Tehnologija Hyper Arc za izvedbo stereotaktične radiokirurgije v zdravljenju možganskih metastaz

Abstract: Hyper Arc je inovativna programska in strojna oprema za izvedbo stereotaktične radiokirurgije, ki je od junija 2020 dostopna na Onkološkem inštitutu Ljubljana. Omogoča visoko natančno, monoizocentrično obsevanje v teoriji neomejenega števila možganskih tarč. Najbolj uporabna je v lokalnem zdravljenju možganskih metastaz, ki so vedno pogostejši vzrok obolevnosti in umrljivosti bolnikov z malignimi boleznimi.Glede na najnovejše raziskave in smernice se stereotaktična radiokirurgija obravnava kot primarna izbira za izboljšanje preživetja in je lahko najboljša izbira zdravljenja več metastaz v možganih. Lokalna kontrola vsake obsevane lezije in morebitna pozna toksičnost sta vedno pomembnejšega kliničnega pomena, ki lahko vplivata ne le na preživetje, temveč tudi na nevrokognitivno funkcijo in kakovost življenja.

Environmental impacts and immobilization mechanisms of cadmium, lead and zinc in geotechnical composites made from contaminated soil and paper-ash

Abstract: Paper-ash is used for remediation of heavily contaminated soils with metals, but remediation efficiency after longer periods has not been reported. To gain insights into the mechanisms of immobilization of cadmium (Cd), lead (Pb), and znic (Zn), a study was performed in the laboratory experiment in uncontaminated, artificially contaminated, and remediated soils, and these soils treated with sulfate, to mimic conditions in contaminated soil from zinc smelter site. Remediation was performed by mixing contaminated soil with paper-ash to immobilize Cd, Pb, and Zn in the geotechnical composite. Partitioning of Cd, Pb, and Zn was studied over one year in seven-time intervals applying the sequential extraction procedure and complementary X-ray diffraction analyses. This methodological approach enabled us to follow the redistribution of Cd, Pb, and Zn over time, thus, to studying immobilization mechanisms and assessing the remediation efficiency and stability of newly formed mineral phases. Cd, Pb, and Zn were effectively immobilized by precipitation of insoluble hydroxides after the addition of paper-ash and by the carbonization process in insoluble carbonate minerals. After remediation, Cd, Pb, and Zn concentrations in the water-soluble fraction were well below the limiting values for inertness: Cd by 100 times, Pb by 125 times, and Zn by 10 times. Sulfate treatment did not influence the remediation efficiency. Experimental data confirmed the high remediation efficiency and stability of insoluble Cd, Pb, and Zn mineral phases in geotechnical composites.

Unfree wage labour

Abstract: The article develops the concept of ‘unfree wage labour’ that refers to a situation in which a worker must (temporarily and conditionally) give away possession of his labour power to a third party who then sells it to the buyers of labour power. From the perspective of contract freedom and legal equality, it seems irrelevant whether a worker sells his or her labour power to an employer or to an intermediary. However, there is no doubt that labour intermediation increases the economic dependence and social subordination of workers. First, given that the intermediary negotiates with the employer, the worker has a lower ability to influence the terms of employment. Second, given that he or she is a contract worker, they have a lower ability to engage in collective action in the company. Undoubtedly, this worker has significantly lower control over her or his working and living conditions. The question arises whether his or her contract freedom lessened to the extent that we can speak of unfree wage labour.

Environmental acceptability of geotechnical composites from recycled materials : comparative study of laboratory and field investigations

Abstract: The environmental properties of three geotechnical composites made by recycling wastes were investigated on a laboratory scale and in the field with the use of lysimeters designated for the revitalization of degraded mining sites. Composites were prepared by combining the mine waste with paper-mill sludge and foundry sand (Composite 1), with digestate from municipal waste and paper ash (Composite 2), and with coal ash, foundry slag and waste incineration bottom ash (Composite 3). The results of laboratory leaching tests proved that Composites 1 and 3 are envi- ronmentally acceptable, according to the legislative limits, as the potentially hazardous substances were immobilized, while in Composite 2, the legislative limits were exceeded. In the field lysimeters, the lowest rate of leaching was determined for optimally compacted Composites 1 and 3, while for Composite 2 the leaching of Cu was high. This study proved that optimally installed Composites 1 and 3 are environmentally acceptable for use in construction as an alternative to virgin materials, for the revitalization of degraded mining sites or, along with Composite 2, for closure operations with landfills. In this way, locally available waste streams are valorised and channelized into a beneficial and sustainable recycling practice.

Sluznični melanom : Mucosal melanoma

Abstract: Sluznični melanom je redka bolezen. Za njo najpogosteje zbolijo starejši ljudje, ženske bolj kot moški. Incidence ja relativno stabilna. Ob postavitvi diagnoze ima četrtina bolnikov s sluzničnim melanom napredovalo obliko bolezni, kar je posledica agresivnosti bolezni in ˝skritih˝ lezij s posledično zapoznelo diagnozo. 5-letno preživetje je najnižje v primerjavi z drugimi oblikami melanoma, če izključimo najredkejšo obliko melanoma, primarni leptomeningealni melanom. Biološke lastnosti sluzničnega melanoma se precej razlikujejo od lastnosti kožnega melanoma. Adjuvantno sistemsko zdravljenje je nujno potrebno, zaenkrat ni dovolj podatkov o njem. Kombinirana imunoterapija je terapija izbora zdravljenja metastatske oblike bolezni. Tarčna terapija ima svoje mesto le pri zdravljenju bolnikov z natančno določenimi tarčami (V600 Braf mutacija oz. cKIT mutacija).

Sluznični melanom : prikaz primera

Abstract: Sluznični melanom je redka bolezen. Najpogosteje se pojavi na sluznici področja glave in vratu, genitalnega trakta in anorektalni sluznici. Bolniki imajo ob diagnozi v večjem deležu prisotne regionalne ali oddaljene zasevke (najpogosteje v jetrih in pljučih). Sluznični melanom se od kožnega melanoma razlikuje v biologiji, kliničnem poteku in prognozi. Aktivirajoče mutacije gena c-KIT so prisotne pri večjem številu bolnikov s sluzničnim melanomom. V primeru prisotne c-KIT mutacije pride v poštev tarčno zdravljenje z imatinibom.

Leaching and geochemical modelling of an electric arc furnace (EAF) and ladle slag heap

Abstract: Old metallurgical dumps across Europe represent a loss of valuable land and a potential threat to the environment, especially to groundwater (GW). The Javornik electric arc furnace (EAF) and ladle slag heap, situated in Slovenia, was investigated in this study. The environmental impact of the slag heap was evaluated by combining leaching characterization tests of landfill samples and geochemical modelling. It was shown that throughout the landfill the same minerals and sorptive phases control the leaching of elements of potential concern, despite variations in chemical composi- tion. Although carbonation of the disposed steel slags occurred (molar ratio CO3/(Ca+Mg) = 0.53) relative to fresh slag, it had a limited effect on the leaching behaviour of elements of potential concern. The leaching from the slag heaps had also a limited effect on the quality of the GW. A site-specific case, however, was that leachates from the slag heap were strongly diluted, since a rapid flow of GW fed from the nearby Sava River was observed in the landfill area. The sampling and testing approach applied provides a basis for assessing the long-term impact of release and is a good starting point for evaluating future management options, including beneficial uses for this type of slag.

Residual compressive behavior of self-compacting concrete after high temperature exposure - influence of binder materials

Abstract: This paper presents an experimental investigation of the compressive behavior of highstrength self-compacting concrete exposed to temperatures up to 600 C. Ten different concrete compositions were tested, in which part of the cement (by weight) was replaced by three different mineral additives (5–15% metakaolin, 20–40% fly ash and 5–15% limestone). The stress–strain curves, compressive strength, modulus of elasticity and strain at peak stress were evaluated from uniaxial compression tests. Scanning electron microscope micrographs were also taken to evaluate the damage caused by the high temperatures. A sharp decrease in mechanical properties and an increase in peak strain were observed already after 200 C for all mixes tested. The different mineral additives used in this study affected the variations of residual compressive strength by 24% and peak strain by 38%, while the variations of residual modulus elasticity were 14%. Comparing the obtained results with the recommendations for compressive strength given in regulatory code EN 1992-1-2 for high strength concrete, it can be concluded that the strength loss observed in EN 1992-1-2 at temperatures up to 400 C is too conservative. The Popovics model for the relationship between stress and strain provided a good approximation for the experimentally determined stress–strain curves at different temperatures.
