Testing of mechanisms of action of rituximab and clinical results in high-risk patients with aggressive CD20+ lymphoma

Povzetek: Background. Rituximab has been applied successfully in the treatment of indolent and aggressive CD20 positive B cell lymphomas, yet the exact in vivo mechanisms of its action have not been unambiguously explained. This study wastherefore aimed to confirm the presumed major mechanisms of action of rituximab and concomitantly to assess the effectiveness of first-line chemoimmunotherapy in high-risk patients with aggressive CD20 lymphomas. Patients, materials and methods. The activity of rituximab was tested in vitroon Raji and SU-DHL-4 cells using the cell proliferation assay and flow cytometry. In the clinical part of the study, 20 high-risk patients with aggressive CD 20 lymphomas were treated with R-CHOP. Results. Only complement-mediated cytotoxicity was observed under the in vitro applied experimental conditions. Neither the direct apoptotic effect nor the antibody-dependent cell-mediated cytotoxicity was detected probably due to a too low concentration of rituximab and a too low ratio of cytotoxic lymphocytes to tumor cells. The treatment outcome in patients was excellent since complete remissions were achieved in 90% of poor-risk patients at the end of primary treatment and 80% of patients were disease free at 18.5 months median observation period. Conclusions. According to our results, the complement-dependent cytotoxicity is an important mechanism of rituximab action in vitro. To achieve direct apoptosis, higher concentrations than 20 micro g/ml of rituximab should be used, while for an effective antibody-dependent cell-mediated cytotoxicity, the ratio of cytotoxic lymphocytes to tumor cells should be higher than 1:1. In the high- risk patients with aggressive CD20 lymphomas, the addition of rituximab to CHOP substantially improves the therapeutic results.

Surgical treatment of malignant pleural mesothelioma

Povzetek: Background. The aim of the study was to identify perioperative morbidity and mortality, the category and mode of adjuvant treatment, local recurrence and survival in patients treated by extrapleural pneumonectony (EPP) for malignantpleural mesothelioma (NLPM). Methods. From 2000 to 2003, 18 patients with MPM were referred to the Department of Thoracic Surgery in Ljubljana, and17 of them were operated on. Two patients underwent explorative thoracotomy, and 15 patients were evaluated. Five female and nine male patients (aged 52-68 years) were treated by EPP and one male patient by pleurectomy. Eight patients received both adjuvant chemotherapy (ChT) and radiotherapy (RT), with cisplatin 100 mg/m2 + mitomycin C 6-10 mg/m2 gemcitabine 1000 mg/m2 and external beam radiation with 24 Gy - 58 Gy respectively, three patients received no adjuvant therapy, three patients weretreated by adjuvant ChT, two of them were given cisplatin 100 mg/m2 + mitomycin C 6-10 mg/m2, and one patient cisplatin 100 mg/m2 on the first day and gemcitabine 250 mg/rn2 in prolonged 6 hours infusion on the first and on the eighth day. One patient was treated only by adjuvant RT. Results. There were no perioperative deaths and the postoperative morbidity was 42%. Of the 15 evaluable patients, and in the median follow up of 40 months (28-64), we noticed nine (60.0%) recurrences, seven local and two abdominal. Eight (53.3%)patients died, all because of the local progress of disease. (Abstract truncated at 2000 characters)

Rituximab affects the prognosis of patients with nonHodgkin's lymphomas

Povzetek: Backround. Rituximab - the most widely used monoclonal antibody in the B cell lymphoid malignancies has been applied successfully in the treatment of relapsed and refractory indolent CD20 positive B cell lymphomas and more recently, also in the treatment of aggressive lymphomas in combination with standard chemotherapy. Albeit the chemo-immunotherapy has a wide range of potential applications, there are still several issues that have to be resolved: (1) the optimal scheduling of antibody-chemotherapy combinations, (2) the most active of these combinations, as well as (3) the predictors of response to rituximab. Patients and methods. To facilitate addressing the first two questions, we performed an analysis in 25 patients with different histological types of CD20 positive nonHodgkin's lymphomas (10 aggressive and 15 indolent). Seventeen patients were treated with chemo-immunotherapy for a relapse, and just in 8 patients rituximab was added to first line chemotherapy. Most of the responders received the CHOP regimen, but also otherregimens (FC, BVCPP) were effective in combination with rituximab. Results. The overall response rate was 76%, with 68% complete remissions. The median response duration has not been reached yet. The response was markedly better in the group of previously untreated patients, where the overall response rate reached 100%, with 7 patients in complete and 1 patient in partial remission. Most of the treatment failures occurred in heavily pretreated patients with aggressive lymphomas. No serious adverse effects wereobserved. Conclusion The chemo-immunotherapy improves the treatment outcomes in patients with untreated and relapsed CD20 positive nonHodgkin's lymphomas in comparison to chemotherapy alone. The combined treatment is the most effective when used as soon as possible (preferably as the first line treatment). (Abstract truncated at 2000 characters).

Ten years of the journal Radiology and oncology: some bibliometric evaluation

Povzetek: Background. Bibliometrics and its methods are a useful set of tools for analysing a scientific journal's relative position in the field. By measuring different quantitative data and comparing them with other journals in the field, certain decisions can be made as to the future of the journal. Objectives and methods. We thought as appropriate to take last ten years of Radiology and Oncology (1992-2001) and put that content to double scrutiny: first, by applying various guantitative measurements to the journal's content to get a more objective picture of the whole and of its development in the past ten years; then by additionally comparing it to another international journal from the field and of similar orientation, Neoplasma, to illustrate ifdifferences and/or similarities between the two are in favour of or detriment to Radiology and Oncology. Results and conclusion. Results show thatRadiology and Oncology has been progressing in the right direction, but that extra efforts should be made by the editors and the editorial board to attract more articles per issue and to gradually increase the share of experimental articles to boost its impact in the field. Also, to improve its visibility, editors, reviewers and also authors that publish in Radiology and Oncology could consider citing the articles published in this journal, in the articles published elsewhere, when appropriate.

Renal cell carcinoma in the ectopic kidney prospects of diagnosting and treatment of the carcinoma of the kidney: case report

Povzetek: Background. An extensive use of ultrasonography and computed tomography have enormously contributed to the early detection of adenocarcinoma of kidney taking into consideration that they have been so far often detected by chance.In addition to provide us with an image of a tumor and contributing to define more easily the nature of tumor, MRI is most helpful in detecting the infiltrations of tumor into its surroundings and changes in the veins. Case report. This case report presents the patient with adenocarcinoma of the ectopic kidney. The ultrasonography, computed tomography and magnetic resonance imaging detected and abundant non-homogenous tumor mass in pelvis minor. Additionally to the suspected ectopia of one of the kidney, it was alsosuspected that there was another pathological process of different etiology too. This suspicion was based on the detection of a different tumor mass in between the intestinal loops. Two months after the nonradical excisionof the adenocarcinoma from the ectopic kidney, the following changes were observed: progress of the residual tumor in the pelvis minor and the tumor in between the intestinal loops (that was not removed during the first operation). The patient was given immunochemotherapy and local radiotherapy. Conclusions. According to the available data in literature, the localization of tumor in ectopic kidney is extremely rare.

Plasma membrane fluidity alterations in cancerous tissues

Povzetek: Plasma membrane is a heterogeneous structure with several coexisting domains having different fluidity characteristics. It plays an important role in the control of cell growth, differentiation and transformation. Fluidity of the whole plasma membrane reflects the ordering and dynamics of phospholipid acyl chains in specific membrane domains, as well as the fraction of each domain inthe membrane. Changes in the membrane fluidity affect processes on the membrane such as transport, enzyme activities and expression of the redeptors.In this paper we present results of our recent electron paramagneticresonance (EPR) studies of plasma membrane fluidity characteristics, which take into account heterogeneous nature of the plasma membrane, By the computer simulation of the EPR spectra line-shapes, the number of coexisting domains in the plasma membrane, their relative portion inthe membrane as well as the ordering and dynamics of each domain be determined. Therefore, we could distinguish the contribution of the relative portion of each domain from the contribution of fulidity alterations in the domain to the entire fluidity changes in the membrane. Two examples will be discussed: membrane fluidity characteristics of excised lung tumor tissues andinfluence of microtubule depolymerizing agent vinblastine on membrane fluidity of vinblastine sensitive and resistant HeLa cells.

Cytology of mediastinal tumors

Povzetek: Our experience with cytological examinations of tumorous mediastinal lesions is evaluated. A group of 117 patients with mediastinal tumor have been included into the study. Among them carcinomas prevailed (60,7%), followed by lymphomas (18,8%), other tumors (15.4%) and thymic neoplasms (5.1%). Malignantor suspicious cells were found in 77.4% of patients with carcinoma. The cells indicating a possibility of non-Hodgkin's lymphoma were found in 9 out of 14 patients. In 5 out of 6 thymic neoplasms the cytological pattern wasconsistent with the diagnosis of thymic neoplasm. One case of thymoma was cytologically falsely diagnosed as malignant lymphoma. One case of neurofibroma was falsely diagnosed as adenocarcinoma. The sensitivity of cytological examinations was 67.5%. If 18 patients with diagnostically unsatisfactory material were excluded from the analysis, the sensitivity wouldincrease to 80.8%. Owing to the wide variety of primary and metastatic tumors that can occur in the mediastinum, apart from the routine cytological techniques, additional staining methods should be used. For final cytological diagnosis the integration of cytological findings with clinical and radiological data is often required. Owing to the characteristics of the obtained material and biological behaviour of some mediastinal tumors, some tumors cannot be definitively diagnosed by cytological examinations alone.
