Fetal treatment and long-term neonatal outcomes in severe maternal red cell alloimmunization : a single-centre experience

Povzetek: Objectives: Haemolytic disease of the fetus and newborn (HDFN) occurs due to maternal IgG alloantibodies that actively cross the placenta and bind to paternally derived fetal antigens on the erythrocytes. The aims of this study were to describe the Slovenian cohort of patients with severe HDFN, who required fetal treatment, to review the fetal treatment strategies, and to describe pregnancy and neurodevelopmental outcomes. Case series presentation: Data on patients who developed severe HDFN between 2006 and 2021 and were treated at our institution were collected retrospectively. Primary care pediatricians were contacted regarding neurodevelopmental outcomes of surviving infants. There were 19 pregnancies affected with severe HDFN. The most commonly implicated antigen was RhD. Seventeen children were liveborn. Sixteen fetuses were treated with intrauterine transfusion (IUT). Two children had developmental delay at the corrected age of 2 years. Conclusions: In this study, the Slovenian national cohort of severe cases of HDFN is described for the first time. Prevalence of RhD alloimmunization was higher in comparison to the literature. A combined treatment with therapeutic plasmapheresis, immunoglobulins and IUT was successful. Three quarters of newborns were born in the late preterm period. Overall survival rate and long-term neonatal adverse outcomes in our cohort were in line with the literature.

Serum vitamin D and inflammatory markers in SARS-CoV-2 positive pregnant women

Povzetek: Objectives: The SARS-CoV-2 pandemic was declared by World Health Organisation (WHO) in March 2020, causing health and economic disruptions and millions of deaths. Pregnant women represent a vulnerable group, as COVID-19 during pregnancy increases the risk of preterm birth, preeclampsia, and severe maternal illness. Nutritional status, such as vitamin D deficiency, may influence these outcomes, yet data on its status in the cohort of SARS-CoV-2 positive pregnant women as well as its association with inflammatory and angiogenic markers is scarce. The aim of this study was to evaluate the levels of vitamin D in the cohort of SARSCoV- 2 positive pregnant women and its association with inflammatory and angiogenic markers. Methods: Prospective cohort study at Ljubljana Maternity Hospital (Oct 1, 2020–Mar 30, 2021) enrolled singleton pregnancies with PCR-confirmed SARS-CoV-2 within the last 14 days, delivering at our institution. Results: Among 235 SARS-CoV-2-positive pregnant women 62.1% had adequate and 21.3% insufficient level of vitamin D and 13.6%were vitaminDdeficient. Statistical analysis revealed no significant correlations between 25-OH-vitamin D and Creactive protein (CRP), procalcitonin (PCT), leukocyte count, soluble fms-like tyrosine kinase-1 (sFlt-1), placental growth factor (PlGF), the sFlt-1/PlGF ratio, or body mass index (BMI). Conclusions: Our prospective cohort study revealed that high proportion of pregnant women has inadequate levels of vitamin D. Although maternal insufficiency is linked to adverse outcomes, its association with inflammatory and angiogenic markers remains unclear. Rigorous studies in pregnancy are essential to clarify vitamin D’s role in COVID-19 complications in pregnancy.

Clinical factors in SARS-CoV-2 antibody response in unvaccinated mothers

Povzetek: Objectives: Understanding the clinical factors influencing the SARS-CoV-2 antibody response during and after pregnancy is critical for optimizingmaternal care and vaccination strategies. This prospective cohort study aimed to evaluate associations between maternal clinical characteristics and SARS-CoV-2-specific IgG and IgA antibody levels at delivery and 42 days postpartum in unvaccinated pregnant women. Methods: A total of 387 pregnant women with confirmed SARS-CoV-2 infection during pregnancy were included. SARSCoV- 2 infectionwas confirmed using real-time RT-PCR. Clinical data, including age, body mass index (BMI), smoking status, pre-existing morbidities, and obstetric complications, were recorded. SARS-CoV-2-specific IgG and IgA antibodies were quantified using ELISA at delivery and 42 days postpartum. Results: Higher preconception BMI significantly correlated with increased odds of detecting IgG and IgA antibodies at both delivery and postpartum assessments (p<0.05), independently ofmaternal age and chronic diseases.Women without chronic systemic diseases exhibited lower antibody levels at delivery, whereas smokers had significantly lower odds of IgG antibody presence at delivery. Additionally, preexisting cardiovascular diseases were associated with reduced antibody presence at six weeks postpartum. Other clinical parameters did not show significant associations. Conclusions: Preconception BMI and pre-existing systemic diseases may modulate SARS-CoV-2 antibody responses in pregnant women. These clinical factors should inform assessments of maternal and neonatal infection risks and guide vaccination strategies in pregnant populations. Further research is needed to elucidate the mechanisms underlying these associations.

The role of regulatory T cells and their therapeutic potential in hypertensive disease of pregnancy : a literature review

Povzetek: Abstract: Hypertensive disorders of pregnancy (HDP), including preeclampsia (PE) and gestational hypertension (GH), are major causes of maternal and foetal morbidity and mortality. This review elucidates the role of regulatory T cells (Tregs) in the immunological aspects of HDP and explores their therapeutic potential. Tregs, which play a critical role in maintaining immune homeostasis, are crucial in pregnancy to prevent immune-mediated rejection of the foetus. The review highlights that Tregs contribute to immunological adaptation in normal pregnancy, ensuring foetal acceptance. In contrast, HDP is associated with Treg dysfunction, which is marked by decreased numbers and impaired regulatory capacity, leading to inadequate immune tolerance and abnormal placental development. This dysfunction is particularly evident in PE, inwhich Tregs fail to adequatelymodulate the maternal immune response against foetal antigens, contributing to the pathophysiology of the disorder. Therapeutic interventions aiming to modulate Treg activity represent a promising avenue for HDP management. Studies in animal models and limited clinical trials suggest that enhancing Treg functionality could mitigate HDP symptoms and improve pregnancy outcomes. However, given the multifactorial nature of HDP and the intricate regulatory mechanisms of Tregs, the review explores the complexities of translating in vitro and animal model findings into effective clinical therapies. In conclusion, while the precise role of Tregs in HDP is still being unravelled, their central role in immune regulation during pregnancy is indisputable. Further research is needed to fully understand the mechanisms by which Tregs contribute to HDP and to develop targeted therapies that can safely and effectively harness their regulatory potential for treating hypertensive diseases of pregnancy.

Routine 36-week scan : diagnosis and outcome of abnormal fetal presentation

Povzetek: Objectives: First, to report the incidence of non-cephalic presentation at a routine 36-week ultrasound scan, the uptake and success of external cephalic version (ECV) and the incidence of spontaneous rotation from non-cephalic to cephalic presentation. Second, to determine the maternal and pregnancy characteristics that provide a significant contribution to the prediction of non-cephalic presentation at the 36-week scan, successful ECV from non-cephalic to cephalic presentation and spontaneous rotation from non-cephalic to cephalic presentation. Methods: This was a retrospective analysis of prospectively collected data from 107 875 women with a singleton pregnancy who had undergone a routine ultrasound scan at 35 + 0 to 36 + 6 weeks' gestation. Patients with breech or transverse/oblique presentation were divided into two groups: those scheduled for elective Cesarean section for a fetal or maternal indication other than abnormal presentation, and those that would potentially require ECV. The latter group was reassessed after 1-2 weeks and, if the abnormal presentation persisted, the parents were offered ECV or elective Cesarean section at 38-40 weeks' gestation. Multivariable logistic regression analysis was carried out to determine which maternal and pregnancy characteristics provided a significant contribution in the prediction of non-cephalic presentation at the 36-week scan, successful ECV from non-cephalic to cephalic presentation and spontaneous rotation from non-cephalic to cephalic presentation. Results: At the 36-week scan, fetal presentation was cephalic in 101 664 (94.2%) pregnancies and either breech, transverse or oblique in 6211 (5.8%). In 0.3% of cases with cephalic presentation at the 36-week scan, there was subsequent spontaneous rotation to non-cephalic presentation, and in half of these, the diagnosis was made during labor or at birth. ECV was attempted in 1584/6211 (25.5%) pregnancies with non-cephalic presentation at the 36-week scan and was successful in only 44.1% of cases. In the remaining 74.5% of cases, ECV was not attempted because of any of the following reasons: ECV was declined; Cesarean section was planned for a reason other than abnormal presentation; ECV was planned for the subsequent 1-2 weeks but, in the meantime, there was spontaneous rotation to cephalic presentation; or there was spontaneous onset of labor or rupture of membranes before planned ECV. In 5513/6211 (88.8%) pregnancies with non-cephalic presentation at the 36-week scan, ECV was not attempted or was unsuccessful, and in 37.7% of these, there was subsequent spontaneous rotation to cephalic presentation. Among the 6211 pregnancies with non-cephalic presentation at the 36-week scan, the presentation at birth was cephalic in 43.8%; in 74.8%, this was due to spontaneous rotation, and in 25.2%, it was due to successful ECV. Multivariable analysis demonstrated that the likelihood of non-cephalic presentation at the 36-week scan, that of successful ECV and that of spontaneous rotation from non-cephalic to cephalic presentation was affected by several maternal and pregnancy characteristics, but the predictive performance for these events was poor, with the area under the receiver-operating-characteristics curve ranging from 0.608 to 0.717 and the detection rate at a 10% false-positive rate ranging from 19.0% to 33.7%. Conclusions: Routine ultrasound examination at 35 + 0 to 36 + 6 weeks' gestation could improve pregnancy outcome by substantially reducing the risk of unexpected abnormal presentation in labor. However, an additional ultrasound scan for fetal presentation should be considered in all women when they present in labor.

At mid-gestation, markers of placental function rather than maternal cardiac function are stronger determinants of birthweight

Povzetek: Background: The role of maternal cardiac and hemodynamic assessment during normal and complicated pregnancies has gained attention during the last few years. Some researchers suggested that the manifestation of complications in pregnancy suffering from impaired placentation is mainly driven by preexisting cardiac changes, identifiable at an early stage by echocardiographic and hemodynamic assessment. It is therefore of great importance to determine the link between placental perfusion and maternal cardiac function and hemodynamics. Also, the impact of maternal cardiac function on birth weight has not been thoroughly studied. Objective: To elucidate the possible association of maternal cardiovascular indices with placental perfusion at mid-gestation and birthweight. Study design: Prospective study on women with singleton pregnancies attending Kings' College Hospital, London, UK for a routine hospital visit at 19 to 24 weeks of gestation. We recorded maternal characteristics and medical history, measured mean arterial pressure, heart rate, uterine artery pulsatility index, umbilical artery pulsatility index, middle cerebral artery pulsatility index, and serum placental growth factor. We also performed maternal echocardiogram to assess cardiac output and peripheral vascular resistance as well as indices of diastolic and systolic cardiac function. Multivariable regression modeling was used. Results: Our cohort included 4006 women. Higher uterine artery pulsatility index values were associated with lower mean arterial pressure, heart rate, and left ventricular systolic function, after adjustment for maternal characteristics and subsequent development of hypertensive disorders of pregnancy and gestational diabetes mellitus. In a multivariable approach that explained 17.9% of the variance of the birthweight, we found that some cardiovascular indices provided small but significant contribution to the model after accounting for maternal factors and development of hypertensive disorders of pregnancy and gestational diabetes mellitus, uterine artery pulsatility index, and placental growth factor. Conclusion: The findings of our study indicate a weak but significant association between maternal cardiovascular indices with placental perfusion at mid-gestation and birthweight. Our data would not support routine maternal cardiovascular assessment for predicting birthweight.
