Age-related effects of COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI : results of the ISACS-STEMI COVID-19 registry

Povzetek: Background: The constraints in the management of patients with ST-segment elevation myocardial infarction (STEMI) during the COVID-19 pandemic have been suggested to have severely impacted mortality levels. The aim of the current analysis is to evaluate the age-related effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI within the registry ISACS-STEMI COVID-19. Methods: This retrospective multicenter registry was performed in high-volume PPCI centers on four continents and included STEMI patients undergoing PPCI in March–June 2019 and 2020. Patients were divided according to age (< or ≥75 years). The main outcomes were the incidence and timing of PPCI, (ischemia time longer than 12 h and door-to-balloon longer than 30 min), and in-hospital or 30-day mortality. Results: We included 16,683 patients undergoing PPCI in 109 centers. In 2020, during the pandemic, there was a significant reduction in PPCI as compared to 2019 (IRR 0.843 (95%-CI: 0.825–0.861, p < 0.0001). We found a significant age-related reduction (7%, p = 0.015), with a larger effect on elderly than on younger patients. Furthermore, we observed significantly higher 30-day mortality during the pandemic period, especially among the elderly (13.6% vs. 17.9%, adjusted HR (95% CI) = 1.55 [1.24–1.93], p < 0.001) as compared to younger patients (4.8% vs. 5.7%; adjusted HR (95% CI) = 1.25 [1.05–1.49], p = 0.013), as a potential consequence of the significantly longer ischemia time observed during the pandemic. Conclusions: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures, with a larger reduction and a longer delay to treatment among elderly patients, which may have contributed to increase in-hospital and 30-day mortality during the pandemic.

Measuring biological age : insights from omics studies

Povzetek: Biological ageing is a systemic, multifactorial process driven by progressive molecular and cellular alterations whose complexity necessitates systems-level approaches. Advances in high-throughput omics technologies now allow simultaneous quantification of millions of biomolecules from a single specimen, enabling longitudinal, integrative profiling across multiple molecular layers. This review synthesizes recent progress in applying genomics, epigenomics, metabolomics and microbiomics to ageing research, highlighting their contributions to biomarker discovery, mechanistic insight, and translational opportunities. Genomic studies reveal genetic variants that promote extreme longevity, while epigenetic clocks provide robust predictors of biological age. The blood proteome can be used to calculate proteome-based scores and evaluate temporal changes in ageing trajectories in an organ- and sex-specific manner. Metabolomic signatures identify key metabolites reflecting ageing trajectories, and microbiome research demonstrates that gut microbial composition mirrors and modulates biological ageing, with microbiome clocks emerging. The omics approaches have further elucidated the impact of exercise and diet providing evidence that interventions can reduce biological age. The integration of multi-omics with clinical and lifestyle data, powered by machine learning and artificial intelligence, is paving the way for a holistic definition of biological age and the development of personalized healthy ageing strategies. This review highlights how the omics technologies and computational modelling are transforming ageing biology into strategies for personalized healthy ageing.

The effect of dietary intake of coenzyme Q10 on skin parameters and condition : results of a randomised, placebo-controlled, double-blind study

Povzetek: Coenzyme Q10 (CoQ10) is a natural constituent of foods and is also often used in both functional foods and supplements. In addition, it is a common ingredient of cosmetics where it is believed to reduce the signs of skin ageing. However, the existing data about the effect of dietary intake of CoQ10 on skin parameters and condition are scarce. To gain an insight into this issue, we conducted a double-blind, placebo-controlled experiment with 33 healthy subjects. Our objective was to investigate the effects of 12 weeks of daily supplementation with 50 and 150 mg of CoQ10 on skin parameters and condition. Study was conducted with a water-soluble form of CoQ10 with superior bioavailability (Q10VitalVR ). While the results of some previous in vitro studies showed possible protection in UVB response, we did not observe significant changes in the minimal erythema dose (MED). On the other hand, the intake of CoQ10 limited seasonal deterioration of viscoelasticity and reduced some visible signs of ageing. We determined significantly reduced wrinkles and microrelief lines, and improved skin smoothness. Supplementation with CoQ10 did not significantly affect skin hydration and dermis thickness.

The effect of a 12-week dietary intake of food supplements containing collagen and MSM on dermis density and other skin parameters : A double-blind, placebo-controlled, randomised four-way study comparing the efficacy of three test products

Povzetek: Dietary supplementation with collagen has been gaining popularity as an anti-ageing remedy. The aim of this randomised, double-blind placebo-controlled four-way study was to investigate and compare the effects of 12-week daily dietary supplementation with products containing 10 g hydrolysed fish collagen (HC) or a combination of 5 g or 10 g HC with 1.5 g methylsulphonylmethane (MSM) on dermis density in healthy human subjects. The secondary objectives were to investigate the effects on a variety of other skin parameters. The study results showed improved dermis density, skin texture and reduced wrinkles for all the active products. However, products containing MSM were superior in the improvement of skin thickness and roughness, and a higher dose of HC combined with MSM was crucial for improvement in hydration. On the other hand, no significant effects of the supplementation on viscoelasticity and transepidermal water loss were determined with any of the products.

Neuromuscular junction instability with inactivity : morphological and functional changes after 10 days of bed rest in older adults

Povzetek: The neuromuscular junction (NMJ) plays a key role in modulating muscle contraction,but the impact of short-term disuse on NMJ structure and function, particularly in older humans,remains unclear. This study aimed to investigate NMJ alterations following 10 days of horizontalbed rest in 10 older males (68.5 ± 2.6 years). Before and after bed rest, vastus lateralis musclebiopsies were obtained to evaluate NMJ morphology, intramuscular EMG (iEMG) was recordedto assess NMJ function and blood samples were collected to determine circulating C-terminal agrinfragment (CAF) concentration, a biomarker of NMJ remodelling. In a sub-cohort of six participantswho had NMJs in both pre- and post-bed rest biopsies, we observed altered NMJ morphology,including reduced overlap between NMJ terminals, as well as increased endplate area and peri-meter. CAF concentration was elevated after bed rest, suggesting ongoing NMJ remodelling. iEMGanalysis showed increased motor unit potential complexity and reduced firing rate. In addition,we observed impaired NMJ transmission, inferred from increased near-fibre jiggle and segmentjitter. These findings suggest that older male individuals are susceptible to NMJ remodelling andimpaired transmission with short-term disuse, providing valuable insights into the morphologicaland functional consequences of inactivity in an ageing population. Our study highlights theimportance of developing interventions for mitigating the detrimental consequences of inactivityon neuromuscular health in older adults, which they frequently experience following injury, trauma,illness or surgery.

Relationship between Mediterranean diet adherence and body composition parameters in older adults from the Mediterranean region

Povzetek: Abstract: In recent decades, the rapid spread of various communication media has led to changes in traditional eating habits. In the Mediterranean region, the classic (Mediterranean) dietary pattern has been lost as a result. This has led to a shift in eating habits towards unhealthy eating patterns, which in turn has resulted in an inadequate distribution of body composition. It is known that, among other things, the number of non-communicable diseases increases with the inadequate distribution of body composition. The aim of our study was to examine the level of adherence to the Mediterranean diet (MD) of older adults in the Mediterranean region in relation to specific body composition parameters. This study included 521 older adults with a mean age of 69.6 ± 6.3 years. Body composition was measured using the BIA 101 Anniversary device (Akern s.r.l., Florence, Italy) and adherence to the MD was assessed using the MEDLIFE index questionnaire. This study found significant differences in body composition between males and females. The mean adherence to the MD was 17.0 ± 3.3 points among the participants and there was higher adherence in females (p = 0.002, ηp 2 = 0.019). A multiple linear regression was performed to assess the relationship between the body composition parameters and MD. Multiple linear regression models were significant for reactance, fat mass (%), fat-free mass (%), skeletal muscle index, and total body water (%), with specific individual MEDLIFE items such as the consumption of processed meat, meat, white meat, fruit, vegetables, olive oil and limiting snacks between meals. Moreover, promising correlations were found between certain MD characteristics and BIA parameters, but the overall health effects of the MD remain unclear.

Neuromuscular component of muscle quality assessment in older adults : narrative review

Povzetek: The concept of muscle quality encompasses both microand macroscopic aspects of muscle architecture and composition and has gained increasing attention with inclusion in the definition of sarcopenia, indicating the significance of muscle quality in evaluating muscle function and strength among older individuals. Muscle quality consists of two main components: neuromuscular and morphological and is often defined as the ratio between the two. The aim of this review is to present currently used methods for assessment of muscle quality with an emphasis on neuromuscular component in older adults. The most used methods for assessing morphological component are imaging techniques, such as magnetic resonance imaging, computed tomography, dualenergy X-ray absorptiometry and non-imaging bioimpedance analysis. In the neuromuscular component upper and lower body strength are assessed with different methods such as hand grip strength, isokinetic lower limb strength and isometric lower limb strength. Currently, there are three proposed muscle quality assessment methods for field or population studies: muscle quality index, ultrasound sarcopenia index and bioimpedancederived phase angle. Despite the exploration of muscle quality through various assessment methods, a consensus on the most appropriate and universally applicable approach has yet to be established.
