peer-reviewed records and 348 grey literature reports were screened. Following full-text screening, 57 peer-reviewed papers were included.
Results
In total, 24 individual HESs in 10 countries were identified. The majority (n = 38, 67%) of the surveys were conducted in the Netherlands or Sweden. None of the HESs used nationally representative samples, having been conducted regionally. There was notable heterogeneity in how persons with a migration background and ethnic minorities were defined and grouped. Reported risk factors included obesity, hypertension, hypercholesterolemia, and hyperglycemia. Measurement methods of these risk factors varied by the migrant origin group and country where the study was conducted.

6. Serological response after COVID-19 infection compared to vaccination against COVID-19 in children with autoimmune rheumatic diseasesTjaša Šinkovec Savšek, Mojca Zajc Avramovič, Tadej Avčin, Miša Korva, Tatjana Avšič-Županc, Nataša Toplak, 2024, izvirni znanstveni članek Povzetek: Background Paediatric patients with autoimmune rheumatic diseases (pARD) have a dysregulated immune system, so infections present a major threat to them. To prevent severe COVID-19 infections we aimed to vaccinate them as soon as possible. Studies have shown that the BNT162b2 vaccine is safe, effective, and immunogenic, however, in a short observation period, only. Methods The main objective was to compare the serological response between three groups of pARD: after SARSCoV-2 infection, after vaccination against COVID-19 with two doses of the BNT162b2 vaccine, and after experiencing both events. Data on demographics, diagnosis, therapy, and serology (anti-SARS-CoV-2 IgG/IgA) were collected from March 2020 to April 2022. For statistical analysis ANOVA, Mann-Whitney U test, Chi-square test and Fisher’s exact test were applied. To compare adverse events (AE) after vaccination we included a control group of healthy adolescents. Results We collected data from 115 pARD; from 92 after infection and 47 after vaccination. Twenty-four were included in both groups. Serological data were available for 47 pARD after infection, 25 after vaccination, and 21 after both events. Serological response was better after vaccination and after both events compared to after infection only. No effect of medication on the antibody levels was noted. The safety profile of the vaccine was good. Systemic AE after the first dose of the vaccine were more common in healthy adolescents compared to pARD. In the observation period of 41.3 weeks, 60% of vaccinated pARD did not experience a symptomatic COVID-19 infection. Conclusions IgG and IgA anti-SARS-CoV-2 levels were higher after vaccination and after both events compared to after infection only. Six months after vaccination we observed an increase in antibody levels, suggesting that pARD had been exposed to SARS-CoV-2 but remained asymptomatic. 7. Acrocyanosis : primary or secondary form?Salvino Bilancini, Massimo Lucchi, Giusto Trevisan, Luigi Di Pino, Sandro Tucci, 2025, izvirni znanstveni članek Povzetek: Introduction: Acrocyanosis is the most common form of angiodystonic vascular acrosyndrome, characterized by functional microcirculatory alterations without structural vessel damage. It is traditionally classified as either primary or secondary, the latter often associated with underlying conditions. Methods: Some observations suggest a frequent association between acrocyanosis and connective tissue diseases (CTDs). To investigate this, we conducted a study on 53 patients diagnosed with acrocyanosis: 45 females and eight males, 15 to 82 years old, with a mean age of 35 years. Secondary acrocyanosis was identified in 24 patients (45.3%). Results: Advanced age (≥ 40 years) was a significant risk factor for secondary acrocyanosis (relative risk = 2.5, 95% confidence interval: 1.4–4.5, p = 0.002). No significant differences were observed between sexes. CTDs were the most common conditions associated with acrocyanosis (32% of the study population and 71% of the secondary forms). Conclusions: Although generally considered benign, acrocyanosis may indicate an underlying systemic disease. Clinical examination remains essential for the diagnosis of acrocyanosis. Our findings reveal a high prevalence of secondary acrocyanosis associated with CTDs. Patients with strong clinical suspicion should be referred to specialized centers for capillaroscopy and antinuclear antibody testing. 8. Methods: This was a registry-based observational prospective study. All subjects with jBD from the Autoinflammatory Diseases Alliance (AIDA) Network BD Registry showing ocular manifestations before 18 years were enrolled. Results: We included 27 of 1000 subjects enrolled in the registry (66.7% male patients, 45 affected eyes). The median (interquartile range [IQR]) age at ocular involvement was 14.2 (4.7) years. Uveitis affected 91.1% of eyes (anterior 11.1%, posterior 40.0%, panuveitis 40.0%), retinal vasculitis 37.8% and other manifestations 19.8%. Later onset (p = 0.01) and male predominance (p = 0.04) characterized posterior involvement. Ocular complications occurred in 51.1% of eyes. Patients with complications had earlier onset (p < 0.01), more relapses (p = 0.02) and more prolonged steroidal treatment (p = 0.02). The mean (standard deviation [SD]) central macular thickness (CMT) at the enrolment and last visit was 302.2 (58.4) and 293.3 (78.2) μm, respectively. Fluorescein angiography was pathological in 63.2% of procedures, with a mean (SD) Angiography Scoring for Uveitis Working Group (ASUWOG) of 17.9 (15.5). At the last visit, ocular damage according to the BD Overall Damage Index (BODI) was documented in 73.3% of eyes. The final mean (SD) best corrected visual acuity (BCVA) logMAR was 0.17 (0.47) and blindness (BCVA logMAR < 1.00 or central visual field ≤ 10°) occurred in 15.6% of eyes. At multivariate regression analysis, human leukocyte antigen (HLA)-B51 + independently predicted a + 0.35 change in the final BCVA logMAR (p = 0.01), while a higher BCVA logMAR at the first assessment (odds ratio [OR] 5.80; p = 0.02) independently predicted blindness. Conclusions: The results of this study may be leveraged to guide clinical practice and future research on this rare sight-threatening condition. 9. Incidence of symptomatic Lyme borreliosis in nine European countriesFrederick J. Methods: Data from seroprevalence studies and estimates of the symptomatic proportion and duration of antibody detection in Bbsl-infected individuals, derived from reviews of the published literature, were used to adjust public health LB surveillance data to estimate the incidence of symptomatic Bbsl infection in nine European countries from 2018 to 2022. Results: The prevalence of anti-Bbsl antibodies ranged from 2.3% in Romania to 9.4% in Germany. Under- detection multipliers varied across surveillance systems; using 10-year duration of antibody detection, multipliers were 2.4-10.5 in countries reporting all LB cases and 54.6-722.2 in countries reporting only Lyme neuroborreliosis cases. The incidence of symptomatic Bbsl infection adjusted for under-detection was highest in Finland, Germany, Norway, Poland, and Switzerland, intermediate in the Czech Republic and Denmark, and lowest in Ireland and Romania. Conclusion: Adjustment of LB surveillance for under-detection found a high incidence of symptomatic Bbsl infection in several European countries. Differences in LB surveillance systems should be considered when comparing surveillance data between countries and when estimating LB disease burden. 10. Furuncular myiasis caused by Dermatobia hominis in a Slovenian travelerMark Sergej Bartenjev, Jan Stanič, Mateja Dolenc-Voljč, Igor Bartenjev, 2025, drugi znanstveni članki Povzetek: A case of furuncular myiasis caused by the larva of Dermatobia hominis is described, involving the ankle region of a 35-year-old woman that had returned from a vacation in Colombia. After removal of the larva from the skin, the lesion healed in 2 weeks. Due to an increasing rate of intercontinental travel, dermatologists in Slovenia may need to consider Dermatobia myiasis in the differential diagnosis of furuncular lesions in patients with a relevant travel history.
