COVID-19 pandemic impact on cytopathology practice in the post-lockdown period : an international, multicenter study

Abstract: Background. In a previous worldwide survey, the authors showed a drastic reduction in the number of cytological specimens processed during the coronavirus disease 2019 “lockdown” period along with an increase in malignancy rates. To assess the continued impact of the pandemic on cytological practices around the world, they undertook a second follow-up worldwide survey collecting data from the post-lockdown period (2020). Methods. Participants were asked to provide data regarding their cytopathology activity during the first 12 weeks of their respective national post-lockdown period (2020), which ranged from April 4 to October 31. Differences between the post-lockdown period and the corresponding 2019 period were evaluated, and the authors specifically focused on rates of malignant diagnoses. Results. A total of 29 respondents from 17 countries worldwide joined the survey. Overall, a lower number of cytological specimens (n = 236,352) were processed in comparison with the same period in 2019 (n = 321,466) for a relative reduction of 26.5%. The overall malignancy rate showed a statistically significant increase (12,442 [5.26%] vs 12,882 [4.01%]; P < .001) during the same time period. Similar results were obtained if both malignancy and suspicious for malignancy rates were considered together (15,759 [6.58%] vs 16,011 [4.98%]; P < .001). Conclusions. The data showed a persistent reduction in the cytological specimen volume during the post-lockdown period (2020). However, the relative increase in the cytological workload in the late part of the post-lockdown is a promising finding of a slow return to normality.

Covid-19 pulmonary pathology : the experience of European pulmonary pathologists throughout the first two waves of the pandemic

Abstract: Autoptic studies of patients who died from COVID-19 constitute an important step forward in improving our knowledge in the pathophysiology of SARS-CoV-2 infection. Systematic analyses of lung tissue, the organ primarily targeted by the disease, were mostly performed during the first wave of the pandemic. Analyses of pathological lesions at different times offer a good opportunity to better understand the disease and how its evolution has been influenced mostly by new SARS-CoV-2 variants or the different therapeutic approaches. In this short report we summarize responses collected from a questionnaire survey that investigated important pathological data during the first two pandemic waves (spring-summer 2020; autumn-winter 2020–2021). The survey was submitted to expert lung pathologists from nine European countries involved in autoptic procedures in both pandemic waves. The frequency of each lung lesion was quite heterogeneous among the participants. However, a higher frequency of pulmonary superinfections, both bacterial and especially fungal, was observed in the second wave compared to the first. Obtaining a deeper knowledge of the pathological lesions at the basis of this complex and severe disease, which change over time, is crucial for correct patient management and treatment. Autoptic examination is a useful tool to achieve this goal.

International consensus on lung function testing during COVID-19 pandemic and beyond

Abstract: COVID-19 has negatively affected the delivery of respiratory diagnostic services across the world due to the potential risk of disease transmission during lung function testing. Community prevalence, reoccurrence of COVID-19 surges, and the emergence of different variants of the SARS-CoV-2 virus have impeded attempts to restore services. Finding consensus on how to deliver safe lung function services for both patients attending and for staff performing the tests are of paramount importance. This international statement presents the consensus opinion of 23 experts in the field of lung function and respiratory physiology balanced with evidence from the reviewed literature. It describes a robust roadmap for restoration and continuity of lung function testing services during the COVID-19 pandemic and beyond. Important strategies presented in this consensus statement relate to the patient journey when attending for lung function tests. We discuss appointment preparation, operational and environmental issues, testing room requirements including mitigation strategies for transmission risk, requirement for improved ventilation, maintaining physical distance, and use of personal protection equipment. We also provide consensus opinion on precautions relating to specific tests, filters, management of special patient groups, and alternative options to testing in hospitals. The pandemic has highlighted how vulnerable lung function services are and forces us to re-think how long term mitigation strategies can protect our services during this and any possible future pandemic. This statement aspires to address the safety concerns that exist and provide strategies to make lung function tests and the testing environment safer when tests are required.

Lower-limb muscle contractile properties, explosive power and the subjective response of elite soccer players to the Covid-19 lockdown

Abstract: The present study examined the effects of the lockdown period on basic anthropometric measures, countermovement jumping performance, skeletal muscle contractile properties derived from tensiomyography (TMG), injury incidence, and self-assessed general well-being in elite soccer players. A total of 266 players were assessed before (PRE) and 32 players were reassessed 11 days after (POST) the COVID-19 period. Significant changes in the TMG parameters were observed POST compared to PRE: contraction time (Tc) increased from 6% to 50% in vastus lateralis [VL] (p = 0.009) and biceps femoris [BF] (p < 0.001), respectively; whereas radial displacement (Dm) increased for 19% in BF (p = 0.036) and 17% in VL (p < 0.001), respectively. Jumping performance remained unchanged from PRE to POST In addition, athletes rated the lockdown period as a positive event and felt psychologically better during the lockdown, primarily because they spent more time with family members and friends. Although there were no differences in any of the variables describing lower limb muscle power following the two-month lockdown, the altered contractile properties of the assessed muscles suggest suboptimal conditioning of the football players.

COVID-19 related travel restrictions prevented numerous wildlife deaths on roads : ǂa ǂcomparative analysis of results from 11 countries

Abstract: Millions of wild animals are killed annually on roads worldwide. During spring 2020, the volume of road traffic was reduced globally as a consequence of the COVID-19 pandemic. We gathered data on wildlife-vehicle collisions (WVC) from Czechia, Estonia, Finland, Hungary, Israel, Norway, Slovenia, Spain, Sweden, and for Scotland and England within the United Kingdom. In all studied countries WVC statistics tend to be dominated by large mammals (various deer species and wild boar), while information on smaller mammals as well as birds are less well recorded. The expected number of WVC for 2020 was predicted on the basis of 2015%2019 WVC time series representing expected WVC numbers under normal traffic conditions. Then, the forecasted and reported WVC data were compared. The results indicate varying levels of WVC decrease between countries during the COVID-19 related traffic flow reduction (CRTR). While no significant change was determined in Sweden, where the state-wide response to COVID-19 was the least intensive, a decrease as marked as 37.4% was identified in Estonia. The greatest WVC decrease, more than 40%, was determined during the first weeks of CRTR for Estonia, Spain, Israel, and Czechia. Measures taken during spring 2020 allowed the survival of large numbers of wild animals which would have been killed under normal traffic conditions. The significant effects of even just a few weeks of reduced traffic, help to highlight the negative impacts of roads on wildlife mortality and the need to boost global efforts of wildlife conservation, including systematic gathering of roadkill data.

Mnenje za spremljanje bolnikov po preboleli covidni pljučnici

Abstract: Pljučnica je najpogostejši vzrok za težji potek okužbe z virusom SARS-CoV-2 in s hospitalizacijo. Potek covidne pljučnice je lahko različen; infiltrati, vidni na rentgenski sliki, se lahko resorbirajo spontano, včasih pa je potrebno zdravljenje s sistem-skimi glukokortikoidi. Ob odpustu iz bolnišnice zdravljenje običajno še ni končano, zato je Združenje pulmologov Slovenije v želji po enotnem obravnavanju bolnikov s covidno pljučnico izdelalo mnenje za obravnavo in sledenje bolnikov po od-pustu iz bolnišnice. Zavedamo se, da ob novi bolezni ne gre za dokončno priporočilo, saj bodo nova spoznanja o covidni pljučnici zanesljivo zahtevala obnavljanje priporočil.

Vpliv epidemije covida-19 na genetsko obravnavo Oddelka za onkološko klinično genetiko Onkološkega inštituta Ljubljana

Abstract: Izhodišče: Pandemija covida-19 je v začetku leta 2020 močno omajala temelje zdravstvenega sistema ne samo v Sloveniji, ampak tudi po svetu. Povsem nepričakovano nas je postavila pred izzive, ki so jih prinesli ukrepi za zajezitev te nalezljive bolezni. Z njimi smo se spoprijeli tudi zaposleni na Oddelku za onkološko klinično genetiko Onkološkega inštituta Ljubljana (OIL). Ob tem nam je bilo v pomoč učinkovito timsko sodelovanje v kolektivu. Namen: Ugotoviti vpliv epidemije covida-19 na število napotitev preiskovancev na onkološko genetsko svetovanje in testiranje, število opravljenih genetskih posvetov ter število prenaročitev in dobljene rezultate primerjati s podatki pred epidemijo ter ugotoviti razliko v povprečni starosti preiskovancev ob napotitvi v obdobju razglasitve prve in druge epidemije. Metode: Analiza podatkov o številu napotitev, opravljenih genetskih posvetov in številu prenaročanj je bila pripravljena na podlagi podatkovne zbirke Oddelka za onkološko klinično genetiko OIL z opisno statistiko. Rast opravljenih posvetov smo dokazovali z metodo bivariatne regresije, s t-testom pa primerjavo povprečne starosti preiskovancev ob napotitvi med prvo in drugo razglasitvijo epidemije. Rezultati: V obdobju prve razglasitve epidemije smo aprila 2020 imeli 78,8-odstotni upad rednih napotitev na genetsko svetovanje in testiranje glede na april 2019. Obenem smo v istem primerjalnem obdobju zabeležili 70,4-odstotni porast prednostnih napotitev. Starostna struktura napotenih preiskovancev se med obdobjema razglasitve epidemije ne razlikuje. V obdobju prve razglasitve epidemije smo opravili skupno 68 % manj genetskih posvetov kot v istem časovnem obdobju leto prej. V obdobju druge razglasitve epidemije smo opravili 12,7 % manj posvetov glede na enako časovno obdobje v letu pred epidemijo. Skupaj je bilo leta 2020 opravljenih 19,4 % genetskih posvetov manj kot leta 2019. V letu 2021 pa bomo napovedno opravili 21,4 % posvetov več kot v letu 2020. Ne glede na vztrajanje epidemije je število prvih posvetov v porastu. V prvem raziskovanem obdobju je bilo skupno prenaročenih 400 preiskovancev, od tega smo jih zaposleni prenaročili 227, kar znaša 55 %. V drugem raziskovanem obdobju je bilo 550 prenaročitev, zaposleni smo jih prenaročili 91, kar znaša 16 %. Več je bilo teh, ki so želeli nov termin ali pa se posveta niso udeležili brez predhodne odpovedi. Zaključek: Epidemija covida-19 je močno posegla na vsa področja zdravstvenega sistema, kjer smo se bili primorani spopasti z zajezitvenimi ukrepi. Na Oddelku za onkološko klinično genetiko smo v skladu z uradnimi odloki in priporočenimi ukrepi reorganizirali vsakdanjo prakso. Soočili smo se z novimi izzivi, kot so prenaročanje, telegenetskimi posveti, delo od doma in uvajanje novih kliničnih poti genetske obravnave. Kljub epidemiji se onkogenetska dejavnost v nasprotju z drugimi evropskimi državami ni ustavila ne v prvem ne v drugem valu epidemije. V letu 2021 beležimo celo porast tako napotovanja zaradi zdravljenja kot tudi števila opravljenih posvetov.

Učinkovitost (in varnost) cepljenja onkoloških bolnikov proti covidu-19 med aktivnim zdravljenjem

Abstract: Onkološki bolniki so bili že ob začetku pandemije covida-19 prepoznani kot skupina s povečanim tveganjem zapletov ob morebitni okužbi z virusom SARS-CoV-2, vendar so bili izključeni iz registracijskih raziskav cepiv proti covidu-19. Šele v zadnjih mesecih imamo na voljo več raziskav, ki so preučevale učinkovitost cepljenja pri onkoloških bolnikih z različnimi načini zdravljenja. Te so pokazale dobro učinkovitost (tako z razvojem humoralnega kot celičnega odgovora) in varnost cepiv predvsem pri bolnikih s solidnimi raki. Pričakovano slabši humoralni odziv je bil pri hematoloških bolnikih oziroma ga skoraj ni bilo pri bolnikih na terapiji anti-CD20 in terapiji z zaviralci Brutonove kinaze. Vendar je bil pri hematoloških bolnikih ohranjen celični odgovor in tako je smiselno cepljenje tudi pri tej skupini bolnikov, tudi s tretjim odmerkom. Pri bolnikih, ki so predhodno preboleli okužbo s SARS-CoV-2 ter bili nato cepljeni, so ugotavljali bistveno boljši humoralni odgovor kot pri cepljenju neprebolevnikov.
