Psihoonkologija kot del celostne obravnave bolnikov z rakom

Abstract: Ko zdravstveni delavci stopamo v odnose z bolniki z rakom in njihovimi svojci v različnih obdobjih zdravljenja, se srečujemo tudi z njihovo stisko, ki jo opisujemo na kontinuumu – od pričakovane in prehodne stiske do stiske, ki je globoka, dolgotrajna ter ovira posameznikovo delovanje ter dosega kriterije duševne motnje. Z dobro komunikacijo, empatično podporo, obvladovanjem telesnih težav in informiranjem lahko pomembno podpremo bolnikove lastne vire moči za spoprijemanje z rakom. Prav tako je pomembna vloga prepoznavanja njihove stiske ter po potrebi uvedba zdravil in napotitev v ustrezno vrsto pomoči. Bolnik v hujši stiski ali z duševno motnjo lahko težje sodeluje v komunikaciji z zdravstvenim osebjem in se težje spoprijema s simptomi telesne bolezni in stranskimi učinki zdravljenja; njeno obvladovanje lahko posredno vpliva tudi na izhod zdravljenja maligne bolezni

Cognitive functioning in a cohort of high-grade glioma patients

Abstract: High grade gliomas are associated with cognitive problems. The aim of the study was to investigate cognitive functioning in a cohort of patients with high grade glioma, according to isocitrate dehydrogenase (IDH) and methyl guanine methyl transferase (MGMT) status and other clinical characteristics. The patients with the high-grade glioma treated in Slovenia in given period of time were included in study. Postoperatively they completed neuropsychological assessment consisting of Slovenian Verbal Learning Test, Slovenian Controlled Oral Word Association Test, Trail Making Test Part A and B and self-evaluation questionnaire. We analysed results (z-scores and dichotomized results) also according to IDH mutation and MGMT methylation. We examined differences between groups using T-test, Mann-Whitney U, χ2 and Kendall's Tau tests. Out of 275 patients in the cohort, we included 90. Forty-six percent of patients were unable to participate due to poor performance status and other conditions related to tumour. Patients with the IDH mutation were younger, with better performance status, larger proportions of grade III tumours and MGMT methylation. In this group cognitive functioning is significantly better in the domains of immediate recall, short delayed recall and delayed recall, and in the fields of executive functioning and recognition. There were no differences in cognitive functioning in regard to MGMT status. Grade III tumours were associated with more frequent MGMT methylation. Self-assessment proved week tool, associated only with immediate recall. We found no differences in cognitive functioning according to MGMT status, but cognition was better when IDH mutation was present. In a cohort study of patients with high-grade glioma, almost half were unable to participate in a study, which points to an overrepresentation of patients with better cognitive functioning in the research.

Experiencing professional strains of nurses, radiation engineers and physicians working at the Institute of oncology in Ljubljana

Abstract: Background. Since 1974 the term burnout is used in psychology. Burnout describes the end result of stress and has been described by Maslach comprising three basic components: emotional exhaustion, depersonalization andlow personal accomplishment. In this paper we would like to describe some aspects of burnout experiences of the employees of the Institute of Oncology in Ljubljana. Subjects and methods. We used Questionnaire of professional stress, created by Zunter Nagy and Kocmur. In our research 137 health workers from four professional groups participated: physicians, graduated nurses, nurses and radiation engineers, representing 38% of all employees. Results. Wefound out that in the experience the professional stressof all four professional groups is relatively equalized. The most prominent feelings are of fatigue, irritability and work overload. There were no signs of depersonalization - as described by Maslach - reported in our group. In nursesand in radiation engineers a distress is significantly more often displayed due to poorer personal income and poorer material status. Nurses reported significantly more often the intention to change work position (51%),institution (57%) or job (47%). Conclusions. Workstress impacts on the experience and on the thought patterns in those participating in the study. Wecan describe those signs as burnout signs. However, there are more new questions opening in the future as well as the need to a longitudinal approachto the research of this more and more prominent field.
