Physicochemical and thermal properties of aluminosilicate gels based on metakaolin doped with different amounts of samarium(III)-oxide

Abstract: Aluminosilicate materials, known for their high strength, corrosion resistance, and thermal stability, are synthesized through the alkali activation of metakaolin, incorporating Sm2O3 to investigate the impact on their physicochemical properties. This study takes a look at the synthesis and physicochemical characterization of aluminosilicate gels doped with samarium(III)-oxide (Sm2O3), focusing on their potential as thermal insulators due to their enhanced thermal conductivity and absorption properties. Two samples with 1% and 5% Sm2O3 by weight were investigated, referred to as S1 and S2, respectively. Characterization techniques such as energy-dispersive X-ray fluorescence spectrometry (EDXRF), diffuse reflectance infrared Fourier transform spectroscopy (DRIFT), scanning electron microscopy (SEM), and photothermal beam deflection (PBD) were employed for the physicochemical characterization of aluminosilicate materials. The structural analysis shows an integration of Sm2O3, which did not significantly affect the gel’s density or porosity but enhanced its thermal conductivity. This study shows the potential of Sm2O3-doped aluminosilicates in applications requiring improved thermal management and stability, positioning them as potentially suitable materials for insulation.

Alkali-activated materials from diverse solid precursors : structural, mechanical and radiological properties

Abstract: This study investigates the gel characteristics of alkali-activated materials (AAMs) synthesized using wood ash (WA), and metakaolin (MK) as solid precursors. The research explores the influence of precursor type and sodium hydroxide (NaOH) concentrations in the alkali activator solution on the resulting physicochemical, microstructural, mechanical, and radiological properties of gels. The alkaline activators were prepared by mixing sodium hydroxide solutions (6 M and 12 M) with a sodium silicate (water glass) solution at a volume ratio of 1.5. The physicochemical characteristics of raw materials and AAMs were thoroughly analyzed using X-ray fluorescence (XRF), Diffuse Reflectance Infrared Fourier Transform (DRIFT) spectroscopy, X-ray diffraction (XRD), and scanning electron microscopy (SEM) with EDS elemental mapping. FTIR analysis confirmed the formation of an amorphous gels geopolymer network. XRD revealed the presence of characteristic crystalline phases (quartz, calcite) within an amorphous matrix. Mechanical properties, such as compressive strength, depended on precursor type and alkali molarity: metakaolin (12 M) reached ~14 MPa, while wood ash showed ~4 MPa (6 M) and ~0.5 MPa (12 M) due to high CaO, low Si and Al, and unfavorable SiO2/Al2O3 (5.71) and Na2O/Al2O3 (3.19) ratios. Furthermore, this research estimates radiological doses by quantifying radionuclide content via gamma-spectrometry. Alkali activation significantly reduced radiological haz- ard parameters, with radium equivalent activity (Raeq) decreasing to 238.0 Bq/kg and the external hazard index (Hex) to 0.643 for A12MK, while the annual effective dose rate for A12WA was only 0.265 nSv/y-all values remaining well below the recommended safety limit of 370 Bq/kg (≤1 mSv/y). The decrease in activity concentration index (Iγ), Raeq, and Hex with increasing NaOH concentration indicates effective radionuclide immobilization within the geopolymer matrix, confirming the suitability of these alkali-activated materials for safe use in construction from a radiation protection perspective.

Radiological characterization of alkali-activated material doped with Sm[sub]2O[sub]3 and its polymerization products

Abstract: The aim of this study was determination of radiological characterization of alkali-activated material with Sm2O3 and its polymerization products. Alkali-activated materials with 1 wt.% and 5 wt.% addition of Sm2O3 were synthesized and their natural radioactivity was determined. Energy dispersive X-ray fluorescence showed changes in the phase composition, or the formation of stable compounds, at higher temperatures. All samples demonstrated good pozzolanic activity, while the percentage of Sm2O3 was slightly changed. The X-ray photoelectron spectroscopy confirmed that the obtained material has a very low carbon content making it environmentally friendly, due to its low carbon content. A detailed analysis of the oxygen peak indicates variations in the stoichiometry of the oxides, which may affect the changes of natural radioactivity. Scanning electron microscopy confirmed that with the increase in the temperature of the thermal treatment, the opening of the pores in the alkali-activated material occurs, as well as further propagation of reaction that increased porosity and crystallization. Radiological measurement confirmed that examined alkali-activated material is safe for usage and exploitation. It should be emphasized that the presence of artificial radionuclide cesium 137Cs was not detected.

Mobocertinib in patients with EGFR exon 20 insertion-positive non-small cell lung cancer (MOON) : an international real-world safety and efficacy analysis

Abstract: EGFR exon 20 (EGFR Ex20) insertion mutations in non-small cell lung cancer (NSCLC) are insensitive to traditional EGFR tyrosine kinase inhibitors (TKIs). Mobocertinib is the only approved TKI specifically designed to target EGFR Ex20. We performed an international, real-world safety and efficacy analysis on patients with EGFR Ex20-positive NSCLC enrolled in a mobocertinib early access program. We explored the mechanisms of resistance by analyzing postprogression biopsies, as well as cross-resistance to amivantamab. Data from 86 patients with a median age of 67 years and a median of two prior lines of treatment were analyzed. Treatment-related adverse events (TRAEs) occurred in 95% of patients. Grade ≥3 TRAEs were reported in 38% of patients and included diarrhea (22%) and rash (8%). In 17% of patients, therapy was permanently discontinued, and two patients died due to TRAEs. Women were seven times more likely to discontinue treatment than men. In the overall cohort, the objective response rate to mobocertinib was 34% (95% CI, 24–45). The response rate in treatment-naïve patients was 27% (95% CI, 8–58). The median progression-free and overall survival was 5 months (95% CI, 3.5–6.5) and 12 months (95% CI, 6.8–17.2), respectively. The intracranial response rate was limited (13%), and one-third of disease progression cases involved the brain. Mobocertinib also showed antitumor activity following EGFR Ex20-specific therapy and vice versa. Potential mechanisms of resistance to mobocertinib included amplifications in MET, PIK3CA, and NRAS. Mobocertinib demonstrated meaningful efficacy in a real-world setting but was associated with considerable gastrointestinal and cutaneous toxicity.

Abstract: Leta 2019 so bila objavljena Priporočila za obravnavo bolnikov s pljučnim rakom, ki so v slovenski prostor vnesla prepotrebno poenotenje diagnostike in zdravljenja z namenom izboljšanja preživetja bolnikov s pljučnim rakom. Posodobitev Priporočil tri leta po izidu izvirnika prinaša največ novosti v poglavju o sistemskem zdravljenju bolnikov s pljučnim rakom. To kaže na izjemen napredek na področju razumevanja onkogeneze in biologije pljučnega raka ter s tem razvoja novih zdravil. Breme pljučnega raka ostaja veliko, saj je pljučni rak pri nas in v svetu še vedno najpogostejši vzrok smrti zaradi raka. Za vsako peto smrt zaradi raka je odgovoren pljučni rak. Skoraj tretjina bolnikov s pljučnim rakom ne prejme specifičnega onkološkega zdravljenja, bodisi zaradi slabega stanja zmogljivosti, spremljajočih bolezni ali obsega bolezni. Polovica bolnikov ima ob diagnozi razsejano bolezen, zaradi česar izboljšanje preživetja z malimi koraki sledi napredku v zdravljenju bolnikov s pljučnim rakom. Ti podatki nas opominjajo, da se bomo morali za velike premike v obravnavi bolnikov s pljučnim rakom lotiti drugačnih pristopov. Kot najbolj obetavno se ponuja zgodnje odkrivanje bolezni, ko so možnosti ozdravitve pljučnega raka najboljše. Zapisana Priporočila so usmeritev za obravnavo bolnikov s pljučnim rakom. Le s sodobnim multidisciplinarnim pristopom obravnave lahko bolniku ponudimo zdravljenje, ki mu omogoča najboljši izhod prognostično neugodne bolezni.

Solid cancer patients achieve adequate immunogenicity and low rate of severe adverse events after SARS-CoV-2 vaccination

Abstract: Background: SARS-CoV-2 vaccination in cancer patients is crucial to prevent severe COVID-19 disease course. Methods: This study assessed immunogenicity of cancer patients on active treatment receiving mRNA-based SARS-CoV-2 vaccine by detection of anti-SARS-CoV-2 S1 IgG antibodies in serum, before, after the first and second doses and 3 months after a complete primary course of vaccination. Results were compared with healthy controls. Results: Of 112 patients, the seroconversion rate was 96%. A significant reduction in antibody levels was observed 3 months after vaccination in patients receiving immune checkpoint inhibitors versus control participants (p < 0.001). Adverse events were mostly mild. Conclusion: Immunogenicity after mRNA-based vaccine in cancer patients is adequate but influenced by the type of anticancer therapy. Antibody levels decline after 3 months, and thus a third vaccination is warranted.

Immunotherapy for metastatic non-small cell lung cancer : real-world data from an academic Central and Eastern European center

Abstract: Background. Immunotherapy with immune checkpoint inhibitors (ICIs) recently became the standard treatment for patients with advanced non-small cell lung cancer (NSCLC). Here, we present the first results of a real-world observational study on the effectiveness of ICI monotherapy in patients with advanced NSCLC treated at a single academic center in a Central and Eastern European (CEE) country. Materials and methods. Overall, 66 consecutive patients with advanced NSCLC treated with ICIs in everyday clinical practice, either with first-line pembrolizumab (26 patients) or second-line atezolizumab, nivolumab, or pembrolizumab (40 patients), from August 2015 to November 2018, were included. All data were retrieved from a hospital lung cancer registry, in which the data is collected prospectively. Results. Included patients had a median age of 64 years, most were male (55%), 6% were in performance status >/=2, and 18% had controlled central nervous system metastases at baseline. In first-line, the median progression-free survival (mPFS) was 9.3 months, while the median overall survival (mOS) was not reached. The 1-year overall survival (OS) was 62%. In second-line, the mPFS and mOS were 3.5 months and 9.9 months, respectively, with a 1-year OS of 35%. In the overall population, adverse events of any grade were recorded in 79% of patients and of severe grade (3-4) in 12% of patients. Conclusion. The first real-world outcomes of NSCLC immunotherapy from a CEE country suggest comparable effectiveness to those observed in clinical trials and other real-world series, mainly coming from North America and Western European countries. Further data to inform on the real-world effectiveness of immunotherapy worldwide are needed.
